The Guardian view on Test Match Special: 60 years of tacit pleasure – The Guardian

Sound of the summer: Test Match Specials Michael Vaughan and Jonathan Agnew. Photograph: Gareth Copley/Getty Images

It is 19 years since BBC television ceased broadcasting live Test cricket and 12 since Channel 4 drew stumps on its own free-to-air coverage. Since then, Sky has had the monopoly of rights to what was once the national summer game. This will change a bit in 2020, when the BBC will start showing highlights, and some live T20 games, with Sky retaining the bulk of the rights.

Yet for many, free-to-air cricket coverage is now synonymous with radios Test Match Special. On Thursday, as Test cricket resumes for the first time in this English summer, TMS marks its 60th year of ball-by-ball radio commentaries. This deserves salute from the paper of John Arlott, even while recognising that TMS is a Marmite taste that some adore and others dislike.

Many are devoted to TMSs gentle rhythms in spite of the male public school tone that still lingers. Others want it to converse better with todays Britain. The tension between tradition and innovation will always shape responses to TMS and cricket itself.

In a Wisden survey of TMSs role in the game, Matthew Engel this year highlighted an important truth. While televised sports relentless underlying dogma, especially in Skys treatment, is always that this is thrilling and it really matters, he wrote, radios values are subtler. On radio, there is a tacit pact with the listener, who will be registering the cricket as an agreeable extra in their own daily routine. Exactly so. Just as there is more to life than politics, so there is also more to life than cricket. But its good to welcome TMS back for the summer.

Stevie Ryan chronicled her extensive depression treatments, including TMS, on her mental health podcast – Starcasm.net

Posted: July 4, 2017 at 8:25 am

Saturday night comedian and pop culture commentator Stevie Ryan lost a long and hard fought battle with depression. Stevie has been very open and public about her quest for depression treatment, peppering in tweets about 3-hour-long talk therapy sessions and Instagram posts about transcranial magnetic stimulation (TMS) treatments (above) along with her scathing humor about pop culture and reality stars like the Teen Mom cast and the Kardashians.

Three months ago Stevie Ryan launched a mental health podcast called Mentally Ch(ill) with fellow comedian Kristen Carney. The episodes, which are embedded below, involve frank and humorous about all aspects of living with depression and seeking treatment.

In podcast posted on May 23, 2017, Ryan talks about embarking a month-long set of every day TMS (transcranial magnetic stimulation) treatments. As pictured above, the treatments involve her sitting there with a giant magnet on her brain. According to the Mayo Clinic website, TMS is often used when most other depression treatments have been ineffective.

The magnet is supposed to deliver a pulse that stimulates nerve cells in parts of the brain that regulate mood. I just have to sit there for a half-hour while this magnet just pounds on my head, Ryan explained on the podcast. It will move your eyebrows a little.

She went on to say that they fitted her for her personal device by testing to see which part of the brain they could stimulate that would make your fingers and hands start twitching. They told her they could also do that to her legs. Shed only had one real treatment before she already started to see a difference in her mood in the morning. Usually, Ryan said she cried every single morning when she woke up, but the day after her first TMS session, she didnt feel the need to cry. I dont know what it is, Im just so sad inside, she said about why she regularly cried.

In her last podcast episode, which was uploaded two days before her death, Ryan was severely grieving the death of her beloved grandfather. The episode was recorded just a few hours after her grandfather died. Ive just kind of watched him wither away in the past two weeks and its really, really sad to watch someone who youve grown up your entire life and who you see as the manliest man ever just slowly deteriorate. And, its actually the hardest thing that Ive ever dealt with in my entire life, Ryan said through tears. It just sucks and I will miss him forever. This is just a part of life, but I am just worried that this is gonna send me into a deeper depression. Or maybe it will take me out of my depression a little bit to see that life is short and that we just dont have as much time as we think we do with people that we love or care about.

Co-host Kristen Carney said that they had just recorded a very candid episode about suicide, which they couldnt put out because of audio quality issues. She expressed an interest in rerecording the episode, but wondered if the death of Ryans grandfather had changed her views about suicide.

Ryan expressed confusion over how well her mother was handling the death of the grandfather compared to her. Carney recommended that she ask her doctors at her next TMS treatment to possibly get her on some sort of depression medication to stave off a further spiral. Stevie expressed concern about asking for medication because she says on the last visit they wanted to prescribe her Lithium, which she wanted to stay away from because of the side effects like hair loss and weight gain. Ryan had previously been on Prozac.

Carney also suggested that since Ryan was going through the grieving process, she should avoid isolating herself. Ryan replied that for the first time in a while, she did not feel like isolating herself.

Stevies story is a heartbreaking example of some of the painful realities of dealing with mental health disorders like severe depression, especially during very fragile times in our lives. People are not usually open to talking about their feelings or talking openly like like this, Ryan said in the podcasts first episode. The honesty and gallows humor in the podcast is much like comedian Paul Gilmartins Mental Illness Happy Hour, which has been running since 2011.

Another common theme in these two podcasts is the wisdom that professional success and fame is not the answer to happiness or depression relief. Gilmartin earned a good living on the TBS show Dinner and Movie, while Ryan found a lot of success with YouTube which led to her own VH1 sketch comedy show. When I was doing well in life, I was so lonely and depressed, she said of her time on television.

A New Hope For Depression: Part 1 – Boca Newspaper (press release) (blog)

Posted: July 3, 2017 at 8:22 am

By: Raul J. Rodriguez MD, DABPN, DABAM, MRO Special to the Boca and Delray newspapers

Treatment resistant depression is one of the most crippling and emotionally devastating conditions known to man. The loss of hope in ever recovering from a disease that does not respond to conventional treatment and has hopelessness as one of its symptoms can lead to a deadly downward spiral. Many good medications have been developed to treat Major Depression over the years, but sometimes the disease can be too severe and the medicine does not work. Some intensive treatments, such as ECT, are very effective but have severe side effects. This leaves many patients seeking a better solution. This dire need for a better solution led to the development of TMS.

So what exactly is TMS? TMS is the acronym for Transcranial Magnetic Stimulation, which is a cutting edge treatment for depression that involves stimulation of brain tissue using electromagnetic pulses. This noninvasive procedure improves symptoms of depression by activated the parts of the brain associated with mood control and depression. TMS has grown in popularity due to the high degree of effectiveness while having very few side effects and absolutely no circulation of medication in the bloodstream. The process starts with the placement of an electromagnetic coil over the scalp near the forehead. The motor strip, the area of the brain responsible for movement of limbs and muscles, is then mapped out. This is necessary in order to subsequently determine the location of the lateral pre-frontal cortex. The actual procedure involves a delivering a sequence of magnetic pulses to the left lateral pre-frontal cortex to induce stimulation. The magnetic pulses pass through the scalp and the skull to reach the target brain tissue. TMS activates the actual neurons by inducing an electrical current in the part of the brain cell called the axon. This current travels to the nerve cell body and then affects other connecting brain cells. This stimulation ultimately creates the powerful antidepressant effect. TMS is indicated when conventional treatments for Major Depression have not worked. TMS is also an option for individuals who suffer from Major Depression and do not wish to take medications. TMS was approved by the FDA for the indication of treatment resistant depression in 2008. Deep TMS, possible with the newer generation of TMS technology, was approved by the FDA for the indication of treatment resistant depression in 2013.

Deep TMS, also known as dTMS, refers to a more deeply penetrating form of TMS now possible using the newer generation of TMS technology produced by the Brainsway Corporation. Brainsway has engineered the H1 dTMS coil that was approved by the FDA for the indication of Treatment Resistant Depression. The H1 coil produces a magnetic pulse that stimulates a larger area than a traditional figure 8 coil. The stimulation also penetrates deeper, reaching 1.7 cm into the cortex. The benefits of this have to do with both a more complete stimulation of the target area as well as some stimulation of the neighboring areas of the brain that interconnect with the target area. The risk of dramatically losing efficacy from deviating even just 1mm from the target area is greatly diminished using a Brainsway device, as compared to a traditional figure 8 coil, due to the larger and deeper area of stimulation. The treatment session consists of 2,500 pulses delivered over a time span of approximately 20 minutes. This all helps yield higher rates of both response and remission of Treatment Resistant Depression.

bob’s guide to treasury management systems: How to avoid disruption during TMS implementation – Bobsguide (press release)

Posted: June 29, 2017 at 11:24 am

So far in the bobs guide to treasury management systems series, weve looked at how organisations can determine whether they need a TMS, the kinds of systems that are available, and what treasury professionals should consider before making their selection. Now its time for the nitty gritty implementation.

As weve covered in previous instalments of the series, the importance of ensuring a new treasury system is properly implemented cannot be understated. Get it wrong and not only might the new software make everyday operations more cumbersome and less efficient, but the treasury team will be driven back to old systems, and the time and resources taken to install the solution will have been wasted.

You might have chosen the right system, but if its mis-implemented, or ill-implemented, then your user wont be able to use it. Theyll go back to the spreadsheet, says Dimos Dimitriadis, partner and founder of consultancy Treasury Technology Associates.

Its a concern shared by many of you when it comes to TMS implementation. In our treasury management systems survey among bobsguide readers, conducted earlier this year, difficulty integrating new solution seamlessly with existing systems emerged as the top concern about implementing a TMS, with 62% respondents selecting it, closely followed by cost, at 58%.

By working closely with the vendor, and a consultancy if necessary, treasury teams can ensure their new system is optimally configured for their everyday operations. But during this transitional period, which calls upon many different players with many different roles, how can treasury teams avoid disruption to their day-to-day operations? The first step is preparation.

How can my treasury team prepare for the TMS implementation process?

Before implementation even begins, treasury teams must organise which personnel are going to be involved, and who will work closest with the vendor during the process. We covered assembling a team in an earlier instalment of this series. The next step is establishing a relationship with the vendor.

Patrick Cannon, head of professional services and customer success for SaaS provider Reval, says that the key to successful implementation is to involve the vendors professional service team early in the pre-sales process. Engagement starts with an understanding and alignment of the clients vision, he says.

Corporate treasury teams need to have better alignment internally as well, he adds. [They need] a clear understanding and commitment to achieving the business goals in a deployment, top to bottom.

Where projects succeed or fail is measured by the organisations clarity of vision and designation of an executive sponsor there needs to be an internal counterpart to the vendor who works in step to achieving value in the relationship.

Mathilde Sanson, chief client officer at cloud treasury solutions provider Kyriba, says that assuming evaluation of the TMS is complete, and that an extensive list of requirements was identified during that stage, the implementation process will be smoother, guided with clear business requirements.

To prepare for a TMS implementation, she says, a treasury team should understand the strategic functions of the organisation that they wish to automate, where in their workflows they would benefit from increased financial controls, security, auditability.

She continues: Having their requirements understood by the implementation consultants will help to ensure the project is completed according to plan.

Cannon suggests a straightforward way of confirming mutual understanding of the project, and what it is expected to deliver: If I were part of a corporate treasury team, I would want professional services to articulate my vision back to me to make sure they understand what I want to achieve, and leave no stone unturned when it comes to functionality and what the system can do.

What are some setbacks we need to be prepared for?

With so many individuals from different teams involved in TMS implementation, and many moving parts in the system itself, its expected to hit a few obstacles on the journey. What should treasury teams expect, and how can they prepare for any setbacks?

Many project risks can be identified during the discovery process and mitigation plans can be made, Sanson says. Good communication and a prepared client-vendor team can resolve and smooth out most bumps on the implementation road.

For Cannon, sticking to project best practices, including open communication, regular status reports, and strong governance, is crucial for keeping the wheels moving. Dont underestimate the commitment to getting this right, he says.

Depending on the complexity of the system, and the nature of the organisation, it can take six, 12, even up to 18 months to fully implement a TMS. Theres a risk the project will lose momentum, that the project will take even longer, that it will be disruptive, and treasury teams could lose enthusiasm.

Sanson, like Cannon, recommends executive sponsorship and regularly scheduled status calls to ensure there is ongoing triage and prioritisation to meet goals.

She continues: A treasury team should expect to have a clearly defined process with key milestones and checkpoints identified to recognise progress and maintain quality controls.

Lars Schroeder, senior engagement manager at consultancy SkySparc, says that working with a consultancy can take some of the load off the treasury team during TMS implementation, particularly when it comes to testing.

We help with implementing the new system, by allowing our clients to learn the new system but without doing all the testing which can be boring for them, he says.

Five tips for avoiding disruption during TMS implementation:

Sanson has five tips for treasury teams looking to minimise disruption during TMS implementation:

Establish your team lead and main point of contact. This person should be able to validate processes against the state requirements, she says.

Ensure the initial discovery phase is a priority for your internal stakeholders. Having all the requirements agreed upon up front will greatly reduce day-to-day disruption.

Communicate frequently and clearly about reaching check points and expected milestones. Ensuring everyone on the treasury team understands what should be done and when will keep the project on track.

Set aside a time in the day that you can dedicate to the implementation. If you do this proactively, your meetings will be predictable and according to your schedule. If you cannot free up your staff from day-to-day roles then an external consultant should be hired to have the project move forward.

Enable your vendors implementation team to become your trusted partner. Not only will this increase productivity, but you will also gain more out of the process.

Schroeder says that the key to avoiding disruption is involving the vendor in a smart way, but it will depend on your size and ambition, and how much you want to outsource. If you are smart you can outsource the boring stuff and have your key people be involved in the interesting part.

As is the case with many things, preparation is key to minimising disruption during a TMS implementation project. Success in implementation will depend on assembling the best possible team, ensuring everyone on the team has a defined role, and good communication with the vendor throughout the project.
