An Overview of Type 2 Inflammation and Atopic Dermatitis – MD Magazine

Peter A. Lio, MD: Hello, and welcome to this HCPLive Peer Exchange titled Management of Type 2 Inflammation in Atopic Dermatitis. Im Peter Lio from Medical Dermatology Associates of Chicago [Illinois]. Joining me in this discussion are my colleagues Dr Neal Jain, a pediatric allergist in Gilbert, Arizona; Dr Mark Serota, a dermatologist and an allergist at MD Integrations in Denver, Colorado; and Dr Matt Feldman, an allergist from Dallas, Texas.
Our discussion will focus on treatment selection and utilization of recently approved treatments, including interleukin pathway inhibitors for the treatment of type 2 inflammation in atopic dermatitis.
Were also going to share our thoughts on the newer mechanisms of action being studied and some of the unmet needs for patients with moderate to severe atopic dermatitis as well. Welcome, everyone. Lets get started.
Lets begin by thinking about atopic dermatitisspecifically the kind of inflammation were seeing herethis type 2 inflammation. When we think about this, we understand that broadly speaking, the immune system has 2 major arms: type 1 inflammation and type 2 inflammation. We know type 2 is the type were talking about with allergic diseases, IgE-mediated things like food allergy, asthma, allergic rhinitis, and of course atopic dermatitis. Other diseaseslike psoriasis, probably rheumatoid arthritis to some degreeseem to be on the other side of the immune system. Although we understand that its probably a little more complicated than this.
Its not quite a teeter-totter. Theres some overlap. We know, for example, in atopic dermatitis, as we get to later stages, we see the role of Th22 and maybe even Th1. We also know, fascinatingly, that its probably not the same for everybody. This is not 1 disease. There are multiple phenotypes, genotypes, and biomarkers that are part of this, and were just in the infancy of understanding this. For different groups of patients and even different ages, pediatric disease vs adult disease, there probably are very distinctive phenotypes that were just learning about.
That being said, we understand that this is generally a Th2-mediated disease, and weve been able to identify some of the cytokines that are associated with the early phases in particular and some of the moderate phases of atopic dermatitis, like IL-4, IL-5, IL-13, and IL-31, the master itch cytokine, with thymic stromal lymphopoietin being released from keratinocytes, in particular. All these guys driving inflammation, driving itch, and also damaging the skin barrier.
This brings us to that pathophysiology of thinking about it, but what does it mean for us in terms of managing the disease when we think about this from a population standpoint? Do you feel that there are populations that seem to have certain features or subtypes? Dr Jain, maybe you could talk to us a little about that to start us off.
Neal Jain, MD: You gave a great overview, and you hit the nail on the head: its different in different individuals. There are certainly distinct phenotypes that we see and different patterns of inflammation.
Sometimes the different patterns of inflammation can affect the way that skin might appear in atopic dermatitis; for example, patients with skin of color with different racial and ethnic backgrounds. Those are things we have to keep in mind.
Fundamentally, as you said, this is type 2 inflammatory disease. Although there are other types of inflammation that often can come into playchronicity, racial or ethnic phenotypethat background type 2 inflammatory pathway is activated even in those who have different phenotypes, whether you have whats historically been known as an extrinsic vs intrinsic phenotype. Type 2 inflammation is fundamentally important in the disease process.
Peter A. Lio, MD: I love it. Dr Serota, when youre seeing a patient, are there certain characteristics that make you think, This ones going to be more difficult? Or that its going to have a higher likelihood to persist or recur?
Marc Serota, MD: That breaks down to 2 things. The first part is making sure you have the diagnosis right. When I hear that a patient has failed typical therapies, either topicals or systemics, the first thing you always have to do is look at yourself in the mirror and ask, Do I have the diagnosis right? Is this atopic dermatitis, or is it 1 of the mimics of atopic dermatitis?
Assuming you do have the diagnosis right, certain factors will prompt you to the idea that the patient may be more difficult to treat. One would be if they failed prior therapies. The second would be if they have a large body surface area or more severe disease. The third would be if they have other atopic comorbiditiesconcomitant asthma or allergiesor triggers that theyre living with that they cant avoid. Those are some of the things that might prompt you to say this might be a more difficult patient.
The first step is identifying that you have the diagnosis right. The second is to identify some of the historical points and examination points that might make you say it would be a more challenging patient: increased body surface area, more severe disease, other atopic comorbidities, and other external factors that might make it more challenging for you to get that patients atopic dermatitis under control.
Peter A. Lio, MD: Thank you. Dr Feldman, when you see patients presenting with multiple allergic comorbidities, do you feel like they usually fuel one another, that theyre more challenging patients if they have atopic dermatitis plus food allergy plus asthma or allergic rhinitis? Or do you feel like you can see even isolated allergic diseases that can be just as bad or even worse?
Matt Feldman, MD: Thats a great question. Its patient dependent, and part of it is also if were focusing on the pediatric patient whos following along the atopic march. On the 1 hand, its helpful to educate the family about that atopic march and how all these other comorbidities at times can feed one another. The asthma flares as theyre playing outside at summer camp, walking through the grassy fields, etc. Theyre sneezing, wheezing, and itching all at once.
That can be a helpful way to educate the family about the underlying pathophysiology of type 2 inflammation that theyre seeing living and breathing biomarkers in front of them. At times it can, no pun intended, get us in the weeds a little. Sometimes I have familieskids and adultsthat will come in thinking that the food is the driver of their atopic dermatitis as opposed to maybe the dog in the home that they have a specific IgE of 95 IU/mL too. Sorting through those different atopic comorbidities in the individual patient is important on an individual basis because all our patients are so different and heterogeneous.
Neal Jain, MD: You made some good points, Peter, as did Mark and Dr Feldman. One thing we need to inform our patients about when we see young kids and even adults is the chronic nature of this disease. Were learning more about these different phenotypes. I agree entirely with Dr Serota. When its confusing and you see these patients with severe disease, especially with skin of color, which may have a more psoriasiform-appearing dermatitis, [we have to] look for those atopic comorbidities can help us identify that this is atopic dermatitis. [That will] help ground us in the fact that that is whats going on.
There have been some interesting studies published even recently about phenotypes of atopic dermatitis. To the point Dr Feldman was making, we know that comorbid food allergy may seem to define a population of patients with atopic dermatitis that tends to be a little more severe. We see increased levels of transepidermal water loss and decreased filaggrin expression. Its not that the food allergy is caused by the atopic dermatitis, but it helps us define a subtype of atopic dermatitis that may be more severe.
Marc Serota, MD: For patients with atopic dermatitis, its very important to assess their comorbidities for a few reasons. One [is that its] sometimes difficult to diagnose someone with atopic dermatitis as opposed to a mimic, and you can use the fact that around 50% of your patients with atopic dermatitis will have another atopic condition, like asthma, like allergic rhinitis, like food allergies. If you identify that they have 1 of those comorbidities, the first thing it can do is help you make the diagnosis of atopic dermatitis. It adds to your clinical history and can help bolster the argument that you do have a patient with atopic dermatitis.
The second reason that its important to assess for those is just like psoriatic arthritis and our patients with psoriasis, psoriatic arthritis is the game-over scenario of psoriasis. Its nice that we clear their skin, but if their joints are being affected and we dont address that, then potentially they can have morbidity associated with that undiagnosed problem. The same is true in the atopic world. We want to assess patients specifically for asthma because were not going to fix that by just giving them a topical steroid cream, for example. If they have uncontrolled asthma, 1, that might tip us into using a systemic we might not otherwise have, and 2, we have treatments that can treat both conditions simultaneously.
Its very important to assess for comorbidities, even if your specialty isnt allergy asthma immunology. If youre a dermatologist, just as we assess patients for psoriatic arthritis for psoriasis, its also important that we assess patients for uncontrolled asthma. I ask a few screening questions. How often are you using your albuterol? If its every day, youre not under good control. Are you waking up at night coughing and wheezing? Have you been to the ED [emergency department] for asthma? Have you been hospitalized for asthma? Have you ever been intubated for asthma? Those are red flags that their asthma may be poorly controlled. Youll want to factor that in when youre considering what treatment youre going to choose to treat their atopic dermatitis as well.

Ron Paul: Is Washington Fighting Russia Down To The Last Ukrainian? OpEd – Eurasia Review

As the Russian invasion of Ukraine moves past its third week, there are slight hopes that negotiations between the two sides may soon produce a ceasefire. But with the shrill warmongering talk in Washington, it almost seems like the US government would hate to see that happen.
Congress and the US Administration seem determined to drag the United States into a war with Russia over Ukraine. Senator Lindsay Graham is openly calling for someone to kill the Russian president and many in the US House have demanded that the Administration establish a no-fly zone over Ukraine.
Are they insane? A no-fly zone means you destroy anything and everything that can prevent total US air dominance. That means an attack on Russian missile and air defense systems within Russia. In other words, World War III.
We can all feel disgust at the destruction in Ukraine, but is it really necessary for us to gamble with our own nuclear annihilation?
Sadly, a large bipartisan group in Congress seems to think so.
Much of what is happening in Ukraine can be traced back to the Obama Administration. State Department officials like Victoria Nuland and Antony Blinken planned and executed the overthrow of the Ukrainian government in 2014. This is what set us on this path to conflict, as the government put in place after the coup began demanding NATO membership.
Blinken, Nuland, and the others responsible for this heinous act returned to government in more senior positions under President Biden and they have continued to push their Ukraine agenda.
Last week Secretary of State Blinken our top diplomat sought to send Soviet-era Polish fighter jets into Ukraine to shoot Russians. When the Poles said theyd be happy to ship the planes to a US base in Germany and let the Pentagon transfer them to Ukraine, the Pentagon finally stepped in to quash an extraordinarily high-risk move that even the Pentagon said would have no real effect on the outcome of the war.
The State Department is trying to get us into a war and the Pentagon is trying to keep us out. How ironic!
Back when I was on the campaign trail I would say that we have a few thousand diplomats in government, it might not be a bad idea to use them. But I certainly did not mean that we should use them to try and get us further involved in a war!
Three weeks into this terrible war, the US is not pursuing talks with Russia. As Antiwar.com recently reported, instead of supporting negotiations between Ukraine and Russia that could lead to a ceasefire and an end to the bloodshed, the US government is actually escalating the situation which can only increase the bloodshed.
The constant flow of US and allied weapons into Ukraine and talk of supporting an extended insurgency does not seem designed to give Ukraine a victory on the battlefield but rather to hand Russia what Secretary of State Blinken called a strategic defeat.
It sounds an awful lot like the Biden Administration intends to fight Russia down to the last Ukrainian. The only solution for the US is to get out. Let the Russians and Ukrainians reach an agreement. That means no NATO for Ukraine and no US missiles on Russias borders? So what! End the war then end NATO.

A Texas lawmaker has introduced legislation that would allow private U.S. citizens to seize assets and property owned by Russian oligarchs who have been sanctioned in response to Russia's ongoing war with Ukraine.
The bill, introduced on Feb. 28 by Rep. Lance Gooden from Dallas, would require President Joe Biden to issue letters of marque and reprisal for the seizure of yachts, planes or other property owned by those on the Treasury Department's list of sanctioned Russians. The proposal comes as the U.S. continues to increase sanctions against Russia's elite in response to the country's war against Ukraine. While Biden has warned Russian oligarchs that the U.S. will seize their ill-begotten gains," Gooden says it's not enough.
"Putins inner circle have planes and yachts sitting at airports and harbors all over the world," Gooden said in a March 1 news release. "If President Biden refuses to act quickly, then it is time Congress and American citizens take matters into their own hands. Russian oligarchs must be held accountable for enabling the disgusting invasion of Ukraine.
Letters of Marque and Reprisal, a historic tool authorized in Article 1 of the U.S. Constitution, allow private citizens or groups to seize enemy property on behalf of the U.S. government. Such letters were used during the War of 1812, but have not been issued since.
This new bill would authorize "privately armed individuals and entities to seize the assets of certain Russian citizens" and states "the holder of such a letter [of Marque] shall be authorized to employ all means reasonably necessary to seize any asset, such as a yacht or plane, outside of the United States that belongs to a Russian citizen on the Office of Foreign Asset Control's List of Specially Designated Nationals and Blocked Persons."
Gooden's office told Fox News that the letters could even be used in cyberspace for U.S. hackers to go after Russia. "Corrupt Russian oligarchs have enabled Putin's war," Gooden tweeted. "That's why I have introduced legislation to allow U.S. citizens to SEIZE yachts and jets of sanctioned Russians. We must use every tool at our disposal to stand up to Russia and stand with Ukraine."
Former Texas Rep. Ron Paul introduced his Marque and Reprisal Act following the 9/11 attacks, which would have authorized private citizens to "seize the person and property of Osama bin Laden and any other individual responsible for the terrorist attacks of Sept. 11." In 2009, Paul again proposed issuing letters of marque to combat Somali pirates.

FIRST ON FOX: The political wing of a top grassroots group that represents Jewish Republicans across the country is endorsing GOP Sen. Ron Johnson as he runs for re-election this year in the crucial battleground state of Wisconsin.
And the Republican Jewish Coalitions (RJC) PAC, in an announcement shared first with Fox News on Wednesday, also backed nine House GOP lawmakers and four congressional candidates as it made its second round of endorsements ahead of November's midterm elections, when Republicans aim to win back majorities in the House and Senate.
SEN. RON JOHNSON OF WISCONSIN ANNOUNCES 2022 RE-ELECTION
Johnson, a conservative senator first elected in the 2010 Tea Party-fueled red wave and a strong supporter of former President Trump, announced in January that he would run for a third term. Democrats view Johnson as vulnerable, and hes one of the most heavily targeted GOP senators running for re-election this year.

The race in Wisconsin is one of a handful that could decide which party will control the Senate next year. The chambers currently split 50/50 between the two major political parties, but the Democrats hold the majority thanks to the tie-breaking vote of Vice President Kamala Harris through her constitutional role as president of the Senate. That means the GOP needs a net gain of just one seat to reclaim the majority it lost when it was swept in the January 2021 twin Senate runoff contests in Georgia.
In the House, Republicans need a net gain of just five seats in the 435-member chamber to win back the majority they lost in the 2018 midterm elections.

The RJC is also endorsing Republican Reps. David Schweikert of Arizona (AZ-01), David Valadao of California (CA-22), Andy Harris of Maryland (MD-01), Peter Meijer of Michigan (MI-03), Don Bacon of Nebraska (NE-02), Yvette Herrell of New Mexico (NM-02), Nicole Malliotakis of New York (NY-11), Brian Fitzpatrick of Pennsylvania (PA-01) and Nancy Mace of South Carolina (SC-01).
Mace was one of the earliest supporters of Trumps successful 2016 campaign to win the White House. But while the freshman House lawmaker didnt vote to impeach Trump 14 months ago, she did publicly say that his rhetoric leading up to the storming of the Capitol "put all of our lives at risk."

And in October, Mace was the lone South Carolina Republican to join congressional Democrats in voting to hold former Trump White House senior aide Steve Bannon in contempt of Congress for defying a subpoena issued by the House select committee investigating the riot at the Capitol.
Trump has repeatedly lashed out at Mace and is backing Republican Katie Arrington who is primary challenging the congresswoman. Trump took aim at Mace and once again praised Arrington at a rally on Saturday in South Carolina.

The RJC is also endorsing Republican congressional candidates Esther Joy King of Illinois (IL-17), Monica De La Cruz (TX-15) and Wesley Hunt (TX-38) of Texas, and Derrick Van Orden of Wisconsin (WI-03).
In making the endorsements, RJC executive director Matt Brooks told Fox News that "we need Republicans in Congress who will fight for Americas freedom, security, and prosperity, and who support our ally, Israel. The RJC PAC enthusiastically supports these GOP incumbents and challengers, who will help Republicans take back the House and Senate majorities and will bring much-needed sanity back to Capitol Hill."
Noting that the RJCs PAC raised over $10 million in the 2020 cycle for its endorsed House and Senate candidates, Brooks emphasized that "the RJC PAC had a significant impact in 2020 and will continue to make a difference in highly competitive races like these. The candidates we are endorsing today are talented, principled, dedicated public leaders whose elections are top priorities for the GOP in 2022."

Wednesdays endorsements are the second batch this cycle by the RJC. Its PAC last year endorsed Sen. Marco Rubio of Florida and Reps. Carlos Gimenez (FL-26) and Maria Salazar (FL-27) of Florida, and Reps. Mike Garcia (CA-25), Young Kim (CA-39) and Michelle Steel (CA-48) of California.

The RJC is a nearly four-decades-old group that describes itself as "the national grassroots organization of Jewish Republicans and represents tens of thousands of Jewish Republicans across this country."
The late Republican mega donor and casino magnate Sheldon Adelson, who died at age 87 in January of last year, for years played a key leadership role with the RJC and gave it generous financial support.

Weekend Rundown: One Suspect in Deaths of Five Homeless Men in DC and NYC – Washington City Paper

Police in D.C. and New York City believe one man is responsible for shooting five men experiencing homelessness in both cities. Three men were shot in D.C., one of whom died. One of the two men shot in NYC over the weekend has died.
Mayor Muriel Bowser and New York City Mayor Eric Adams released a joint statement yesterday asking for information on the suspect and advising unhoused people to seek shelter.
Three of the shootings took place in Northeast D.C. from March 3 to March 9, and the two NYC shootings occurred over the weekend. The string of shootings follow repeated calls from unhoused residents and housing advocates for Bowser to stop or pause encampment clearings in D.C.
An Metropolitan Police Department statement released yesterday said Fifth District officers responded to sounds of gunshots at the 1100 block of New York Avenue NE on March 3 at about 4 a.m. On March 8 at 1:21 a.m., Fifth District officers responded to a report of a shooting at the 1700 block of H Street NE. In both cases, they found an unhoused man with non-life threatening gunshot wounds.
On March 9 at 2:54 a.m., an MPD officer noticed a tent fire near the 400 block of New York Avenue NE. DC Fire and EMS put out the fire and found the remains of a man inside the tent. An autopsy determined that the cause of death was multiple stab and gunshot wounds.
Similarities in the victims status and the assailants actions, as well as recovered evidence, lead police in both cities to believe theyre looking for the same suspect, according to MPDs statement. MPD and NYPD will work with the Bureau of Alcohol, Tobacco, Firearms and Explosives to investigate the crimes.
Anyone with information should contact MPD at (202) 727-9099 or send a text to 50411. MPD is offering a reward of up to $25,000 for information that leads to the arrest and conviction of the suspect; the ATF Washington Field Division is offering another $20,000.
It is heartbreaking and tragic to know that in addition to all the dangers that unsheltered residents face, we now have a cold-blooded killer on the loose, but we are certain that we will get the suspect off the street and into police custody, Bowser and Adams said in their statement.
In other news
On Friday afternoon, an elderly driver crashed into a group of diners at the Parthenon Restaurant & Chevy Chase Lounge on the 5500 block of Connecticut Avenue NW. Two women from Northwest D.C., 73-year-old Terese Dudnick Taffer and 76-year-old Jane Bloom, died in the hospital. Six others were hospitalized, and three more people were treated and released.
D.C. police believe the incident was accidental, and said the driver was cooperating with investigators. The drivers name was not released. Pete Gouskos, owner of the longtime Chevy Chase restaurant, told WUSA9 that the driver was a regular diner for the past 20 years, and a very nice guy who would often come in for a glass of Coke, a burger, and to watch a sports game.Parthenon reopened after staff helped clean up the patio. The restaurants outdoor dining will reopen in time, Gouskos said.

Why Werner Herzog thinks human space colonization will inevitably fail – Ars Technica

Enlarge / Could humans eventually fly to exoplanets on massive "generational" spaceships? Last Exit: Space explores such a hypothesis with intriguing and sometimes darkly hilarious results.
Last Exit: Space is a new documentary on Discovery+ that explores the possibility of humans colonizing planets beyond Earth. Since it is produced and narrated by Werner Herzog (director of Grizzly Man, guest star on The Mandalorian) and written and directed by his son Rudolph, however, it goes in a different direction than your average space documentary. It's weird, beautiful, skeptical, and even a bit funny.
In light of the film's recent streaming launch, father and son Herzog spoke with Ars Technica from their respective homes about the film's otherworldly hopes, pessimistic conclusions, and that one part about space colonists having to drink their own urine.

"[As a narrator], I always spoke in a deadpan [voice], and of course there's a certain humor in it because listening to my accent is a joke already," Werner says from his current home in Los Angeles. His son Rudolph, phoning in from Germany, scoffs at this, to which Werner replies, "Well, to some!"
Werner notes that the script is his son's, who says that "all of my films are comedies, even if they don't look like comedies." Rudolph's inclination for dark humor is seen throughout Last Exit: Space, which is largely anchored by interviews with researchers, engineers, and ex-astronauts, though the director is also eager to feature skeptics, futurologists, and voices that he admits he "politely disagrees with."
Werner, as narrator, periodically clarifies certain points about humankind's interest in space colonization. In rare instances, he editorializes, such as when Werner describes the efforts of SpaceX, Blue Origin, and Virgin Galactic as humans "venturing out into space in a testosterone-fueled competition." Other times, Werner opts for dryly funny narration of how bleak certain space colonization efforts may turn out.
"The reality of life on Mars would be sobering," he says. "Astronauts would hunker down in radiation-proof bunkers, enjoying drinks of recycled urine."
"I knew when I speak about [drinking] your own urine, if I say it deadpan, it becomes hilarious," Werner tells Ars Technica. "If I had made a big fuss about it with my voice, it wouldn't have worked." He then tells Ars that he's familiar with an ecosystem of comedians and YouTube creators that parody his voice, acknowledging that he gets the comedy of it. "I made a film in Antarctica once, and before I even started editing, there was already a satire outabout a film I hadn't really started yet!" he says.
Though I was unable to find the satirical video in question, the finished product leans into Werner's proclivity for darkly funny deadpan, both in narration and visual content. Here's an example from the gorgeous 2007 documentary Encounters at the End of the World:

Scientists may have found a way for humans to breathe on Mars in the future.
Space travel is tricky for numerous reasons but the lack of oxygen in space is one of the biggest obstacles.
Bacteria may be the answer to all our space breathing problems.
According toMashable, cyanobacteria could be used by scientists to figure out a way in which humans could easily access oxygen in space.
Cyanobacteria take carbon dioxide and turn it into oxygen.
The bacteria is found in very hostile environments on Earth so its hoped cyanobacteria would be able to survive on Mars.
Some experts have suggested sending the bacteria to Mars and seeing if it could create oxygen for humans who end up there in the future.
Experiments have already proven that cyanobacteria can grow in a Martian-like environment.
Astrobiologist Cyprien Verseuxpreviously worked on one of these experiments.
They explained: Here we show that cyanobacteria can use gases available in the Martian atmosphere, at a low total pressure, as their source of carbon and nitrogen.
Under these conditions, cyanobacteria kept their ability to grow in water containing only Mars-like dust and could still be used for feeding other microbes.
This could help make long-term missions to Mars sustainable.
NASA has its sights set on sending humans to Mars and Elon Musk has big plans for a human colony on the Red Planet.
The SpaceX CEO has previously expressed desires to ferry to take one million humans to Mars by 2050.
By that time, scientists may have worked out a way for bacteria to provide oxygen on this long-haul mission.
Humans would still need to live in very protective shelters on Mars as the planet is a harsh environment with dramatic temperature changes and constant radiation exposure.
