Jonathan is a 10-month-old child who has been developing an itchy rash on the face, elbows and knees. His skin in these areas looks red, dry and scaly. He has also been scratching these areas frequently. After a yellow crust was noted and his mom took him to the pediatrician she was told he has atopic dermatitis, a form of eczema. She asks the doctor what might have caused this.
Atopic dermatitis, also known as eczema, is a common skin condition which affects 1 in 10 children. While it is well known that family history of atopy (eczema, asthma, or seasonal allergies) are risk factors for eczema, the exact cause of eczema remains a mystery.

NIH Wants to Test New Bacteria Therapy for Eczema – NBC News – NBCNews.com

Myles and colleagues have been growing bacteria in the lab taken off healthy volunteers and people with atopic dermatitis. They've focused on one particular species: Roseomonas mucosa.
Tests on human skin cells in lab dishes and on mice show that healthy versions to R. mucosa can eradicate eczema and make skin cells healthier, Myles said.
"We took the bacteria from healthy people and we saw that the bacteria does everything you would want it to do to improve atopic dermatitis in a petri dish and in mice," he said.

"In mice, we can actually make their disease go away with this bacteria," Myles said.

The phase 1 study is aimed at ensuring the treatment is safe, but Myles hopes to show it's effective, also. If it is, it would be years away from the market because it will have to go through phases of testing and then be licensed to a company to produce it.
They are not the only team trying to use good bacteria to help eczema. Richard Gallo of the University of California, San Diego, and colleagues have been trying a lotion using two benign staph bacteria Staphylococcus hominis and Staphylococcus epidermidis to fight the effects of the harmful Staphylococcus aureus.

It is estimated that around 60% of British people will suffer from a skin condition at some point in their life, and for many the cause will be eczema which occurs mostly in children, but can also affect adults.
Even if you don't have eczema yourself, the chances are you know somebody who does. It is a condition that fluctuates in its severity, often 'flaring' to become red, itchy and sometimes weepy in patches. In between flares, the skin may appear normal or may show signs of chronic eczema with dry, thickened patches.

Although eczema like many skin conditions tends to run in families, it is possible to develop it as a result of external factors, and you might be surprised at some of the causes. Dr Daniel Glass, consultant dermatologist at the Dermatology Clinic London, gives us some more insight.
"There are a large array of different factors can make eczema worse. These include foodstuff in selected groups, dust exposure, unfamiliar pets, seasonal variation, stress, and irritants may be important in provoking flares of eczema."
He adds:
"Other factors that can exacerbate eczema include bacterial or viral skin infections. Contact allergic types of eczema occur after an individual comes in to contact with a substance they have become allergic to. This can be from a vast range of chemicals including those contained in hair dye, nail varnish, preservatives, antiseptics or similar. Once the culprit is found using patch tests, it's a question of removing contact with the offending substance."

Once you have a flare of eczema, it's very easy to get into a cycle of itch and scratch whether it be conscious or subconsciously.
"Stopping this may require a combination of treatment for the eczema and at times involvement of a psychologist for a course of habit reversal sessions, to try to stop the habitual scratching."

When it becomes clear that treating the skin alone is not sufficient to help the patient regain control of the eczema, then it's time to start thinking about taking a psychological route.
"If the skin disease is preventing a patient getting on with their life and psychological input can be useful. Patients with skin diseases can develop psychological difficulties with stress or anxiety due to their skin condition quite common. Skin disorders can affect people's coping and adjustment; stress itself can also have a negative impact on people's skin. Dealing with these issues can dramatically improve patients well being."

Sanofi-Genzyme, which expects to soon win approval for an eczema drug, has filed a federal lawsuit that seeks to preempt a possible patent challenge by Amgen.
The Cambridge-based firm, a unit of French pharmaceutical giant Sanofi SA (NYSE: SNY), filed a suit in Boston federal court along with its partner Regeneron Pharmaceuticals (Nasdaq: REGN). The complaint asks a judge to rule that the drug does not infringe a patent held by Amgen (Nasdaq: AMGN).

The treatment for atopic dermatitis, which will be sold under the brand name Dupixent, is expected to win FDA approval by March 29. In the complaint, Sanofi-Genzyme and Regeneron described the drug as a game-changer, citing its success in clinical trials.

To prevent it, Dr. Kathleen Zimmerman said you can use a humidifier and gentle, fragrance-free body washes and detergents. You can also use a thick daily moisturizer on the skin, preferably also fragrance-free. Petroleum jelly can act as an excellent barrier, she said.
If the eczema patches are very itchy, prescription creams may be recommended, Zimmerman said.

They warn its important to look for signs of dehydration, including increased thirst, fatigue, yellow urine, headache and dizziness.
Penn State Childrens Hospital reports seeing viral upper respiratory infections and the common cold. They are also still seeing cases of the flu and they are starting to treat injuries related to spring sports.
WellSpan Medical Group providers are still seeing a lot of stomach bug cases in Adams, Lancaster, Lebanon and York counties. They suggest frequent handwashing and keeping patients hydrated.
WellSpan also said its anticipating an increase in seasonal allergies in the near future.

A clinical trial led by the Murdoch Children's Research Institute is challenging the myth that wool is a possible source of allergy and irritates the skin for eczema sufferers.
The study published in the British Journal of Dermatology is the first study of its kind to examine the effects of superfine merino wool on childhood eczema. The aim was to improve outcomes for babies, children and families of children suffering mild to moderate symptoms.
The results showed superfine merino wool may actually reduce rather than exacerbate eczema symptoms
The trial involved 39 children aged from four weeks to three years over a 12-week period, where they wore superfine merino wool garments against the skin for six weeks before changing to cotton, and vice versa.
Despite limited evidence, woollen clothing has traditionally been considered an irritant that should be avoided by children/people individuals with eczema. However wool fibres come in a range of diameters and the superfine wool garments used in the study did not cause irritation and were well tolerated by patients.
The researchers found when children switched to wool after wearing cotton, they showed a significant decrease in eczema severity whereas eczema worsened when those wore wool changed to cotton.
The study's lead author Associate Professor John Su from Monash University and Murdoch Children's Research Institute says the results "challenge our current practice" when advising about the suitability of wool for children with eczema.
"When comparing with cotton, there are inherent differences in fibre properties, [Merino] wool's greater ability to transfer moisture vapour and heat than other major apparel fibres enable it to maintain a more stable microclimate between the skin and garment," A/Prof Su says.
Parents said their children enjoyed wearing the woolen garments, found them soft and also reported less itching and skin clamminess.
Ellyda Stone explains how her son, Ayden, who took part in Prof Su's trial, responded well to the merino wool garments.
"We noticed that merino wool garments tended to allow for better absorption of moisture, and that this protected Ayden's skin from dryness and improved his eczema," Ellyda says.
The research highlights the need for further studies on the effects of clothing and the microenvironment between clothing and the skin on eczema.

Skin creams have been linked to dozens of deaths in fires, leading to new warnings by manufacturers.
Products used to treat conditions like eczema and psoriasis put users at risk because the paraffin in them can soak into their bedding or clothes, making them flammable.
The medicines regulator has updated its guidance and it says all creams containing paraffin should carry a warning.
There have been 37 deaths in England since 2010 linked to the creams, despite warnings going back more than ten years.
Carol Hoes husband Philip died at Doncaster Royal Infirmary in 2006 when sparks from a cigarette ignited his moiturising cream.
She told Radio 5 Live : Philip sneaked off on to a landing for a sneaky cigarette, a gust of wind must have caught the lighter, and it set fire to him.
Within seconds Mr Hoe, who was having treatment for psoriasis, was engulfed in flames and he died shortly after.
He was covered with 90% burns, said Mrs Hoe.There was nothing they could do.
Christopher Holyoake died in a house fire in September 2015, after his skin cream, now known to be flammable, caught fire.
The coroner at the inquest into the death of 63-year-old Christopher Holyoake in Leicester in 2015 heard his bedding was covered in residue from an over-the counter dermatological cream called E45, according to the BBC .
When the flame from his cigarette lighter came into contact with the bedding, the residue acted as an accelerant, giving Mr Holyoake little chance of surviving the fire.
After the inquest the coroner wrote to the manufacturer of E45 - outlining her concerns there were no warnings on the packaging about the product being highly flammable.
At his inquest, the coroner drew attention to the dangers. The Medicines and Health Care Products Regulatory Agency issued two more warnings, but deaths have gone on.
John Smith, Chief Executive of PAGB, (Proprietary Association of Great Britain), the UK trade association for manufacturers of branded over-the-counter medicines, said: We want to reassure people that the normal use of emollients in the home is considered appropriately safe provided the products are used in accordance with the on-pack instructions and accompanying patient information leaflet.
Manufacturers of emollients are not at present required by regulation or statute to include fire safety warnings on packaging.
"Safety is nonetheless of paramount importance to the OTC medicines industry.
"In the light of this investigation, PAGB is looking to explore this issue further with the member companies and relevant bodies to see if in future, safety warnings should be added to on-pack labelling for all paraffin based emollients as standard practice across the industry, a step which some manufacturers have already taken.
