TMS Treatment for Autism: What is it and Does it Work? – Autism Parenting Magazine

Talk about transcranial magnetic stimulation, or TMS therapy, is raising hopes of parents who wonder if the treatment could work for their child with autism spectrum disorder (ASD).
Parents with children on the spectrum are wary of the term cure. For some, its because theyve been disappointed by snakeoil and false hope more than they care to remember; for others, the term cure seems more fitting to a disease. When you talk with parents of kids on the spectrum, most are willing to do anything to help their children thrive while firmly embracing who he/she is.
And yet, even autistic adults who celebrate the beautiful difference of their minds, admit that certain symptoms associated with autism create difficulties in everyday life. For example, those on the spectrum often have trouble with social situations and communication. Addressing these challenges in a way that embraces who the child is, but modifies behavior to make life easier, seems like a worthy (and lofty!) goal.
Some see transcranial magnetic stimulation (TMS) as a treatment worthy of optimism. While researchers urge caution, studies suggest promise and both diagnostic and therapeutic potential (Oberman et al., 2016).
TMS or transcranial magnetic stimulation, is a treatment that directs recurring magnetic energy pulses at specific regions of the brain involved in mood control. The definition sounds a little frighteningenergy or magnetic pulses entering the brain brings up images of inhumane psych wards in horror movies. But actually the treatment is noninvasive, with relevantly little discomfort, and its helped many patients with debilitating depression.
TMS therapy (or treatment) uses short pulses of magnetic fields to stimulate nerve cells in specific areas of the brain, like the prefrontal cortex which is involved with mood regulation.
When mentioning TMS, people are most familiar with its potential as a treatment for major depressive disorder, but its therapeutic benefits are being investigated for other conditions too. In this regard, researchers believe TMS may possibly alleviate symptoms associated with obsessive compulsive disorder (OCD), autism spectrum disorder (ASD), attention deficit hyperactivity disorder (ADHD), anxiety, and posttraumatic stress disorder (PTSD).
When someone undergoes TMS, the person administering (usually a psychiatrist, physician, or sometimes a clinical psychologist) presses a device to the left side of the patients head, and starts the process of moving magnetic fields into the brain with the flip of a switch.
These magnetic fields produce tiny magnetic currents as they move into the brain. The currents activate cells to release neurotransmitters like serotonin and dopamine. The release of these feel-good chemical messengers may restore balance in the brain which may bring relief for those suffering from depression.
As individuals with autism may have low brain serotonin concentrations (Ademsen et al., 2014), its no wonder researchers see promise in the treatment for those with autism spectrum disorder (ASD).
Most doctors state sessions lasting between 30-40 minutes are best, with a recommended 30 sessions over the course of six weeks. Patients usually experience relief (like an elevation in mood) within two to four weeks of the treatment commencing.
Although TMS is FDA approved, this authorization applies to administration in adults (between the ages of 18-70) and only for severe depression that doesnt respond to other medicationsFDA authorisation now applies to other conditions like OCD too. Should a child therefore undergo the treatment for autism, it would be considered off-label.
While TMS may sound good in theory, parents are obviously concerned about any type of therapy administered to their childs brain. However, a study (Yang et al., 2019) reported significant reduction in social and language related symptoms after children with ASD underwent TMS treatment courses. The study also reported possible improvements in cognition and imitation as reported by caregivers. The study saw promise in TMS to improve core deficits in low-functioning children with ASD.
Even though suggestions of success are encouraging, parents also want reassurance that the treatment does not have serious side effects. In addition, parents may wonder about the level of discomfort and the sensory implications of the therapy.
As mentioned, much research is still needed in terms of the application and side effects of TMS, especially when administered to children on the spectrum. In the meantime, studies pertaining to the effects of TMS on neurotypical children may be an appropriate source.
In a study by Krishnan et al. (2014) noninvasive brain stimulation techniques such as TMS were examined to assess the treatments safety when administered to children and adolescents.
The study found repetitive TMS to be safe in children and adolescents when safety guidelines were followed. When side effects were present, it seemed similar to side effects experienced by adults. Out of 513 of the children (or adolescents) involved in the study, 11.5% experienced headaches, 2.5% experienced scalp discomfort, 1.2% experienced twitching, and a very small percentage experienced mood changes, fatigue and tinnitus (Krishnan et al., 2014).
When looking at anecdotal evidence of pain and discomfort experienced, when undergoing TMS treatment, the verdict seems positive. The treatment is generally well tolerated and some feel the lack of serious side effects make it a great alternative for those sensitive to medication.
Parents should, however, be aware of reports of patients experiencing pain, especially at the site of stimulation. Croarkin et al. (2011) detailed the pain experienced (during TMS administration) by two children aged 10, and an adolescent aged 17. The 17-year-old mentioned the pain felt like a headache, sort of like a migraine behind my eyes.
This high level of pain experienced may be rare, but parents of children with ASD should be aware of the worst case scenarioespecially when children with sensory difficulties like tactile defensiveness are involved.
An aspect of TMS that probably needs more investigation, especially for children with autism, is the risk of seizures. As children with autism have a higher chance of being diagnosed with epilepsy too, this concern should be addressed with a qualified professional before deciding on the appropriateness of TMS.
Stultz et al. (2020) produced a literature review of the safety of TMS with respect to seizures and mentions that TMS has been successfully used in patients with epilepsy and traumatic brain injuries. It is further mentioned in the review that seizures are a rare but serious side effect of TMS, but the risk may be low at less than one percent.
There may be further good news, as a new treatment combines TMS with Quantitative Electroencephalogram (qEEG) and Electrocardiogram (ECG/EKG) to deliver a tailored treatment to the patients unique brain pattern.
The fact that this personalized protocol, known as Magnetic e-Resonance Therapy (MeRT), ascertains the individuals brain patterns, means greater success may be achieved in restoring chemical imbalances.
For parents who would like to find out more about MeRT, specifically for a child with ASD, the following story of Frankie (who stopped toe-walking and learned to talk after his family discovered MeRT) will be an inspiring read.
Experts agree that, although TMS and specifically MeRT appear promising, research is urgently needed to establish efficiency and the long term effects of the treatment. Researchers are hopeful that future studies will find proof that TMS and MeRT are effective in treating certain ASD symptoms like social and language deficiencies.
The TMS journey of Kim Hollingsworth Taylor (founder of the Clearly Present Foundation) became an internet cautionary tale for the extreme highs and lows she experienced when her son underwent TMS. The treatment relieved some of her sons autistic symptomsbut only temporarily. This only highlights the fact that more research is needed, especially concerning long-term outcomes of TMS.
When a study (Sokhadze et al. 2018) lists significant improvements in measures of executive functions and behavior in kids with autism, parents may rightfully experience a surge of hope. Researchers do urge caution, however, as the one thing they seem to agree on is that many more clinical trials and studies are needed to establish the effectiveness of TMS for core symptoms of ASD.
If youve read the research and you believe your child may benefit from TMS or MeRT, the best course of action would be to see your physician or a psychiatrist. These medical professionals will take your childs medical history (and conditions like epilepsy) into consideration before helping you decide if it is the right course of action for your child.

The use of transcranial magnetic stimulation (TMS) has a modest yet promising history in suicide reduction, according to the authors of a study published in Neuromodulation.
In this seedbased functional connectivity analysis, Jennifer Barredo et al evaluated whether changes in frontostriatal functional connectivity would accompany suicidality reductions following TMS, with the aim of optimising future treatment. This study was a secondary analysis of an open-label trial of 5Hz TMS of the left dorsolateral prefrontal cortex (DLPFC) in patients with post-traumatic stress disorder (PTSD) and major depressive disorder (MDD).
A total of 25 participants were included in this secondary study. For inclusion in the parent study, participants were required to meet criteria from the Diagnostic and Statistical Manuel of Mental Disorders, fifth edition (DSM-V) for both PTSD and MDD. Patients overall illness severity rating were at least moderately ill on the clinical global impressions-severity scale for each diagnosis. However, actively suicidal intent and suicidal planning were exclusionary.
Patients received eight weeks of 5Hz TMS on the left DLPFC. Clinical symptoms were measured on the Inventory of Depressive Symptomatology-Self Report (IDS-SR) and the PTSD checklist from the DSM-V (PCL-5). Investigators derived their definition of suicidality from IDSSR item 18, which is a thoughts-of-suicide item. Possible responses to item 18 include: (0) no thoughts of death or suicide, (1) feeling that life is empty or wondering if it is worth living, (2) thinking of suicide or death several times a week for several minutes, or (3) thinking of suicide or death in detail multiple times each day, having a specific suicide plan, or having made an attempt.
Functional magnetic resonance imaging (fMRI) was collected before TMS, and at the treatment endpoint. Researchers entered these data into analyses of covariance, evaluating the effect of suicidality change across treatment on striatal and thalamic functional connectivity. Changes in other PTSD and depression symptoms were included as covariates, and results were corrected for multiple comparisons.
Suicidal ideation of any severity was reported by 68% of participants at study baseline, with 10 participants reporting a score of one, five participants reporting a score of two, and two participants reporting a score of three. Researchers found that after TMS, suicidality decreased by a minimum of one point in 65% of participants. However, they did also see an increase of suicidality in 12% of patients, including on one participant who did not endorse suicidality at baseline. Across participants, the mean change in overall PCL-5 and IDSSR scores was 41.8% and 42.6%, respectively, in this subset of the original study cohort. In the parent sample, the average decrease in PCL5 was 35.5% and IDSSR was 37.6%.
In their conclusion, the studys authors commented: Collectively, findings argue that the precise targeting of frontopolar circuits via diffusionguided individualised targeting may enhance neuromodulation delivery precision to our highestrisk patients. Combing these methodologies with innovations in 5Hz TMS delivery, such as pulse energy optimisation, may further improve its efficacy as an antisuicidal treatment.

Revenues missed expectations somewhat, coming in at US$38m, but statutory earnings fell catastrophically short, with a loss of US$0.058 some 331% larger than what the analysts had predicted. The analysts typically update their forecasts at each earnings report, and we can judge from their estimates whether their view of the company has changed or if there are any new concerns to be aware of. Drivers concerned by prospect of no-go zone at Texas – RACER

Last weeks NTT IndyCar Series testing at Texas Motor Speedway prompted some concerns by drivers about the potential quality of racing for the doubleheader on the 1.5-mile oval on May 1-2, related to the groove that sits a full lane up from the bottom of Turns 1 and 2.
The area, still noticeably stained by PJ1 Trackbite, an adhesive compound used in the fall of 2019 by the NASCAR Cup Series, was removed and washed thoroughly before last years IndyCar race in June. That 200-lap contest was already hampered enough as a one-day show with limited on-track running due to the impact of COVID-19, but drivers had to cope with a tricky high groove in Turns 1 and 2 that discouraged side-by-side battles. This bogged down the action the event had just five lead changes between three drivers, along with 180 passes for position throughout the field of 23 starters.
IndyCar spent both Tuesday and Wednesday at TMS, with the second day emphasized as a more traditional test day that featured 17 drivers taking part. When asked about the grip level in the opening pair of corners, there was a near-unanimous consensus among the drivers.
Its a no-go zone, said Graham Rahal, 2016 winner at TMS and driver of the No. 15 Rahal Letterman Lanigan Honda. When they repaved the track and used the lime wash, it was slippery. You know, the first Cup race they raced here, a lot of guys crashing, a lot. I remember talking to Jimmie (Johnson) about it. Its just very slippery. Everybody here at TMS does a great job trying to find ways to find grip. Unfortunately, the dark black stuff, from the data we got, is about 20 percent less grip than the bottom lane and a half, so its still going to be a no-go zone, unfortunately.
Pato OWard, who ended up pacing the day overall at 222.25mph, was bold enough to try the lane during some single-car runs in the morning, but found it difficult to make his No. 5 Arrow McLaren SP Chevrolet stick.
I tried opening up the radius into (Turn) 1 a couple of times and the car wasnt very happy, said OWard. Usually, you can sort of feel it out whenever it might come in, but honestly, for what I have felt up until now, its not going to grip up. We need that to grip up in order for us to have a good race. The other thing that might help racing is see how the tire deg [degradation] is going to be. (Wednesdays) a little funky because theres so much downforce. Its very windy, but there is so much more downforce because its cold, and were all flat around the place and it usually isnt easy flat. You have to kind of take a leap of faith and not everybody does it in qualifying.
I think last year, Josef (Newgarden) was the only one that did it. Were all in a situation where were just getting all this extra grip that I dont think itll be here on race weekend it might, but I feel like in that area, it might make the racing very difficult if we dont have a second lane. We truly need that in order for us to either have chances to pass and (put on) a good show.
OWard added that his attempt to push into the stained lane was only with the right-side tires.
I dont know how else to say it, but I havent had the cojones to go four wheels on the black, said OWard. Any time you try and get just a little bit of clean air on your wings behind someone, you lose like 10 car (lengths) because you just plow through. Its obviously very green up there. It needs more rubber. The problem is we dont know if itll rubber in. Were just going to have to see how everything plays out and if people start using that second row.
However, there is some optimism this time around with an improved aero configuration, which RACER understands to be more than 150 pounds of downforce versus last years package at TMS. Although Newgarden, a two-time IndyCar champion and 2019 winner at Texas, couldnt give an exact number, he confessed it is within a couple hundred pounds.
I think its possible to open up that lane, more so now with the aero configuration that were going to be running or potentially running for the race weekend, said Newgarden, driver of the No. 2 Team Penske Chevrolet. Its a bit more downforce, its more consistent, it seems like you can follow a lot closer. If thats the case, people will be more open to driving up into the lane. I think at the moment its not there. We need people to go up there and clean off the track and start laying down rubber. So I think the likelihood of using it is higher, but I think the reluctance of using it is still there at the moment.
Will Power, a two-time winner at TMS, had another suggestion.
If you did a session, lets say, 20 minutes leading into the final practice of the race weekend and everyone gets a set of tires and they can only run the second lane, and that segued into final practice, then you get cars running both lanes, said the driver of the No. 12 Team Penske Chevrolet.
As they get to someone, they just wouldnt stall out. Theyd go up there. So I think that would almost fix the problem. Weve added downforce, the cars are easier and more stuck. You can even do a short line at the moment when youre running by yourself, but if you cant go to another lane, how can you ever pass?
It has been an uphill climb for TMS since undergoing a reconfiguration and repave in 2017 to aid the tracks drainage issues. The renovation reprofiled Turns 1 and 2 widened from 60 feet to 80 feet, and banking in that section decreased from 24 to 20 degrees.
Rahal acknowledged the tracks continued efforts to improve the racing for both the Genesys 300 and XPEL 375.
If the downforce levels are correct and the tire combination is correct and you have kind of an evening race on both days Sunday not so much but on Saturday that gives you an opportunity for the track to cool down and be good conditions to let you go race anyway, said Rahal. I think well be able to put on a better show than what we had the last year or the time before that. But we have to learn.
Everyone at TMS is responding. Im sure when they repaved the track, they thought it was going to be the perfect combination and it wasnt. They know that, theyve acknowledged that, and I think everybody here has done a great job to try to adjust to that and make it racy again and I think its getting closer.
