Tuesday Q and A: Treatments and self-care can help ease eczema discomfort

DEAR MAYO CLINIC: My 10-year-old daughter has very dry skin, and it seems to get worse when school starts. She develops red, itchy patches on her arms and lower legs. They usually fade away after a while, but they really bother her. What could be the cause? Should we be using lotion on these areas, or something stronger? At what point should she be seen by a dermatologist?

ANSWER: From your description, it sounds like your daughter may have a skin condition called eczema. In children, it often flares up when they go back to school in the fall. To find out if it is eczema, make an appointment with her primary care provider or a dermatologist to have her symptoms evaluated. A number of treatments and self-care steps can help ease the discomfort.

Eczema, also called atopic dermatitis, is common in children. Symptoms typically include dry, scaly skin with red patches that may weep. In many cases, the itching caused by eczema can be severe. Eczema symptoms tend to come and go.

Many children who have eczema see their symptoms get worse in the fall. That may happen for a couple of reasons. First, the dry indoor air of most schools can cause skin to become very dry, especially when weather gets cooler and the heat comes on. Second, stress often can lead to eczema flares. When kids go back to school, they may feel more stress and anxiety than they did during the summer, and that may cause a spike in their symptoms.

A primary care provider or a dermatologist should be able to tell if your daughters condition is eczema simply by examining her skin. In most cases, lab tests and skin tests are not needed. A number of over-the-counter and prescription medications are available that can relieve the itching and repair skin damage. Ask the doctor which ones are right for your daughter.

Encourage your daughter to use the following sensitive-skincare steps, too. Soak in a warm bath or shower every day for about 15 minutes. When you get out, gently pat dry with a soft towel. Do not rub your skin. After you have dried off, apply any recommended medication and then cover it with a skin moisturizer. Use only shampoo, soap, moisturizer and other skin products that are hypoallergenic. They should not contain any dyes or perfumes.

Wear layers of clothing. Then if you get too warm, you can take off a layer and not become overheated. Sweating can make eczema symptoms worse, especially if sweat is left to dry on the skin. Avoid wearing tight, rough clothing or fabrics that irritate your skin.

Try not to scratch your skin. Scratching can increase your symptoms and may lead to an infection. If the itching becomes hard to bear, cover the area with cool, wet compresses. Taking a warm bath with baking soda or uncooked oatmeal in the water can ease itching, too.

Gene Therapy Shows Potential for 'Bubble Boy' Disease

WEDNESDAY, Oct. 8, 2014 (HealthDay News) -- A new form of gene therapy may offer a safe and effective way to treat "bubble boy" disease -- a severe immune deficiency that is fatal unless treated in infancy.

Researchers have long known that gene therapy can cure the disease, known medically as severe combined immunodeficiency, or SCID. Over a decade ago, trials in Europe showed that gene therapy worked -- but five of the 20 children treated developed leukemia (a type of cancer) within two to five years, according to background information in the study.

In the new trial, reported in the Oct. 9 New England Journal of Medicine, researchers refined the gene therapy approach to hopefully negate the leukemia risk.

Eight of nine children who received the therapy are still alive one to three years later, the investigators report. And so far, none has developed leukemia.

It's too early to say the therapy carries no leukemia risk, cautioned researcher Dr. David Williams, a pediatric hematologist/oncologist at Dana-Farber Cancer Institute and Boston Children's Hospital.

"We'll continue to follow these children for 15 years," Williams said.

But based on the early results, he noted, the tweaked gene therapy appears as effective at generating a functional immune system as the earlier form of treatment.

SCID refers to a group of rare genetic disorders that all but eliminate the immune system, according to the Immune Deficiency Foundation (IDF). That leaves children at high risk of severe infections.

In the United States, 40 to 100 babies are born with SCID each year, according to the National Institutes of Health. Early detection is becoming more common because a screening test recently became available. So far, 24 states and the District of Columbia have adopted the test, according to the IDF.

A new gene-therapy treatment is showing promise in treating a rare and severe congenital condition that involves extreme immune-system deficiencies.

Bubble boy syndrome, an X-linked condition, takes its name from a famous case in which an affected boy, vulnerable to infection, lived inside a plastic bubble that protected him from the worlds germs. Outside of such sterile environments, babies with the syndrome seldom live longer than a year, the Wall Street Journal reports.

The condition has for decades bested medical researchers, despite occasional bouts of optimism hope for one previous gene-therapy treatment was felled when some recipients developed leukemia.

Gene-therapy treatment works, essentially, by replacing unperforming genes with functional ones. Dysfunctional cells are removed from the childs immune system and exposed to a genetically engineered virus that can reprogram the cells to function properly, explains Reuters. Those cells are then reinserted back into the patient.

In the earlier treatment, the virus to which the cells were exposed apparently activated a part of their genetic code that leads to leukemia, Reuters says.

But initial results reported in the New England Journal of Medicine show that none of the nine babies from the U.S. and Europe who received the latest treatment are exhibiting any signs of cancer.

Of the nine infant participants in the research who were between 4 and 10 months old when they began receiving the therapy eight were still alive 16 to 43 months later, without living in a protective bubble. (The ninth child died four months after treatment began from an earlier infection he had been fighting.)

Out of the eight boys still living, the treatment upped blood T-cell levels, rebuilding the immune system, of seven. In the case of the eighth child, the treatment did not rebuild his immune system, but a successful stem-cell transplant has kept him in improved health, Reuters reports.

Steel Panther, heavy metal parody group playing in Allentown, is no joke

An interview with a member of comedy glam metal band Steel Panther can, like the band's stage show, be uproariously funny. The band's members interview in their characters of profane and politically incorrect stereotypical 1980s hair-metal musicians.

But while you chortle at drummer Stix Zadinia saying he's just come from the gym, where he was "checking out the cougars," listen to what he says about the band's tour with metal gods Judas Priest. (The bands play Wednesday at Allentown's PPL Center.)

There's plenty of truth and wisdom in the answers, too.

Take the explanation of the band's recently released third album, "All You Can Eat," given by Zadinia (he doesn't reveal it, but his real name is Darren Leader.)

"When we went in for 'All You Can Eat,' we wanted to make a complete record that covers all the bases, musically, that we liked," the drummer says. "We didn't want to do 12 tracks that sounded exactly like each other. And we didn't want to do songs that sounded like ones we recorded on previous albums. And we wanted to paint a whole musical picture.

"And I think we were able to totally go in and do that. And the end product, I think, is a ... kicking album."

Someone apparently agrees. Released April 1 (get it?), the disc hit No. 24 on the Billboard albums chart, Steel Panther's highest position ever. And it became the group's third-straight disc to top the Comedy chart, starting with 2009's "Feel the Steel" and 2010's "Balls Out."

The new album's songs are filled with over-the-top juvenile sex, drug and rock 'n' roll humor, with titles such as "Party Like Tomorrow Is the End of the World," "You're Beautiful When You Don't Talk" and several titles unprintable in a family newspaper.

But the disc is, indeed, a collection of styles, and Zadinia says the group is able to do that because of its comic persona.

"I feel like we are allowed to do whatever the ... we want to do," Zadinia says. "You take an artist like, I don't know Maroon 5, right? They're expected to put out pop songs. They're expected to sing about relationships. Or they can sing about dance songs like 'Moves like Jagger.'
