Treatment That ‘Rewires’ the Brain Could Be Used for Depression – Healthline

An FDA-approved therapy using magnetic stimulation on the brain can be given to people who havent responded well to antidepressants.
When it comes to treating depression, its tough to know what will work for specific people who have the condition.
Antidepressants are commonly prescribed, but between 10 percent and 30 percent of people with major depression who take the medications fail to show improvement.
But now, a new treatment that involves the use of magnetic stimulation to "rewire" parts of the brain has been approved by the Food and Drug Administration (FDA).
It has proven effective in treating people with depression and could potentially be used in other applications, as well.
This treatment, called transcranial magnetic stimulation (TMS), targets specific parts of the brain in a noninvasive way.
The way that we believe TMS works is as a brain network treatment, Dr. Andrew Leuchter, a senior research scientist at the Semel Institute for Neuroscience and Human Behavior, and director of the neuromodulation division at the University of California Los Angeles (UCLA) Health, told Healthline.
As opposed to medications, which kind of work from the bottom up, they work at the level of the nerve cell, and then spread from there, Leuchter said. TMS picks specific nodes of a brain network and introduces energy into those.
The number of clinics offering TMS is growing as the treatment offers an alternative for people who havent responded to antidepressants.
Leuchter says that TMS has been around as a diagnostic and research tool since the mid-1980s.
It was in the 90s that people began to realize there are many ways to stimulate the brain, he said. Of course, electroconvulsive therapy (ECT) had been around forever. But people thought if you can use electricity to stimulate the brain, how about using electromagnetic energy? Pulsing the brain with an electromagnet to try to achieve the same result, but with a much better side effect profile. People began to experiment with it, discover that, in fact, you can provide very focal brain stimulation that seemed to relieve the symptoms of depression without nearly the side effects of ECT. It was studied for a number of years, and approved by the FDA in 2009.
A major advantage to TMS when compared with antidepressants is its relative lack of side effects.
A lot of times, antidepressant medications are used for these different conditions, and they can be helpful, said Leuchter. The problem is, with medication youre commonly dealing with side effects. Youre taking a medication and distributing it throughout the bloodstream, throughout the body. You end up, therefore, with effects on virtually every organ system and in many instances the side effects can be problematic nausea, weight gain, headache, dizziness, those kinds of things.
The nice thing about TMS as a treatment is that you are really applying it directly to a specific part of the brain. It has very, very few side effects. Therefore, we are able to achieve a therapeutic effect for the patient with a much more favorable side effect profile, by targeting the treatment to the precise brain area that we need to hit in order to get the patient some benefit.
Eight years after its approval by the FDA for depression, TMS is available in every major metropolitan area in the United States.
Leuchter estimated there are currently between 800 and 1,000 TMS devices being used in practices across the country.
[TMS devices] are still not as widespread as we would like them to be, in order for everybody to have access, says Leuchter. The good news in terms of access is that insurance companies are now pretty routinely covering TMS treatment for depression. So its something that is more and more widely successful, both because the devices are out there in the community, and because its a covered benefit under a lot of insurance plans, including Medicare.
For those without health benefits, however, the cost can be significant.
While out-of-pocket costs vary by geographic region, a 6- to 9-week treatment of TMS typically costs somewhere in the range of $14,000.
The out-of-pocket costs for uninsured Americans may be higher than the cost of antidepressant medications, but Leuchter says there are a few caveats.
It is important to bear in mind that TMS generally is performed in patients who have suffered for years with unrelenting depression and have failed to benefit from multiple courses of medication treatment and psychotherapy, he said.
There have been a few health economics studies that have looked at the cost effectiveness of TMS treatment the cost per QALY, or quality-adjusted life year and have concluded that TMS is a very cost-effective alternative.
There are more potential applications for TMS treatment on the horizon.
TMS works for a whole variety of different conditions, said Leuchter. Anything where focal brain stimulation might be helpful. So its been used with success not just for depression, but for bipolar disorder, for obsessive-compulsive disorder, PTSD too. Weve used it with some success here at UCLA for the treatment of tinnitus thats chronic ringing of the ears. And it also is quite useful for the treatment of chronic pain conditions. So a variety of different applications are in development.
With Americans spending billions of dollars every year on antidepressants and the country in the midst of an opioid epidemic, TMS offers a possible alternative.
Were excited about treatments like TMS that actually can offer a great deal of relief for chronic pain without the side effects of opioids, said Leuchter. I think that what were going to see going forward is that there will be more and more of those brain stimulation neuromodulation methods that can help reduce pain symptoms with fewer side effects. So were very interested in exploring those avenues of research.

QAD brings personalisation to TMS – Enterprise Times

QAD has announced the latest version of its transport management suite. The software has a dual identity and is already known to many as PRECISION Software as well as QAD TMS. PRECISION was a company that QAD acquired in 2006. The release, V17 delivers three key updates. They are the Effective User, PRECISION Freight Bill & Pay and Local Device Access.
QAD Chief Marketing Officer, Carter Lloyds commented: This latest release continues the evolution of the QAD TMS suite, helping customers effectively meet their transportation, global trade and supply chain management requirements across the enterprise and throughout their supply chain. said.
QAD has introduced role based configurations for the software. There are three categories of roles, although QAD did not reveal in the press release what these roles or categories are.
Once logged in, a user is presented with features specific to his or her role. This includes navigation which varies by role. There is a degree of personalisation where users are able to create and store favourite links and to select their own home page.
This appears to fall short of full personalisation. While most companies will find these settings useful, there may be instances where a user has multiple responsibilities and where there is not a perfect specific role or match. The ability to create favourites may alleviate this.
Freight Bill Audit & Pay (FBAP) is a new addition to the suite. Its purpose is to help companies with the review and payment of carrier invoices. It compares the original estimated costs with the final costs. Those estimated costs are normally sourced either by QAD TMS, generated during the shipment execution phase or from an external source.
The system compares the actual with the estimated figures and will raise an exception where the difference is outside of predetermined tolerances. If within tolerance the invoices is authorised and released to the payment system. This automation can significantly reduce the time that staff take to match freight invoices.
This is not a new feature exactly but it enables the continued support of locally attached devices such as weighing scales and printers. Historically this was supported through the use of applets in browsers. Browsers are stopping the support of applets.
To address this QAD has developed an alternate method for the connection of such devices. This allows users to continue to use these devices for label and document printing.
The functional improvements within this latest release are limited to the personalisation and the new FBAP module:
Overall V17 is not a major release in terms of content. If driven by customer needs for continued support of local devices and adding some function, it makes sense.

North Texas students compete in solar car challenge at TMS – Story … – FOX 4 News

DALLAS - Teams from across the United States are competing in a solar car challenge going on now at Texas Motor Speedway.
The event is the largest solar car race in the Western Hemisphere. Thirty-six teams, including several from North Texas, are competing in four divisions.
One of the teams is from Coppell High School. Senior Mackenzie Becker talked to Good Day FOX 4 about the opportunity.
Its really, really cool to be out here. I know its an amazing experience for everyone to see all of this. And its kind of all of our dreams. I mean, weve been working on building cars for years now and being able to bring it out to place where cars are raced and its famous here is really an amazing experience. And wicked cool for all of us, she said.
Each year the competition rotates between a closed-track race at Texas Motor Speedway and a cross-country event. Most cars average about 20 miles per hour.

The cloud-based transportation management software provider Kuebix has created a direct integration with the international freight marketplace SimpliShip, the companies said Tuesday. The integration allows Kuebix users to pull air and ocean freight rates from SimpliShip directly into Kuebixs planning and execution platform. Kuebix will utilize SimpliShips international freight rate application programming interface (API). The integration allows Kuebix to provide procurement capability in international modes to complement its existing capability in domestic modes, and gives SimpliShip access to Kuebix customers that require international rates. SimpliShip Chief Executive Officer Cory Margand called the integration the first step in unlocking the true potential for APIs in international logistics. The days of having to access multiple platforms to manage domestic and international shipments are over. Margand is a former logistics executive with Adidas, and founded SimpliShip to provide a platform to make the procurement of freight forwarding services easier for shippers. The use of APIs to connect logistics and freight transportation systems is steadily growing, especially among the newer breeds of systems providers. APIs are essentially a way for two systems to communicate with one another in a more flexible, real-time, two-way manner, relative to electronic data interchange (EDI). EDI is a more standardized method of conveying information like rates, transit times and invoice information, but that standardization can be perceived as rigidity for some uses. EDI messages are also sent in batches, creating data latency, and EDI integrations can be costly and time-consuming. Kuebix President Dan Clark called the SimpliShip integration a great addition to the Kuebix ecosystem as we continue to expand our global network of providers to maximize visibility and efficiency for domestic and international shippers. Kuebix is a fast-growing TMS provider primarily focused on the small- and medium-sized enterprise segment, but with growing revenue from larger enterprises. The [Kuebix] TMS was built on the Salesforce platform and allows clients to implement the solution very quickly with the core TMS, then seamlessly add premier apps and integrations as needed, Gartner analyst Bart de Munyck wrote about the company in a TMS market study in March. Kuebix focuses on both the SMB and the enterprise market and has over 1,500 customers. Although 90 percent of Kuebix's customers are small (less than $25 million in freight under management), the company is growing on the large enterprise client side. Kuebix offers managed service programs to businesses looking to partially or fully outsource transportation management.

Getting Paid: Why AR Matters in your TMS – Logistics Viewpoints (blog)

Figuring out the right TMS for your business typically boils down to your vendors data structure, which affects every facet of the software. One way to know whether the TMS will work for your needs is to look at its accounts receivable capabilities (AR). This is where the data structure really comes to light.
Whats so important about AR?
Think of an AR record as a sell-side load. While a load is essentially an intermediate point between an order and a carrier freight bill on the buy-side, an AR record is the intermediary between an order and a customer invoice. Both buy-side and sell-side charges are allocated back to the order. An AR record is the primary entity against which sell-side rating occurs for the purpose of invoicing customers.
The flexibility with which a TMS allows you to create and manage AR records drives how fast, easily and accurately you get paid. But the truth is that many TMS AR record parameters wont match up with the realities of your business, so taking a hard look at them will quickly give you an idea of whether youre considering the right TMS.
Precise rating and billing by order, shipment or load
Determine exactly how not just whether the TMS enables you to rate and invoice in multiple ways. For any given load and client, your TMS should be able to create an AR record at the order level, shipment level or load level.
The existence of the sell-side load allows you the flexibility to consolidate multiple clients on a load while still billing each client independently at the load, shipment or order level.
Lets say you have a load originating out of Dallas and it has four different stops:
At Stop #1, you have to bill two different divisions within the same company. At Stop #3, theres an order for a different customer entirely. Some orders will need to be billed separately to different parties, and so require different ARs.
A TMS should allow you to bill several different ways in this situation: If you choose ordered-based invoicing, you will have six AR records (one for each of your six orders associated with the load). If your TMS allows for shipment-based invoicing, you will still receive six ARs because the bill-to location and/or client are different on some of the orders so they will be split into their own AR records. For load-based ARs, you would receive three ARs: One for each combination of client and bill-to location.
Rating and allocation
Once an AR is created, the next step is rating and allocation. AR records can be rated in multiple ways:
Once rated, the AR records charges are allocated back to the order associated with it. The allocation process is used to support both the buy and sell-side rating processes. It determines how the calculated buy/sell charges should be allocated to and from the orders associated with the ARs, and therefore determines a loads profitability and margin. A TMS should give you multiple options for allocations and run them automatically as needed, such as when a load cost changes or order charges change.
By giving you several allocation options including capabilities for determining shared savings for any customer programs a TMS gives you maximum power in determining the real-time profitability of your loads and flexibility in how you invoice. This is critical to managing the logistics of todays transportation industry and taking advantage of more complex (but profitable) distribution methods, including pool distribution.
AR matters
The capabilities of a TMS matter little if you cant bill your customers according to the realities of your business. By examining the AR capabilities of a TMS, you can determine not only what the software is capable of but how it will help you solve your challenges efficiently and profitably.

TMS activity gathers pace – The Maritime Standard (registration)

The Maritime Standard (TMS) is preparing to stage three important events in the second half of the year, strengthening its market position as the regional market leader for shipping related awards and conferences.
The TMS Awards will take place at the prestigious Atlantis, The Palm, Dubai, on Monday 23rd October. Nominations are now open and companies and individuals have until 10th August to submit their entries.
