WASHINGTON, Nov 19 (Bernama) -- Children with psoriasis are about twice as likely to be overweight or obese as children who don't have the disease, according to an international study published Monday in Archives of Dermatology.
And US children with psoriasis have much higher odds than psoriatic children in other countries of being obese or overweight, the study also shows.
Quoting the study, China's Xinhua news agency said that within the US population, Hispanics and African American children had significantly greater rates of being obese and overweight than whites and Asians. The odds ratio of being obese were particularly high for US children with more severe psoriasis.
"There has been increasing attention to the association of psoriasis and metabolic risks in adults, but this study shows that the association of being overweight or obese and having psoriasis may be even higher in affected children than in adults," said Amy Paller, chair of dermatology at Northwestern University Feinberg School of Medicine and lead author of the study.
"This is incredibly significant," Paller said. "It tells us these children are at increased risk of health complications ( cardiac risk and insulin resistance) and need to be watched closely and treated."
Psoriasis is characterised by red, often itchy scaling lesions that can be localised (especially to the scalp, knees and elbows) or cover the entire body. The disease affects 2.5 to 3.5 percent of the global population with up to 33 percent of the cases starting in childhood, particularly during adolescence.

CHICAGO --- The largest study of physician-treated children with psoriasis around the world shows children with the skin disease are about twice as likely to be overweight or obese as children who don't have the disease, according to new Northwestern Medicine research.
And US children with psoriasis have much higher odds than psoriatic children in other countries of being obese or overweight.
In the US, children with psoriasis had four times the odds of being overweight or obese as healthy controls. Within this US population, Hispanics and African American children had significantly greater rates of being obese and overweight than whites and Asians. The odds ratio of being obese were particularly high for U.S. children with more severe psoriasis (7.6).
"There has been increasing attention to the association of psoriasis and metabolic risks in adults, but this study shows that the association of being overweight or obese and having psoriasis may be even higher in affected children than in adults," said Amy Paller, M.D., chair of dermatology at Northwestern University Feinberg School of Medicine and lead author of the study. She also is a pediatric dermatologist at the Ann and Robert H. Lurie Children's Hospital of Chicago.
The paper will be published Nov. 19 in Archives of Dermatology

The study included 614 children five to 17 years old from nine countries in the Americas, Europe and Asia.
Previous studies showing an association between psoriasis and obesity were largely based on registry data, which is less precise and may under identify cases because of the reliance on diagnosis codes rather than a direct examination and measurements to calculate Body Mass Index.

Researchers from Northwestern University recently revealed that children who have psoriasis, a common skin condition that leads to irritation and redness, also have double the chance of becoming overweight or obese.
In particular, the study, considered the largest project to look at children with psoriasis, found that kids with the skin disease in the United States had a higher likelihood (four times the odds to be exact) of becoming overweight or obese as compared to children in other countries. In addition those of Hispanic or African American descent had higher rates of developing obesity as compared to their counterparts who were of white or Asian ethnic background.
There has been increasing attention to the association of psoriasis and metabolic risks in adults, but this study shows that the association of being overweight or obese and having psoriasis may be even higher in affected children than in adults, explained the studys lead author Dr. Amy Paller, the hair of dermatology at Northwestern University Feinberg School of Medicine, in a prepared statement.
Adults with psoriasis have an increased risk of obesity, myocardial infarction, stroke and diabetes mellitus, wrote the authors in the journal article. Recent studies also suggest the association of psoriasis with obesity in children.
615 children between five and seven years of age from nine different nations in Asia, the Americas, and Europe participated in the study. Of the participants who were diagnosed with psoriasis, 30 percent reported that they had a family member who also had the skin disease. As such, researchers believe that genetics could be involved; the genetic issue could be related to metabolism disorders as psoriasis has been linked to the same protein that stimulates insulin resistance and causes people to become overweight.
Perhaps our U.S. environment of eating more calories and getting less exercise along with a strong genetic component for the disease hikes the risk, continued Paller, who also serves as pediatric dermatologist at the Ann and Robert H. Lurie Childrens Hospital of Chicago, in the statement. With psoriasis, those affected have localized red, itchy lesions on the elbows, knees, and scalp, or all over the body.
Its a highly visible disease which can be devastating during the all-important years of psychosocial development of childhood and adolescence, commented Paller in the statement. It affects their social life and even their ability to participate in sports. Psoriasis has a profound effect on childrens quality of life.

CHICAGO A study of children in nine countries found that psoriasis in children was associated with an increased risk for overweight and obesity, regardless of psoriasis severity, according to a report published Online First by Archives of Dermatology, a JAMA Network publication.
"Adults with psoriasis have an increased risk of obesity, myocardial infarction, stroke and diabetes mellitus," the authors write as background in the study. "Recent studies also suggest the association of psoriasis with obesity in children."
Amy S. Paller, M.D., with Northwestern University, Chicago, and colleagues examined the association between excess adiposity (body mass index [BMI] percentile) and central adiposity (waist circumference percentile and waist to height ratio) with pediatric psoriasis severity. The authors conducted a multicenter, cross-sectional study of 409 children with psoriasis and 205 control children from nine countries between June 19, 2009 and December 2, 2011.
Psoriasis was classified as mild (worst Physician's Global Assessment score 3 with body surface area 10 percent) or severe (worst Physician's Global Assessment score 3 with body surface area >10 percent).
Excess adiposity (BMI 85th percentile) occurred in 37.9 percent of children with psoriasis (n=155) vs. 20.5 percent of children in the control group (n=42), but did not differ by psoriasis severity. Waist circumference above the 90th percentile occurred in 9.3 percent of children in the control group (n=19), 14 percent in the mild psoriasis group (n=27) and 21.2 percent in the severe psoriasis group (n=43), internationally.
Additionally, waist to height ratio was significantly higher in children with psoriasis (0.48) vs. those in the control group (0.46) but was unaffected by psoriasis severity. Children with severe psoriasis at its worst, but mild psoriasis at enrollment, showed no significant difference in excess or central adiposity than children whose psoriasis remained severe.
"In conclusion, children with psoriasis internationally, regardless of severity, are more likely to be overweight or obese and thus are at increased risk for complications related to excess adiposity," the authors write. "Should further studies show excess adiposity to be a precursor for psoriasis, attempts at early weight loss and lifestyle modification will be important, not only to decrease the risk of metabolic disease but also to modulate the course of pediatric psoriasis."
