14 Technologies That Could Change Your Life and Your Portfolio – Value The Markets

In September, Bank of America released a 152-page research report identifying 14 technology 'moonshots'. These are 14 "radical technologies that could change our lives and accelerate the impact of global megatrends."
This research report could be a good starting point for investors who like to jump on the next big thing. It pinpoints 14 potential growth areas with a $330 billion market size today that could grow to $6.4 trillion by the 2030s.
These 14 technologies are split into four categories: Computational Tech, Human Tech, Consumer Tech & Green Tech.
The Computational Technology category includes 6G telecom networks, Brain Computer Interfaces and Emotional AI.
While 5G is still being rolled out in the Western world, it's believed it will need to be replaced by the next generation 6G within ten years.
Industry is rapidly investing in next-generation connectivity, mainly 5G. Telecoms stalwart Nokia doesn't expect 6G to start hitting the market until 2030. In the meantime, there are two to three years until 5G peaks.
The Fourth Industrial Revolution will be possible with the combination of 5G and cloud and artificial intelligence. This will then be enhanced and further stabilized with 6G.
Earlier this year, this Chinese government outlined plans to accelerate the development of 6G up to 2025 through the IMT-2030 (6G) Promotion Group.
Brain computer interfaces sound particularly futuristic, but this technology is already promising lifestyle improvements for individuals who have paralysis, along with significant innovation in gaming. Down the line, it will help humans become powered up to keep pace with advancements in AI.
Furthering this sci-fi revolution, Emotional AI, otherwise known as 'Affective Computing' or 'Cognitive Computing', is designed to capture, analyze and respond to human emotions and simulate human thoughts.
Nokia surprised investors by beating expectations in its Q3 earnings. It is enjoying sales growth driven by companies upgrading to 5G, cloud investment and improved network services.
Unfortunately, Nokia's recent status as a 'meme stock' could hurt its chances of making considerable gains going forward. It's also facing supply chain issues and increased costs.
In the 6G race, Samsung is likely to be further ahead. Indeed, it outlined its 6G vision last year. Meanwhile, Apple, Google, Cisco, AT&T, Bell Canada, Ericsson, Facebook, Microsoft, Nokia, Qualcomm Technologies Inc., T-Mobile, Verizon, Hewlett Packard Enterprise, Intel, LG Electronics, VMware, and others have joined a consortium called the 'Next G Alliance'.
The Human Technology category includes synthetic biology, immortality and bionic humans.
According to the National Human Genome Research Institute, Synthetic Biology (synbio) is:
a field of science that involves redesigning organisms for useful purposes by engineering them to have new abilities.
Researchers are harnessing synbio to solve medicine, manufacturing, and agricultural problems by drawing on the vast diversity of nature.
People have been searching for a cure for aging or the elixir of life since time immemorial. Now, immortality may not be entirely out of reach. Genomes could hold the key.
And along with extending life comes the option to enhance our bodies with superhuman abilities. Bionic humans could be created with implants or exoskeletons. Biohacking is already making breakthroughs in enhancing the human body.
Amyris, Inc. is a synthetic biotechnology and renewable chemical company. Its share price has suffered in recent months, but analysis consensus is that this stock is undervalued. Amyris is producing high-value ingredients for the health and beauty industry.
Berkeley Lights (NASDAQ: BLI) is a leading digital cell biology company. Its beacon system gives customers in synthetic biology a competitive advantage. According to Amyris:
The Beacon platform offers a truly advanced solution to synthetic biology by making it possible to test engineered microbial strains in a fraction of the time and at unprecedented scales.
Absci Corporation (Nasdaq: ABSI) is another drug company harnessing deep learning AI and synthetic biology to expand the therapeutic potential of proteins.
Meanwhile, Ekso Bionics Holdings Inc (NASDAQ: EKSO) develops and manufactures powered exoskeleton bionic devices that can be strapped on as wearable robots.
The Consumer Technology category includes eVTOL, Wireless Electricity, Holograms, and the metaverse.
eVTOL is the closest we've come to flying cars. It stands for Electrical Vertical Take-off and Landing Vehicles. These are being developed as an alternative mode of transportation to ease the burden on aging infrastructure and congested roads.
The Internet of Things (IoT) and expanding electric vehicle (EV) infrastructure requires wireless electricity to ensure seamless rollout and stable electricity supplies in remote settings.
Meanwhile, Mark Zuckerberg envisions holograms as a staple of the metaverse. While not actually a Facebook invention, Zuckerberg is pioneering the advancement of the metaverse. And is on a fast-track mission to make virtual reality our present reality.
In a not-so-distant future, the metaverse will allow humans to be present in a virtual world.
Projected to be a future iteration of the internet, aka Web 3.0, simulated environments and holographic projections should allow the physical and virtual worlds to converge.
With its recent name change to Meta, it seems Facebook is going all-in on making the metaverse a reality. For that reason, if you want to invest in these upcoming technologies, this would seem as good a place as any to start.
Facebook has seen its share price rise 168% in the past five years, so it's got plenty of fans. Of course, many don't understand its vision, and therefore FB stock could be in for a rocky time ahead.
Nevertheless, plenty of investors love the potential and see its extensive reach, (booming marketplaces, WhatsApp, community video, Instagram, expansion into VR, AR, AI, e-commerce, and physical tech products) amounting to a perfect collection of entities to bring it success in this brand-new world.

The Green Technology category includes next-generation batteries, OceanTech, green mining, and carbon capture and storage solutions.
With EV expansion growing exponentially, sustainable alternatives to lithium batteries are in vogue. Solid-state, vanadium flow, and sodium ion offer potential.
OceanTech includes harnessing the power of the waves and tides to create energy, land-based aquaculture, and precision fishing using AI.
Green Mining suggestions are questionably green but include deep-sea mining, agromining, mining of wastewater and even asteroid mining.
Meanwhile, carbon capture and storage (CCS) technologies are gaining traction as many oil majors transition into renewables. With legacy pipelines and rigs lying dormant, they have the facilities, talent, and resources to make strides in CCS development to remove and store carbon emissions permanently.
While pivoting from Oil and Gas to renewables, Royal Dutch Shell (NYSE: RDS.A) is developing large-scale commercial CCS projects in Australia, Norway, and Canada.
Many Oil and Gas companies are transitioning in this way. Aker Carbon Capture, an offshoot of Aker Solutions, is advancing its CCS projects around the world. While Aker Biomarine (FRA: 1PG) is investing in precision fishing using AI.

A Twin Cities doctor spread misinformation about COVID-19. Then he died from it – Bemidji Pioneer

As a physician who trained in internal medicine at the University of Minnesota and became a natural medicine doctor, Foleys passion lay truly in taking care of other people, said his son, Logan.
But through his Vadnais Heights-based practice, Foley also spread falsehoods about the COVID-19 pandemic and vaccines.
In blog posts over the past year, Foley wrote on his practices website that it was dangerous to wear masks and that the drug ivermectin was a proven treatment against COVID-19 a drug he prescribed for patients even though the Food and Drug Administration warns against it. He reposted false claims about the vaccine made by Robert F. Kennedy Jr., a well-known opponent of vaccines who has been banned from social media platforms.
These widely debunked claims run contrary to widely held best practices for treating and preventing COVID. But for some who believe them, misinformation has played a role in developing severe illness from the virus, or even dying.
That includes Foley, who died in October of complications from COVID-19. He was 71. At his funeral, Foleys son Logan confirmed his fathers death from COVID and that he was unvaccinated. Foleys death certificate says tobacco use played a role in his death.
Its not clear whether Foleys views on the virus and how to treat it harmed his patients. At his funeral, his son claimed his father helped 50 people through COVID infections.
The circumstances of Foley's life and death reveal a problem that's vexed the medical profession throughout the pandemic: Some licensed practitioners are fueling COVID's spread, seeding doubts about widely accepted research and medical practices, including vaccinations, that have been saving millions of lives for decades.

Doctors can be particularly potent sources of misinformation, said Rachel Moran, a postdoctoral scholar at the University of Washington Center for an Informed Public.
Vocal opposition is especially damaging when it comes from these medical professionals because we ask the general public when they're feeling hesitant about the vaccine to go and discuss their concerns with a doctor, she said.
Patients, she said, trust their doctors with their lives.
If you can go online and find a medical professional who aligns with your political viewpoints about masks or a vaccine mandate, and offers up seemingly legitimate medical advice, that's going to cement your vaccine hesitancy and its not going to provide you with the information that you need to make a sound decision, she said.
Members of Foleys family and many close colleagues and friends either declined to speak with MPR News for this story or didnt return calls.
According to his obituary, Foley graduated from the University of Minnesota School of Medicine in 1976, and worked for what is now M Health Fairview for 22 years in internal medicine. He was in good standing with the Minnesota Board of Medical Practice until his death, with no record of disciplinary action.
Retired nurse and psychologist Anne Hannahan met Foley in the 1990s when she was approached about opening a wellness center within the HealthEast system.
She and Foley shared a passion for integrative medicine, which is also called alternative or natural medicine. It combines pharmaceuticals, testing and other western approaches to healing with nontraditional approaches such as yoga, acupuncture and meditation.
Hannahan described Foley as ahead of his time.
Chris was just solid, he was brilliant. He would research everything, she said. He was very respectful to patients, and people loved him.
Foley went on to open a similar center at M Health Fairviews Woodwinds campus in Woodbury.
He was really trying to help patients do the best he knew how in both conventional and integrative medicine, said Hannahan.
Eventually, Foley landed in private practice in 2001 when he opened Minnesota Natural Medicine. Along with blood pressure tests and customized supplements, he also offered a test that purported to detect cancer early.
Hannahan said she hasnt spoken to Foley in well over a decade, and was surprised by his views on COVID-19 views shes noticed take root in alternative medicine in general.
I dont understand it, she said. Its hard to, being a nurse, a psychologist, a mother, a grandmother, and being double vaxxed.
Alternative medicine typically isnt based in science, said Timothy Caulfield, Canada research chair in health law and policy at the University of Alberta, who studies how misinformation about the pandemic has flourished within the wellness industry.
We had this tolerance of pseudoscience before the pandemic, because many regulators viewed it as somewhat harmless, and not a major issue with respect to health policy, he said. The pandemic has made room for health approaches that don't have a solid, solid scientific basis and can do real harm.
Caulfield said his research shows that people are often drawn to alternative medicine because theyve been dismissed by conventional health care practitioners, who often dont have a lot of time to help patients with difficult-to-diagnose health issues.
Many patients feel like they're not being taken seriously, that they haven't been listened to. And those who are providing alternative medicine often give them that empathy and give them that time, Caulfield said. That sounds very positive, but it's not because what they're really doing is exploiting a genuine problem with the conventional system.
Devin Werthhauser, 26, started seeing Foley a decade ago for a chronic case of Lyme disease, a tick-borne illness that Werthhauser said left her with long-term neurological issues.
Before she found Foley, Werthauser said she saw other doctors who didnt take her symptoms seriously. Foley, she said, didnt cure her but he did make her feel like she wasnt crazy.
He made me feel seen, he made me feel heard, like I'm not alone, like I'm not going crazy, and hopeful, honestly, she said, calling him the best doctor I've ever had.
Werthhauser didnt seek COVID advice or care from Foley, but retired physician Robert Geist did.
Geist said he grew up in the same neighborhood as Foley, but didnt get to know him until they were both adults and practicing medicine. Foley had a moral compass like you can't believe. The kind of guy you want for your doctor.
Geist said hes fully vaccinated, but at 93, worried about his immune response if he was exposed to the virus.
I was anxious to have a prophylactic way of dealing with it, Geist said. Foley prescribed ivermectin, a drug to treat parasitic infections, for Geist. He was very willing to do that. He thought that was a good prophylactic idea.
The Food and Drug Administration has not approved ivermectin to treat COVID and has warned that taking it in large quantities can have deadly consequences. Most doctors and pharmacists strongly oppose prescribing ivermectin outside clinical trials. Studies about ivermectins effectiveness in treating COVID-19 are mixed at best.
Geist said he and Foley shared the view that drugs to treat COVID-19 have been too quickly dismissed by the government and the medical establishment.
Its a topic, Geist said, that he and Foley would frequently discuss with a like-minded group of doctors and others who think COVID-19 isnt as serious as its been made out to be.
Geist points to malaria treatment hydroxychloroquine, a drug that has been found in clinical trials to have little benefit for COVID patients, as another example.
The problem with those drugs is they're politically incorrect because Donald Trump said we ought to try something, Geist said.
Geist said he was surprised by Foleys death. Like others, he described Foley as a healthy eater who was physically active. Still, because of his age, Geist would have advised Foley to get the COVID-19 vaccine.
Geist said he wasnt aware Foley had died of COVID-19 until his son, Logan, announced it at his funeral. Foleys obituary stated he died of an unexpected illness.
He died of complications from COVID. Was he vaccinated? No, he wasn't, Logan said. If hed only been vaccinated, wouldn't he still be here? Obviously, we'll never know.
Its likely Foley would be alive if hed been vaccinated. Evidence shows vaccines provide strong protection against death, and according to the Centers for Disease Control and Prevention, most deaths are now among the unvaccinated.
At the funeral, Logan suggested that following the medical establishment on treating and preventing COVID-19 would have been a betrayal to his fathers medical values, values based on freedom and individual choices in medicine.
My dad had a deep sense of love for this country, and the bedrock of American freedom that makes her great. Individual liberties course through his veins, especially when it came to health care, Logan said. He sought to empower his patients with information and equip them with knowledge that would empower them to make their own decisions about their health.
Logan said that the last time he spoke to his father, he was already in the hospital being treated for COVID-19.
He said, Hey, look, I love you, Logan said.
Logan responded: Yeah, I love you, too, man. I'll see you when you get out of there.

Investing in ourselves
Finally, something positive happens for the U.S. and its citizens: Congress passes an infrastructure bill. Its about time we invested in ourselves and the infrastructure of this country, something that will benefit most Americans in some way. Better highways, safer bridges, cleaner water, cleaner environment, etc.
And what do our beloved Arkansas political leaders say? Oh, its wasteful. And I ask: Where were you when you were passing tax breaks for millionaires and billionaires? Where were you when supporting wars in Iraq in the name of protecting us from weapons of mass destruction? Well, you were right there supporting these mistakes and for spending trillions. Yet when we spend money on ourselves, it's too much.
Yes, the party of no that endorses the myth of voter fraud, supports a president who promoted Jan 6, believes in the Big Lie, and thinks women can't make their own reproductive choices, taking away their freedom.
But Ms. Sanders is going to protect us all from those evil liberals who think we should help disadvantaged and disenfranchised fellow citizens because that would be socialism. And she will protect us from taxes by doing away with the state income tax? What a farce!

On erasing of history
In response to the guest column by Mary Remmel Wohlleb published recently, Ms. Wohlleb states very eloquently, "I pray that Arkansans and Americans learn to see dignity of all human beings, of prisoners, of refugees and immigrants, of those who have traditionally been held down as 'other.'" She makes a clear case that all human beings should be respected and cherished. I believe her pleading could not be clearer in making the case for protecting preborn human beings.
As we go through a period when we look to our past and many want to erase that past because of the dark shadow it casts over our present, we are reminded that all human beings have value. In the past we have classified some humans as being something not-human or below human. We did that to assuage our consciences while taking full advantage of those we labeled subhuman.
Today we classify the preborn as tissue, other, or anything besides "human." As in the past, we do that to assuage our consciences and prevent the accumulation of liability for killing humans. We should learn from history but we rarely do. The lessons of the past should tell us that not honoring all humans as God's creation will ultimately place us on the wrong side of history. And that is a very good reason to not erase any part of history.

Take virus seriously
Starting school in 1954, we children knew about polio. It was the girl in class who wore iron leg braces, the house we passed that contained an "iron lung" and someone lying helpless and immobile inside, and it was my friend with throat scars, a present from polio surgery.
In the first grade we collected dimes for the March Of Dimes to finance research. Polio fear thankfully ended with three sugar cubes saturated with a polio vaccine developed by the lionized Doctor Salk. So strange Dr. Salk was considered a medical hero, not as a witch doctor like Dr. Fauci.
Two years later fear returned. One of our classmates with a raging headache was sent home. She died of meningitis. The school sent home a sheet warning parents with written instructions where and how to get children vaccinated. I don't remember any protests against governmental overreach, but these were different times.
In high school dread resurfaced when equine encephalitis, a disease transmitted to humans from horses, began to be spread by hungry mosquitoes (via their saliva). It was years before my aunt could raise her hands over her head.
Whispers started about a miserable, fatal disease spread by blood contact, AIDS. No vaccine arrived in the nick of time, but after years, treatments to mitigate the effects and reduce the spread were found (thank you, Dr. Fauci) No problem, no sweat, we have plenty of time to continue our foolish infighting. Let's make this about morality and divine punishment.
Now we can add our current contender, covid, to the list of early warnings we've received, and this is just one generation of observations.
We could take these serial scourges seriously and get the message: Medical issues are not political opportunities.
Or, we could save our dimes and join the GoFundMe for Sen. Ted Cruz's project. His apparent plan: Surround Texas with a chicken-wire wall to keep out Big Bird and his politically incorrect messages.

Well, heres a modest addition, minus the block lettering, from yours truly, outside observer: The future of cinema is alternate takes.
That thought, profound or otherwise, occurred alongside the realization that Jason Reitmans Ghostbusters: Afterlifea fourth entry in the Ghostbusters cycleis set for release a week from Friday, on Nov. 19.
The film is unusual not in its reiterative naturein the blockbuster world, franchise pictures are the rule, not the exceptionbut rather in the fact that it overlooks its predecessor, Paul Feigs female-driven Ghostbusters reboot from 2016.
This, of course, caused some hard feelings. Back in 2019, Leslie Jones, a lead player in the reboot, declared on Twitter: So insulting. (Expletive deleted.) We dint count. Its like something trump would do. (Trump voice) Gonna redo ghostbusteeeeers better with men, will be huge. Those women aint ghostbusteeeeers ugh so annoying.
Reitman then paid polite homage to the reboot. Jones apologized for her language, not the sentiment. Columbia Pictures, which made both films (actually, the new one involves kids), forged ahead, and is obviously hoping for the best next week.
But doesnt the contretemps suggest something even better than a model for soldiering through movieland embarrassment? Might it actually hold a key to the Future of Cinema, in an age when you have your truth, I have my truth, and we can no more agree on what we want from a film than on political candidates, vaccine statistics, or the proximate date of climate Armageddon?
Lets face it. Were not coming out of our cultural caves any time soon. With help from social media and narrative-obsessed news organizations, weve become comfortable living within cocoons of safety and affirmation.
For the film industry, this is a problem. The biggest films, once targeted at four quadrants, risk losing one side or another of a polarized audience that divides over casting or story points that touch on race, gender, social justice or, just as often, the competing expectations of an empowered fan base. Marvels Eternals is just the latest outsized film to struggle with split opinions and come up short of predecessors at the box office.
But the Ghostbusters blow-up suggests an experiment. Instead of gambling on one huge film, why not offer two somewhat smaller takes on the same theme simultaneously?
If theres a debate over the sex or race of James Bond, make the film twice, exploring each side of the central question, almost like Peter Howitts Sliding Doors or the Marisa Silver-/Ken Kwapis-directed He Said, She Said, but on a much grander scale. Identify the two main competing points of view toward your picture, then shoot and edit it once for each side. Put the films in theaters (or on devices) simultaneously. Turn negative fan energy into a positive, as rival audience segments slug it out, each getting behind one version or the other. A single, clever marketing campaign should cover the bases. Some viewers might watch one film twice, to boost their movie. Others might buy tickets to both, to see what the other side is thinking. Either way, if costs are contained, you win.
Clearly, this wouldnt work for every film. In fact, it probably wouldnt work for many. But once, or twice, or three times a year, parallel runs of alternate takesfilms that honestly tell a single story from rival points of view (were not talking about edits to please censors or the airlines)might become an event.
Who knows? They might even point toward The Future of Cinema.

Id describe the finish as more satin than dewy, but its definitely not flat the way most matte foundations can be. Ammonia and phosphate levels in the Yamuna river caused by the industrial pollutants have been rising at an alarming rate leading to the formation of hazardous foam at many places. Talking to the media, leading dermatologist, Dr. Deepali Bhardwaj said that exposure to this hazardous foam can lead to skin allergies, irritation, and many types of skin pigmentation. She further added those with a family history of autoimmune diseases like diabetes and thyroid can definitely experience this earlier and in varied forms. Also, vitiligo or other autoimmune diseases can get triggered by industrial pollution in the water. "Skin cancer has also occurred besides common bacterial and viral infections like tuberculosis, viral warts, etc. which can spread from one person to another," she said.
Explaining more about the effects of the toxic foam on the skin, another leading expert in the field of Cancer, Dr. Anshuman Kumar said: "If this water, which has a high concentration of ammonia, is swallowed, these chemicals can cause serious damage to the lungs and lead to gastrointestinal problems and diseases like typhoid." She went on to add, "Long-term exposure to heavy metals like lead, mercury, and arsenic may cause hair fall, conjunctivitis, and hormonal imbalances."
Chhath Puja is an ancient Hindu Vedic festival historically which is celebrated in India especially in the states like Bihar, eastern Uttar Pradesh and, Jharkhand. On this particular day, the devotees take a dive in the river to mark the occasion. However, following the increasing amount of toxic foam in the river, the experts have warned that the devotees must take precautions before taking bath in the Yamuna. "Devotees should apply coconut oil before going into the river as it forms a barrier for some time and can prevent skin irritation," Dr. Sardana said. He further advised, "Avoid wearing synthetic clothes as they leach out allergens. Cotton clothes are ideal. Preferably an old dress will do as that leads to less chance of chemical-induced allergy."

How does the gut microbiome contribute to the immune systems reaction to certain allergens?
I think it's a very complex situation, which I'm trying to explain in a few simple sentences. In short, we know that the gut microbiome, particularly a more diverse gut microbiome, produces certain short-chain fatty acids; that's really been the aspects that I've been looking at. Clearly, there's many other metabolites that are produced by the gut microbiome. But in terms of the short-chain fatty acids, it seems that for now, butyrate is the most important fatty acid. Butyrate sort of has crosstalk with the immune system. It upregulates, or increases, the function of the T regulatory cells. The T regulatory cells just try and calm everything down againif I can put it in simple terms. Really, in short, if we have a healthier or more diverse gut microbiome, which can help us to make more butyrate, perhaps we'll find that our immune system will be more tolerant to all the food allergens that we put into our gut.
How does the microbiome affect different types of atopic diseases?
That's a really good question: whether diet diversity affects all allergic disease outcomes in the same way. Im first author of an [European Academy of Allergy and Clinical Immunology] EAACI position statement, where we looked at diversity in infancy vs asthma, allergic rhinitis, eczema, and food allergy outcomes. At this point in time, we see the biggest signal between diet diversity and food allergy. The data were a little bit more conflicting, particularly for eczema, where almost a certain number of papers indicated that increased diet diversity may prevent eczema later on in childhood vs no effect on eczema prevention. We even had a few papers that showed increased diet diversity was associated with increased eczema or atopic dermatitis outcomes.
I looked at my own data then to see, when we look a little bit more mechanistic and at IgE production, what happens, and clearly we showed that increased diet diversity by 6 months was associated with reduced sensitization to food allergens. But there was no effect seen on the aeroallergens. So perhaps there is some sort of specific interaction between diet diversity, its effect on the gut microbiome, and food allergen sensitization and food allergy outcomes specifically.

Recognising common skin conditions in people of colour – The Pharmaceutical Journal

Identifying common skin conditions in skin of colour (SOC) is inherently difficult for healthcare professionals owing to the lack of inclusivity and representation in medical textbooks and practical training, which have historically focused on Caucasian/light skin tones[1]. These biases in teaching and training have led to delays in diagnosis and appropriate treatment in people of colour, leading to greater morbidity and mortality in this patient group[2,3]. In the UK, 54% of the population is affected by skin disease annually, with 2333% of the UK population having skin conditions that would benefit from medical care. In children, 34% of diseases are skin-related, with atopic eczema affecting 20% of infants[4].
Delayed diagnosis may result in increased severity of a skin condition, leading to increased itching, dryness, skin thickening and hyperpigmentation owing to the prolonged inflammation[5]. This can result in a need for more aggressive and prolonged treatments once a diagnosis is made[2]. A UK study involving 287 GPs showed that, in primary care, malignant melanomas were significantly underdiagnosed in darker skin compared with lighter skin types[6]. The ability to diagnose and understand the main differences in the presentation of redness, inflammation and scaling present in common skin conditions between different skin tones is imperative for pharmacists to properly serve diverse populations[1].
The Fitzpatrick scale (see table) describes skin according to its reaction to sun exposure, which is dependent on the amount of melanin in the skin, and will be used when discussing skin types throughout this article[7].
Darker skin types are more prone to post-inflammatory hyperpigmentation (PIH), as described in an earlier PJ article Common dermatological conditions in skin of colour[1]. The common presentation of redness, scaling and inflammation that is easily visible on fair skin is less likely to present in the same manner in darker skin tones, making diagnosis difficult[8].
Once the correct diagnosis has been made accounting for the potential for variation in presentation in most cases, the management of dermatological conditions is the same regardless of skin type. For this reason, the management of these conditions will not be discussed in detail, but a summary of best practice is included later in the article[2]. This article aims to aid pharmacists in the identification of several common dermatological conditions that pharmacists may encounter in SOC by outlining their characteristic features and any relevant diagnostic questions.
Community pharmacists play a pivotal role in supporting patients of colour who present with inflammatory skin conditions. Patients should be supported and advised on how to manage their symptoms through best use of emollients, lifestyle changes, managing triggers, proper use of prescribed topical treatments, and recognising signs of infection as this would require referral to their GP or dermatologist.
Atopic dermatitis (i.e. eczema) is a commonly occurring chronic inflammatory skin condition that typically presents in the first year of life[9]. Pruritic (i.e. itching), erythematous (i.e. inflamed and red) plaques with fine overlying scale typically develop on the flexor surfaces, such as the skin on the side of a joint that folds; for example, the crooks of elbows, back of the knees and posterior neck[9]. Establishing the length of time the condition has been present is essential, as well as the frequency of flare ups and triggers. Eczema affects 1 in 5 children and 1 in 12 adults in the UK[10]. Differences between incidence in lighter and darker skin tones is not known in the UK; however, a US study has suggested that eczema is more common in black children[11].
Although the general presentation of eczema is considered similar among different skin tones, the appearance may be difficult to recognise in SOC. Presentations such as erythema, which is easily visible in lighter skin tones, is masked in SOC owing to skin being naturally darker. This can lead to late diagnosis, treatment delays and more advanced disease at the point of diagnosis, with hospital admissions shown to be up to six times higher in patients of colour[9,12,13].
There is increased scaling and lichenification (i.e. thickening) in eczema in SOC, compared with fairer skin, which has been linked to genetic mutations, such as the filaggrin or interleukin mutations, and immune pathway differences, such as differences in thymic stromal lymphopoietin, interferon regulatory factor 2 and Toll-like receptor 2 in Asian and black skin, compared with Caucasian skin[9,14].
Signs and symptoms of eczema in SOC include:
The treatment of eczema is not affected by ethnicity or Fitzpatrick skin type[9,14]. However, as PIH is of greater concern than the skin condition itself in SOC, it is important to minimise this risk by managing inflammation with appropriate steroid use and skin hydration with the appropriate emollient application.
A simple skincare routine with a gentle cleanser that does not leave the skin feeling dry after use should be advised. This keeps the skin well hydrated and prevents further dryness and irritation, as this can lead to increased stretching, eczema flares and worsening PIH[18]. Patients with moderate-to-severe dermatitis that are not responding to topical treatments should be referred to their GP for a secondary care referral, as they may benefit from oral treatments such as oral steroids and systemic immunosuppressants. Signs of infection should also be referred for appropriate antibiotic treatment[10].
Psoriasis is an inflammatory skin condition defined by sharply demarcated erythematous (a clear definition and border between inflamed and normal skin), silvery, scaly plaques that are most often seen on the scalp and external flexures; the nails, hands and trunk can also be affected[19].
Inflammatory plaques are typically pruritic and will often result in either post-inflammatory hyper- or hypo-pigmentation in SOC[8]. There is often a ring of intense erythema surrounding the scaly plaques, as well as erythematous papules surrounding existing plaques this is uniform across skin types. Once plaques have resolved, there is decreased scale and central clearing. If nails are involved, look for pitting[20].
When diagnosing psoriasis in SOC, look for sharply demarcated erythematous, silver-scaled plaques. Erythema is more subtle in darker skin tones and papules can appear more pink, violaceous, grey or even dark brown in colour. PIH is common after erythema has resolved in SOC[21]. A patient history should be taken to understand when symptoms first started; if there is a family history; and where on the body plaques often appear, the nature of their appearance and if they are pruritic[22].
For psoriasis that is not responding to treatment, interferes with daily life (prevents the patient from social or physical activities, employment or education) or is causing distress, such as a negative impact on mental health, the patient should be referred to dermatologists in secondary care via a GP[23].
Acneis an inflammatory skin condition of the pilosebaceous units in the skin. Acne lesions are typically classified as inflammatory and consist of papules and pustules, or non-inflammatory lesions, most often referred to as open and closed comedones[24].
Papules are solid, raised palpable lesions that are less than 1cm in diameter. Pustules are of a similar size but are filled with pus[25].
Comedones are defined as pores that are filled with dead skin debris and sebum[25]. Even though the contents of these pores are the same, closed comedones are often referred to as whiteheads because they are under the skin, while open comedones are often called blackheads because of the oxidation of the debris resulting in a black colour.
Post-inflammatory erythema (PIE) is where small, red and flat spots are left behind after the acne lesion heals[25]. PIE, scarring and PIH are possible symptoms of acne, as well as excess oil production[24,26].
Clinically, there is no difference in the appearance of acne lesions in Caucasian skin compared with SOC; however, histologically, there is more inflammation associated with SOC, even with smaller lesions such as comedones. This results in PIH and scarring more commonly occurring in SOC than in Caucasian skin[27].
Acne is the most commonly diagnosed skin condition in SOC[28]. Common differential diagnoses include perioral dermatitis (POD), folliculitis, rosacea and milia.
The following questions can be considered to aid diagnosis, especially in patients with SOC, as sometimes the redness is not fully visible:
Acne treatment can take around six weeks to begin taking effect[29]. However, once acne has healed, PIH can occur, causing skin to appear darker owing to an abnormal release or overproduction of melanin during the healing process[26]. Keloidal scars are firm, smooth and hard growth resulting from scar formation[25]. These types of scars are more commonly seen in SOC in comparison with Caucasian skin as there is more inflammation present, increasing the likelihood of their development[28].
In patients with SOC, the resulting PIH can be more distressing than the acne itself[26]. The duration of PIH can last much longer than the total duration of acne, with epidermal PIH (occurring on the top layer of the skin) taking around 612 months to heal, and dermal PIH (occurring below the epidermis) potentially lasting for years afterwards[26].
There are considerations required for management of acne in SOC. Treatment must be aggressive enough to minimise the risk of recurrence, without causing irritation that leads to PIH; therefore a good balance is needed[26]. This can be achieved by using strengths of treatment that the patient can tolerate without any side effects.
Effective skincare recommendations alongside the treatment should also be considered, as a simple cleanser, moisturiser and sunscreen without any other active ingredients will help reduce the likelihood of irritation and other side effects. This is true for all skin types but will be especially beneficial to patients with SOC to prevent further PIH.
Ringworm (Tinea corporis) is a fungal infection of the skin that presents with a ring-shaped lesion[30]. It can be passed on after contact with infected people and animals, as well as infected items, such as bedsheets, towels and combs[30]. This lesion may be scaly, dry, inflamed and itchy and can appear anywhere on the body, including the scalp (Tinea capitis) and groin (jock itch)[30].
Differential diagnoses include psoriasis, scabies, seborrheic dermatitis and contact dermatitis. The circular lesion is the most common way to identify ringworm and rule out alternatives; however, there are some questions that can be used to rule out differential diagnoses:
Ringworm can sometimes be hard to distinguish when diagnosing patients with SOC because the characteristic redness may be difficult to observe; however, the location of the lesion on the body and the circular rash with scale are the main diagnosing factors.
PIH resulting from inflammatory skin conditions can have a large disease burden in patients with SOC and can be as concerning as the skin condition itself[8]. For this reason, reducing inflammation should be the focus of treatment, while ensuring the risk of side effects from topical treatment is minimised, as skin peeling and irritation can result in PIH[31].
Chronic inflammatory skin conditions can also cause vast psychological distress, owing to the impact of skin disease and the resulting pigmentation which can be severe and for a prolonged period in SOC on self-esteem and self-image, leading to depression and anxiety.
