Secukinumab Retention High in Real-world Study of Patients With PsA, AxSpA – AJMC.com Managed Markets Network

Corresponding author Rubn Queiro, MD, PhD, of the Central University Hospital of Asturias, in Spain, and colleagues explained that secukinumab (SEC), a human monoclonal antibody targeting interleukin 17A, had a favorable safety profile in clinical trials. In addition, postmarketing surveillance has shown the therapy has a low frequency of discontinuation.
However, we currently have no consistent data on long-term survival of SEC in patients with AxSpA and PsA in routine clinical practice, the investigators wrote.
They therefore performed a multicenter retrospective observational study of patients with AxSpa or PsA who were prescribed and received at least 1 dose of SEC. The primary end points were AEs and drug retention rates.
Altogether, 154 patients were included in the study: 59 had PsA and the remaining 95 patients had AxSpA. The overall disease duration was 6.5 years, and most of the enrollees (61%) had previously been treated with at least 2 biologics before being prescribed SEC.
After 1 year, the data showed that 66% of patients were still taking SEC. However, by the end of the second year, the retention rate was 43%. Those who quit primarily stopped taking SEC due to lack of efficacy (59%), and 36% said they stopped the therapy because of AEs. Nearly three-quarters (71%) of patients who quit the therapy due to AEs did so in the first 6 months of treatment.
The most common AEs were gastrointestinal (GI) disorders, which occurred in 6 patients and included symptoms such as nausea, vomiting, and abdominal pain. There were 2 cases of Crohn disease. All 6 patients with GI AEs withdrew, as did 4 of 5 patients who reported infections. Five patients had disorders of the skin and subcutaneous tissue, and all 5 withdrew as well.
Queiro and colleagues found a number of factors that made a patient less likely to discontinue SEC, and these included male gender, obesity, hypertension, and diabetes. The investigators said the finding that obesity was linked with better drug survival was interesting, since previous reports have suggested patients who are obese and who have psoriasis tend to have a poor therapeutic response to SEC.
However, we found that obesity, and other components of metabolic syndrome, were predictors of longer survival for SEC therapy, they wrote. Therefore, SEC could be a good therapeutic choice in obese patients with AxSpA and PsA as opposed to [tumor necrosis factor inhibitor] agents.
Factors associated with discontinuation included number of previous biologics and depression.
Queiro and colleagues acknowledged some limitations, including the relatively small sample size and the retrospective nature of the study. They said their findings ought to be replicated in larger cohorts, but that they still show SEC appears to be performing well in clinical practice.
Treatment persistence has been optimal even in third line treatment, independent of underlying disease, and obesity does not seem a marker of poor treatment response, they concluded.

Study States That Diet Rich in Sugar and Fat Contributes to Skin & Joint Inflammation – Bel Marra Health

New research shows that switching from a Western diet to a balanced diet could help you reduce skin and joint inflammation. The study led by UC Davis Health researchers states that the reduction may be due to a better-balanced gut microbial culture.
Published in the Journal of Investigative Dermatology, the study suggests that cutting out sugar and fat and consuming a more balanced diet can help you restore gut health, which can suppress skin inflammation that may otherwise contribute to inflammatory skin diseases such as psoriasis.
Psoriasis is a chronic skin condition linked to the bodys immune system. Healthy skin cells are attacked mistakenly by immune cells, which cause skin inflammation and the formation of scales and itchy red patches.
Approximately 30% of patients with psoriasis also have psoriatic arthritis, which causes morning stiffness and fatigue, pain in joints, swollen fingers and toes, and changes to nails.
Previous studies have shown how diet can influence psoriasis through microbial balance in the intestines and skin inflammation. Consuming a traditional Western diet can cause a rapid change to the guts microbial community and its functions. This disruption, called dysbiosis, can contribute to gut inflammation.
For the new study, researchers wanted to test whether intestinal dysbiosis affects skin and joint inflammation. A mouse model was used to study the effects of diet on psoriasis and psoriatic arthritis. The mice were injected with Interleukin-23 (IL-23) minicircle DNA to induce a response mimicking psoriasis-like skin and joint diseases.
IL-23 is a protein created by the immune cells responsible for many inflammatory autoimmune reactions. This includes psoriasis and inflammatory bowel disease (IBD).
Researchers found that a short-term Western diet appears sufficient to cause microbial imbalance and to enhance susceptibility to IL-23 proteins that can lead to skin inflammation.
There is a clear link between skin inflammation and changes in the gut microbiome due to food intake, said Sam T. Hwang, study author. The bacterial balance in the gut disrupted shortly after starting a Western diet, and worsened psoriatic skin and joint inflammation.
The study went on to show that eating a diet high in sugar and fat is a major contributor to skin and joint inflammation. However, it can be changed by switching to a more balanced diet. The improvement in skin inflammation seen in mice taken off the Western diet indicated a short-term impact of the Western diet on skin inflammation.
Researchers concluded the study by suggesting that changes in diet could partially reverse the proinflammatory effects and alteration of gut microbiota caused by the Western diet.

Transhumanism and future of humanity towards digital slavery | Daily Sabah – Daily Sabah

In the late 19th century, a society called X-Club was founded by a group of scientists in England. The "X" in the title of the club, where Charles Darwin gave lectures from time to time, symbolized change and evolution according to his esoteric beliefs. The most famous members of the club, also known as X-men, were social Darwinist Herbert Spencer and biologist Thomas Henry Huxley. In fact, Huxley was the one who asked Darwin to pen the theory of evolution he had shaped in his mind for years.
Along with the theory of evolution, Darwin introduced the notion of kabbalah that the human mind would develop into a god as a result of evolution to the world of science and he built the infrastructure of this belief among scientists. The club, supported by capitalists with very strong connections, chose the scientists of the time who believed in creation and ensured that the theory of evolution was accepted by the public. The theory of evolution began to appear in textbooks in England. Huxley was the member who demonstrated the most effort in the lectures and conferences he gave.
Inspired by Darwin and examinations of his ideas, German philosopher Friedrich Nietzsche introduced the idea of bermensch, or superman, taking humanity to the next stage in evolution in the eugenic sense. Hitler, who was a fan of Nietzsche and saw himself as a superman, began to work toward creating a superior race. The scientists who carried out these studies in Germany moved to the U.S. following World War II and established the Cybernetics Group under the control of the Central Intelligence Agency (CIA). Research would no longer be focused on race but on medicine and technology.
Big brother
The Fabian Society in England wanted to establish the socialist New World Order under the control of the capitalists. However, they did not want to do it the same way as in Soviet Russia; instead, they sought to carry out revolutions using an evolutionist approach. The members of the society did not remove Nietzsche's works from their bedside. This society, which involved Huxley's science fiction student H. G. Wells, included two famous writers. These writers, who put forward two dystopias, that is two visions of the future of the world, were George Orwell and Huxley's grandson Aldous Huxley. In both Orwell's 1984 and Huxley's Brave New World, the world is ruled by a certain party, a certain class. The concept of family disappears and a socialist order dominates society. The main difference between the two novels is that people in 1984 are forcibly enslaved by the party while the characters love slavery in Brave New World. Huxley not only wrote the novel but also went to the U.S. and participated in the work to create a superman with the team there.
While Aldous Huxley was conducting research on LSD to control the human mind in the U.S., his brother Julian Huxley introduced the term transhumanism to the literature in his book Religion Without Revelation. Transhumanism (humanity+) is a term used to describe people going beyond their current physical and mental limits through science and technology. According to transhumanists, since human evolution had stopped, it was necessary to push it forward using machines and drugs. One of Julian's favorite friends, who was also the first UNESCO director, was the Jesuit priest Pierre Teilhard de Chardin. Teilhard was known as the Catholic Darwin. He was a leading actor in the Piltdown Man fraud, in which the human skull and orangutan jaw were combined to demonstrate the transitional form in evolution, a development that went down in history as a scandal. De Chardin believed that the minds of all people would unite at a higher level of consciousness, which he called the "noosphere," meaning "mind sphere."
Internet
Studies in the U.S. bore fruit in a short time. Personal computers brought the internet into our lives and the ubiquitous "www," which can read as three waws, meaning 666 in Hebrew (and according to the abjad), which is also the sign of the Antichrist according to the Bible. When a sufficient level of knowledge and technology is reached, the plan is for Big Data to turn into Big Brother thanks to artificial intelligence (AI), the god of transhumanists that will take its place at the top of the pyramid and take control from human hands.
Transhumanists
Although they have esoteric beliefs, transhumanists do not believe in the soul and instead see the human brain as a machine. They believe immortality will be possible by transferring the human mind to servers or other bodies/machines. There are agnostics in this community, as well as atheists like Max More, the head of the Alcor society, who deals with cryonics in order to resurrect people in the future through technology. In his article entitled "In Praise of the Devil," More says: "Lucifer means light-bringer ... The story is that God threw Lucifer out of heaven because Lucifer had started to question God ... Former X employee Mary Lou Jepsen is striving to develop an MRI machine that can be worn as a beanie and read minds; Elon Musk, the owner of SpaceX, is working on a Neuralink project, in which chips and electrodes will be added to the brain and body to connect us to the network like computers. DARPA research and development agency, which serves the U.S. Army, is experimenting with cyborg soldiers with chips in their brains. The X Prize Foundation of Transhumanists also organizes competitions in the hopes of reaching the New World as soon as possible.
Brave new world order
We have stepped into the new world with new advents like robots, genetic engineering, virtual reality (VR), brain and space exploration, nanotechnology, artificial intelligence, cybernetics and synthetic biology to name a few developments. Esoterics, who were unable to evolve spiritually due to being on the wrong path, are realizing it physically and synthetically through technology, almost like a magician. Well, what will we do when the day comes that a chip is implanted into our brain like in "The Matrix," or when drugs in the movies Lucy and Limitless are introduced into the market? Will we resist or will we form queues in front of stores like when a new phone model comes out? I belive the second possibility will become a reality. After all, we live in a Brave New World, one where we are unwilling to give up our phones and internet, even after reading this article, and slavery is relished. As Elon Musk said, we have already turned into cyborgs thanks to computers and smartphones, which we cannot drop or operate without. All that remains is to install these technologies beneath the skin's surface.

WITH the expected release this month of the United States Department of Defenses report on unidentified aerial phenomenon, or UAPs, the question of intelligent life beyond Earths solar system is back in the limelight.
The long-sought answer to Are we alone in the universe? has been contemplated by many, especially those who are curious about its scientific and theological implications.
In an interview in 2015, several months after the discovery of Kepler-452b a so-called super-Earth located approximately 1,400 light years away in the habitable zone of its star Pope Francis was asked for his thoughts on the possibility of intelligent life existing on other planets.
Honestly, I wouldnt know how to answer, he told French news magazine, Paris Match.
Until America was discovered, we thought it didnt exist, and instead it existed.
But in every case I think that we should stick to what the scientists tell us, still aware that the Creator is infinitely greater than our knowledge.
While science and religion were often pitted against each other as separate and irreconcilable camps, St John Paul II saw the benefit of a synergistic relationship that could lead humanity toward a greater understanding of the unknown.
Science can purify religion from error and superstition; religion can purify science from idolatry and false absolutes, he wrote in a 1988 letter to the late Jesuit Father George Coyne, former director of the Vatican Observatory.
Each can draw the other into a wider world, a world in which both can flourish.
This explains why so many Catholic scientists are active in the study of the possibility of intelligent extraterrestrial life.
Among them is Jesuit Father Jos Gabriel Funes, an astronomer and former director of the Vatican Observatory, who leads Project OTHER, a Spanish acronym that stands for Otros mundos, Tierra, Humanidad y Espacio Remoto (Other worlds, Earth, Humanity and Remote Space).
Fr Funes warned that while the subject of intelligent extraterrestrial life could spur interesting and exciting conversations, it also could veer into the realm of conspiracy theories that departed from true science.
We need to address the topic in a professional way. And by professional, I mean in an academic way, Fr Funes said.
Project OTHER, he said, brings together astronomers, biologists, philosophers and theologians at the Catholic University of Cordoba to not only study the possible existence of intelligent extraterrestrial life, but also the impact its potential discovery could have on the scientific, philosophical and religious comprehension of humanity.
Their possible existence raises religious questions, including questions involving the mystery of the Incarnation, in which the Word assumed human nature and thus, as described by the Catechism of the Catholic Church, Jesus is both true God and true man.
Whether the Incarnation is exclusive to Earth or repeated in other planets with intelligent life, Fr Funes said, has been pondered for decades by theologians.
Nevertheless, Fr Funes said, the incarnation of Christ was and remains a unique event.
Im not a theologian, but my conclusion is that one Incarnation is more than enough, he said.
We dont need to complicate things more than they are.
Its already difficult to understand one Incarnation, but this is my way of thinking.
Dominican Fr Thomas OMeara, a retired theology professor at the University of Notre Dame and author of Vast Universe: Extraterrestrials and Christian Revelation, echoed Fr Funes sentiment on the Incarnation, arguing that the Incarnation doesnt require that Jesus also be incarnate on other planets.
Regarding the existence of extraterrestrial life, Fr OMeara said the fact there are billions of planets increases the likelihood of planets with both life and intelligent life.
That belief, he added, would probably be supported by one of the churchs greatest scholars St Thomas Aquinas.
(St) Thomas Aquinas view of the world is that God has made a world that is quite vast and quite diverse, Fr OMeara said.
Now, of course, he had no idea how vast and diverse it was, but thats what he sees just from plants and flowers and fish and stars and things like that.
And he thinks that because the point of the universe is to show the richness and diversity of God.
The belief in the likelihood of extraterrestrial life is also shared by nonbelievers as well, including Anders Sandberg, senior research fellow at the Future of Humanity Institute at the University of Oxford.
Dr Sandberg said it is easy to believe that the existence of life on Earth is just a one-time miracle.
However, much like Fr OMeara, the Swedish transhumanist researcher said the probability that earthlings were alone in the cosmos was low.
Were probably not alone because the universe looks like its actually infinite, he said.
We havent found any evidence that there is any kind of edge on it or that its curving together.
However, with many seeing the upcoming Pentagon report as proof of alien life, Dr Sandberg told CNS that to immediately base a conclusion on inconclusive evidence, such as blurry images, is a mistake, and that the constant flood of information regarding unexplained phenomenon should be taken with a bit of caution.
The world is large, so one-in-a-trillion chances happen monthly and are reported globally, he said.
Instead of assuming that a weird blob in the sky was evidence of alien life, Dr Sandberg suggested people instead maybe say, There are more weird things in the world than I expected.
Intellectual humility is a very useful thing, he said.

First Patient Injected With Cancer Vaccine in Phase II Trial – Futurism

The pharmaceutical company BioNTech has officially begun injecting participants with an experimental cancer vaccine in a new phase II clinical trial.
Preclinical and earlier clinical data showed that the vaccine is safe enough to progress along the clinical research pipeline, according to Clinical Trials Arena. So now, BioNTech plans to measure how well the vaccine, in concert with the medication Libtayo, works as a treatment for severe cases of skin cancer and thats a big deal, because the hopeis that it will be able to treat patients who would have previously been considered terminal.
Just like the COVID-19 vaccine that BioNTech developed in collaboration with Pfizer, the experimental cancer vaccine relies on mRNA technology to teach a patients immune system how to fight off cancer. But there are some key differences between testing a vaccine for cancer and a highly-infectious virus.
Namely, it wouldnt make sense to test a vaccine on healthy patients and then wait around to see if they happen to eventually develop severe cancer. Rather, BioNTech is recruiting existing patients with stage three or four melanoma and then tracking their progress after theyre inoculated.

We were able to demonstrate the potential of mRNA vaccines in addressing COVID-19. We must not forget, that cancer is also a global health threat, even worse than the current pandemic.

A draft climate change report from the United Nations Intergovernmental Panel on Climate Change (IPCC) issues a dire warning that we are on the cusp of planetary destruction thanks to the myriad dangers of worsening climate change.
The report, which was leaked to Agence France-Presse, is 4,000 pages long but its central message can be summarized very clearly: Humanity has screwed up the planet, and well be reaping what weve sown very, very soon.
Specifically, the report predicts even more mass extinction, dangerously high temperatures, increased disease outbreaks, rising sea levels, and the collapse of entire ecosystems within the next few decades.
The worst is yet to come, reads the report, affecting our childrens and grandchildrens lives much more than our own.
The report is slated for release next February, which AFP notes is too late for it to influence any of the upcoming UN summits on climate change, biodiversity, and food security. Thats too bad, because it lays out the very real possibility that humanitys time as stewards of the planet could be coming to an end.
Life on Earth can recover from a drastic climate shift by evolving into new species and creating new ecosystems, the report says. Humans cannot.
While some countries have made progress toward climate goals like the 2015 Paris Agreement, the report suggests that the wheels of climate-related devastation have already been set in motion. To slow them down, the entire world will need to take aggressive and immediate action on all fronts, lest we pass a point of no return.
We need transformational change operating on processes and behaviors at all levels: individual, communities, business, institutions, and governments, the report says. We must redefine our way of life and consumption.

A video shared by aerospace reporter Andrew Jones on Twitter shows a massive rocket booster, reportedly part of Chinas Long March 2F rocket that launched three astronauts into orbit on June 16, lying in the middle of the field, leaking what appears to be nitrogen tetroxide.
According to a post on Chinese social medianetwork Weibo, the piece landed in the Ordos, a highland basin in northwest China. Local roads were closed and people were evacuated to safe areas, according to the post.
While we cant confirm the legitimacy of the video, it wouldnt be the first time a Chinese rocket took off to deliver crews or cargo to orbit and subsequently make an uncontrolled return to the surface below.
TheJune 16 launch was largely successful, allowing three astronauts to travel to the first module of the countrys brand new space station called Tiangong, or Heavenly Palace, where theyre expected to stay several months.
Just last month, the main core stage of the Long March 5B heavy lift rocket, used to deliver the module into orbit on April 29, came crashing back downto Earth. Fortunately, nobody was hurt.
Other rocket companies, like SpaceX, have engineered their rocket boosters to make a controlled landing, allowing them to be reused.
Despite the fact that the rocket managed to avoid making impact with any inhabited areas, experts are worried future launches might end in a disaster.
There is a real chance of damage to whatever it hits and the outside chance of a casualty, Jonathan McDowell, an astrophysicist at the HarvardSmithsonian Center for Astrophysics, told Agence France-Presse.

NASA is still trying to resuscitate the iconic observatory.Orbital CPR
The Hubble Space Telescope is still offline nine days after it first failed, despite numerousattempts to bring its payload computer back online.
NASA still doesnt know for sure what caused the computer glitch that brought the orbital observatory offline, according to Insider. But after three failed attempts to switch to a backup version of the memory module that NASA believes to be a probable culprit, its time to start wondering whether the telescope could, in fact, be dead for good.
The telescope was most recently serviced in 2009, when a human crew went up into orbit to install upgrades including a new computer, batteries, and scientific instruments. But NASA wont send a crew this time around, since it doesnt currently have a spacecraft capable of the mission. Besides, Insider notes, the space agency still has a few tricks up its sleeve when it comes to rebooting the bugged-out computer.
Aside from the backup modules, theres also an entire backup payload on Hubble that NASA can try to switch to, possible circumventing the problem altogether. Theres no word yet, however, on when NASA might switch strategies and give that a go.
If it turns out that NASA cant bring Hubble back even after exhausting its remaining options, NASA has also planned out a viking funeral for the iconic telescope. During that 2009 mission, NASA also installed a device that will steer Hubble back into the Earths atmosphere, where it will incinerate upon re-entry.
All we can do now is hope that it doesnt come to that and that one of the remaining failsafes revives the iconic telescope, just like during a previous glitch back in March.
