Nashville SC interim head coach Rumbani Munthali aims to get a more recognizable look about the team as he prepares for his second game in charge against CF Montreal.

Gary Smith, who led Nashville for seven years, made a shock exit following their 2-0 win over Toronto FC on May 16, with Munthali stepping up from player development coach to take his place.

Nashvilles first game under him ended in a 1-1 draw with Atalanta United on Saturday, leaving them 11th in the Eastern Conference.

With little time to put his stamp on the team, Munthali believes being able to welcome back injured players will help to get them back on track.

"I think as you see players come back into the team [from injury], you see that we're starting to be a little more recognizable as a team that went on a 10-game undefeated streak last season," Munthali said.

"What I would hope is you get a team that represents what the club is: hard-working, forward-thinking and positive, and I hope that's a team we can put onto the field that will compete from start to finish.

"I think those are the cornerstones of who we are as a club."

Montreal are on a four-game losing streak that started with a 4-1 defeat to Nashville, and head coach Laurent Courtois accepts they need to do more to get back to winning ways.

"Doing my 'job' is not enough. We have to be ready to offer a little 'extra' and help each other out. For the moment, in fits and starts, I have the impression that we are just doing our job.

"Now, we are doing our mea culpa, we are coming back to things that we thought we had figured out."

PLAYERS TO WATCH

Montreal Sunusi Ibrahim

In the absence of top-scorer Matias Coccaro, Ibrahim netted Montreals lone goal in their defeat to Toronto last week. Montreal have scored in all four games during their current losing run, with the Nigerian extending that run.

Nashville Hany Mukhtar

Mukhtar attempted five shots, including scoring Nashvilles lone goal from the penalty spot, against Atlanta United on Saturday, taking him to 404 career shot attempts in regular-season MLS play. No other player has attempted 300 shots since Mukhtar joined the league in 2020.

MATCH PREDICTION: DRAW

Montreals lone win in eight all-time meetings with Nashville SC came in their last home match against them, a 1-0 win in June 2023. Nashville has won five of the other seven meetings (D2), including a 4-1 victory on May 4.

However, Nashville have won only one of their last 15 MLS away matches (D5 L9 incl. playoffs) dating back to early June 2023. Nashville scored three goals in the lone win in that time (3-0 at Kansas City) but have totaled just five goals in the other 14 matches.

Meanwhile, Montreal have lost four straight matches within a single regular season for the first time since September 2020. They havent lost five in a row in a single regular season since a five-match losing run to end the 2017 campaign.

Montreal is struggling, winless in its last eight games. In the last week, the club lost 5-1 in a derby match at Toronto FC before falling 2-1 to lower-tier Forge FC of the Canadian Premier League to drop out of the Canadian Championship quarterfinals.

The supporters' sections performed a silent protest during the first half against Forge. By the end of the match, they were booing and exchanging words with Piette with one set of fans holding a white tarp with "INACCEPTABLE" written across it.

"They expressed their displeasure about the desire, the commitment," Piette said Friday, calling the interaction a good discussion. "For them, results are important, but secondary. It's more about the effort, and theyre right."

Montreal (3-7-3) will try to get back on track Saturday when it hosts Nashville SC (3-4-6) at Saputo Stadium.

Piette said he tried to remind the fans that as difficult as it is for them, the players are taking it just as hard.

"We're not proud of ourselves we want to try to turn things around," he said. Its difficult because our confidence is not at its highest.

"We feel like we're walking on eggshells because our confidence is so fragile right now."

Montreal last won a match at its home opener on April 25 and has been outscored 20-9 in all competitions since.

Injuries have taken a toll on a club with the second-lowest payroll in MLS. Eight players are out, including top forwards Matias Coccaro and Josef Martinez.

Piette wouldn't use that as an excuse and said some players arent giving enough effort.

The 29-year-old said the team is giving opponents too much space and offering up easy chances that Montreal hasnt in turn been able to generate for itself. Hes also sensed that his squad lets itself down after conceding a first goal, allowing opponents to quickly get a second and third.

"I dont think its all 11 players, Piette said. There are some who have love for this club, who sweat for the shirt no matter how we play performance-wise. There are some right now honestly who are performing less well, there are players who are less consistent.

"Theres a lack of intensity, in pressing, in challenges, in aggressiveness towards the ball carrier," he added. "We're missing a bit of putting on our work boots, our helmets, doing the dirty work. Lately thats what's missing, and we need to get that back."

Amid the run of bad form, the club is also in turmoil off the pitch.

Montreal parted ways with sporting officer Olivier Renard on May 9, less than two weeks after news surfaced that team MVP Mathieu Choiniere reportedly requested a trade amid a contract dispute, raising questions about the clubs chain of command. Renard said last week the two sides were no longer on the same page.

Piette said he feels especially bad for first-year head coach Laurent Courtois amid the difficult time at the club.

The top coaching job in Montreal has been a revolving door with Courtois becoming the 10th head coach appointed since the team entered MLS in 2012.

After a strong start to the season, the 45-year-old from France is suddenly managing a sinking ship thats conceded the second-most goals in MLS, but he's trying to stay positive.

"The experience I have here coaching this group, in this club, for the people who support us, is a privilege every day even if the last few weeks have been complicated, Im living a dream, Courtois said. But it hurts, it hurts for the supporters."

Courtois has repeatedly taken the blame for Montreals shortcomings.

"Its my job to find a way to make these lows less low," he said. "But Im also sure of what these guys are capable of doing well.

"I'm sure there will be a reaction and in the coming weeks we will be able to rebuild on positive things."

Meanwhile, Nashville (3-4-6) has lost three straight on the road and fired head coach Gary Smith last week, albeit after a 2-0 home victory over Toronto. Nashville also contributed to Montreals misery with a 4-1 win on May 4.

On Saturday night, Montreal hopes to have the fans behind the team as it seeks revenge.

"We need them, but at the same time we understand that weve disappointed them a lot," Piette said. "Its up to us to earn their trust."

Therapy for bacterial lung infections in CF moves closer to testing – Cystic Fibrosis News Today

Evoq Bio said it successfully completed a meeting with the U.S. Food and Drug Administration (FDA), held to discuss the development of its inhaled nanoparticle therapy for treating bacterial lung infections in people with cystic fibrosis (CF) and other lung disorders.

According to the company, a subsidiary of Evoq Nano, the FDA responded favorably during the pre-investigational new drug (pre-IND) meeting. An investigational new drug application, or IND, is a formal request to the FDA asking for permission to start clinical testing, or trials in people, of an experimental therapy.

The positive response from the FDA to our nanomedicine platform is a major milestone in the development of a new treatment for patients with cystic fibrosis and other bacterial lung infections, Shaun Rothwell, CEO of Evoq Nano, said in a company press release.

The release stated that Evoq Bio now is confidently advancing toward Phase [1] clinical trials.

CF is a genetic disorder characterized by abnormally thick and sticky mucus in the lungs and other organs. This thick mucus can serve as a haven for infectious bacteria, and chronic lung infections are a common problem in people with CF.

Moreover, infections with bacteria that are resistant to commonly-used antibiotics are becoming increasingly more prevalent, underscoring a need for new treatment options.

Evoqs new therapy, dubbed EVQ-218, is a silver nanoparticle molecule thats been shown in lab experiments to be able to kill a range of problematic bacteria, including Pseudomonas aeruginosa, that frequently cause chronic lung infections in people with CF. Called P. aeruginosa, this bacteria is commonly found in the environment.

According to the company, EVQ-218 also is able to kill other single-celled organisms like yeast, but it has not demonstrated any toxic effects against lung cells in preclinical tests.

Further, EVQ-218 also can penetrate more than a dozen different biofilms thick, protective coverings that infectious lung bacteria produce to help them evade destruction. In lab experiments, bacteria did not develop resistance to EVQ-218 after nearly a month of treatment. In similar experiments with conventional antibiotics, resistance can usually be detected in less than a week, according to Evoq.

The company is developing an inhaled version of EVQ-218 to be delivered directly into the lungs as a way to combat bacterial lung infections. While Evoq now is planning early clinical trials, it did not provide a specific timeline for the therapys development. To date, the company has received two grants from the Cystic Fibrosis Foundation to develop an inhaled therapeutic using EVQ-218.

Inter Miami (8W-2L-4D) will enter the weekend's matchup against D.C. United first in the Supporters' Shield Standings with 28 points (one point ahead of second place) and leading the league in goals with a total 35 goals (10 more than the next team).

Striker Luis Surez leads the team in goals this regular season with 11 and is tied for first place among the league top scorers. Captain Lionel Messi, meanwhile, is the team's second leading scorer with 10 goals and is tied for second among the league's leading scorers. Additionally, Messi leads the team and the league in assists this regular season with 12.

Messi and Leonardo Campana are the two active leading scorers in Club history, with Campana having 25 across all competitions (22 in MLS) and Messi 23 (11 in MLS). They trail Gonzalo Higuan who scored 29 times for Inter Miami (all in MLS). Messi, meanwhile, leads the team with the most assists in Club history with 19 across all competitions (14 in MLS) and versatile attacker Robert Taylor is second with 17 across all competitions (11 in MLS).

Goalkeeper Drake Callender and Taylor are tied as the players with most appearances in Club history with 90 across all competitions. In terms of MLS appearances, Victor Ulloa is first with 74 (one in playoffs), Taylor second with 72 (one in playoffs) and Callender third with 71 (one in playoffs each).

Saturday's matchup will present the second matchup between the sides this regular season. Last time out, Inter Miami claimed a 1-3 win on the road over D.C. at Audi Field March 13 with a brace from Surez and a goal by Campana.

Overall, Inter Miami and D.C. United have previously faced off eight times in MLS regular season action, with a record of two wins, four losses and two draws for Inter Miami. Campana, meanwhile, will look to add to his tally against D.C., as the Ecuadorian striker has scored four times in the five previous matchups between the sides he's featured in.

The match may also present a reunion for a former Inter Miami player. Current D.C. United defender Chris McVey may once again play at Chase Stadium after being an Inter Miami player for the 2022 and 2023 seasons.

Additionally, Surez and D.C. striker Christian Benteke may face each other once again after previously matching up three times in English Premier League action while Surez played for Liverpool and Benteke was on the books at Aston Villa.

D.C. United will visit South Florida after most recently falling 1-4 at home to the New York Red Bulls on Wednesday. In all, D.C. have recorded four wins, four losses and five draws for a total 17 points this regular season and sit eighth in the Eastern Conference standings.

Forward Benteke leads the team in goals with 11 and is tied for first among this regular season's top scorers, while midfielder Jared Stroud is the team's top assist provider thus far with seven.

Trauma and resilience are 2 sides of the same coin in the CF community – Cystic Fibrosis News Today

Living with cystic fibrosis (CF) feels like riding on a never-ending roller coaster. There are highs and lows, unexpected pitfalls, and twists and turns around every corner. Like a roller coaster, the paroxysmal journey of CF leaves us feeling ill much of the time. But life moves forward, and whether were sick or not, we must move ahead with it.

CF has abruptly changed my schedule and plans too many times to mention here. Acute pulmonary exacerbations, bowel obstructions, kidney failure, and dehydration episodes that appear seemingly out of nowhere cause me to drop everything in my life at that moment to get treatment and rest. This is the nature of CF, over and over and over. We patients know it all too well.

CF could easily pummel us to the ground, both physically and mentally, if we let it. Although there is time for anguish and even despair at times, when you face a lifetime of this sentence, the only option is to find a way to cope and keep fighting. At some point in the journey, resiliency is born.

Many people without health issues have experienced challenges and difficulties, thus becoming more resilient. They bounce back stronger due to the tough situation theyve experienced. Many of these people may have even had a traumatic experience and were able to mentally and emotionally cope and adapt, making them more resilient. It is viewed as a strength, and I agree. When living with cystic fibrosis, this is a constant cycle.

I recently stumbled upon the term trauma resilience, which resonates with me. We CF patients constantly must withstand physical, mental, and emotional trauma, along with multiple medications and their side effects, knowledge of a shortened life expectancy, continuous doctor appointments, endless procedures, hospitalizations, surgeries, and organ transplants. Even financial trauma can happen, with the stress of how to cover exuberant health insurance costs and copays. Worst of all, we must watch CF take the lives of others in our community. From childhood to adulthood, these things fill our lives.

These traumas not only affect those living with CF, but also everyone around us, including our parents, grandparents, siblings, spouses, children, friends, and even our physicians and others on our medical teams. Each of these perspectives is different, but everyone who cares about us witnesses our traumas and is affected to some degree. CF causes a domino effect and is brutal on everyone involved.

With that said, how we come together as a community and support one another is utterly remarkable. I see it every day on social media, and it touches me deeply. There is never an unkind word, only support. People share resources for all of our family members. Much of it originates from the CF community itself, with the goal of helping one another. These resources continue to multiply as more people realize how long CFs tentacles are.

I am thinking about the resilience of our community this month because May is Cystic Fibrosis Awareness Month. I am in awe of how this community as a whole shoulders this burden. Being able to cope and move forward in the face of trauma is what we do.

I am eternally grateful for each and every one of you, as this community is the backbone of my own trauma resilience.

According to the latest report from CIES Football Observatory, a prestigious international website specialising in football data, Valencia CF are the second most stable team in LALIGA EA Sports 23-24. This document takes into account the number of players used in the last 3 years, the ages of the players (percentage of minutes played in the current season by those aged 31 or over, excluding goalkeepers) and the contract policy (percentage of minutes in the current campaign by players whose contracts expire in 2025 at the latest). The Valencia squad have a continuity rate (average of the above variables) of 84.9%.

It is worth noting that Valencia have recently entered the top 5 clubs in the 5 major leagues that have used the most players from their youth academy (34%) in the last year and the youngest team on the pitch.

The Inter Miami CF Academy presented by Baptist Health is set to host the inaugural edition of the Youth International Cup this upcoming Memorial Day Weekend from May 24 to 27. The Inter Miami CF Youth International Cup will be open for boys and girls teams from the U-9 through U-19 age groups and will offer a high level of competition, with the tournament drawing teams from across Florida, the United States, and internationally.

Teams in the inaugural Youth International Cup will be competing in one of four levels of competition - MLS NEXT (only for U-13 to U-19 age groups), Gold, Silver and Bronze - with matches set to take place at Inter Miami CFs the Florida Blue Training Center in Ft. Lauderale, as well as other facilities and complexes across Broward County. Group play games will be played on Saturday and Sunday, May 25 and 26. Teams will have the option to play on Friday, May 24. All Finals will be played on Monday, May 27.

This past week in Luxembourg, Valencia CF, TM Grupo Inmobiliario, LALIGA, INNSIDE by Meli Luxembourg and the Luxembourg Chamber of Commerce continued their international tour discussing sustainable tourism. Businessmen, investors, clients and institutional representatives of the country were present at the event.

TM Grupo Inmobiliario, the Alicante-based holding company notable for responsible residential, hotel and vacation tourism and specialists in the construction and promotion of homes along the Mediterranean and Caribbean coast, shared their sustainability and corporate responsibility strategy with 50 environmental, social and good governance commitments. These are structured around the strategic axes of climate action, sustainable construction, people and shared social value.

Jorge Blay, Europe Office Manager for TM, spoke about the possibility of sharing TM's vision of sustainability in this international tour of afterwork events. Luxembourg is one of the countries where clients have shown the greatest interest in this matter, converting it into one of their main decision elements for investment.

Joseba Arriortua, vice president of the Luxembourg Chamber of Commerce, added We are receiving more and more inquiries from residents of Luxembourg and Belgium, who want to invest in the Mediterranean area and especially in the region and city of Valencia. A lung transplant can be lifesaving for children with cystic fibrosis (CF) admitted for respiratory failure to a pediatric intensive care unit (PICU). Only 35% of these young patients went on to be discharged without a transplant, according to an analysis of children with CF treated at a French hospital.

Over the years following discharge, a lung transplant also associated with the best odds of survival for these patients. Factors influencing a risk of death or likely transplant outcomes included female sex and disease severity.

Children with CF who need critical care because of endstage respiratory failure need to be transplanted in [a] short-term period, the researchers wrote, calling a transplant their main chance to survive after leaving a hospital.

The study, Acute respiratory failure due to pulmonary exacerbation in children with cystic fibrosis admitted in a pediatric intensive care unit: outcomes and factors associated with mortality, was published in Respiratory Research.

The buildup with CF of thick and sticky mucus in the lungs causes inflammation and damage, leading to disease symptoms like wheezing, shortness or breath, or cough. Should the lungs become severely compromised, patients can go into acute respiratory failure and need to be hospitalized.

With the emergence of more effective disease treatments and better overall care, mortality rates for adult CF patients hospitalized for acute respiratory failure have significantly declined in recent decades.

Outcomes among young CF patients in a similar situation, however, have been poorly studied, the scientists wrote.

Paradoxically, short and longer-term outcomes at discharge from PICU have not been reported, they added.

A team at Hpital Necker Enfants Malades in Paris explored outcomes among 29 children with CF, ages 6-17 (median age of 13.5), admitted to its PICU due to acute respiratory failure between 2000 and 2020.

These patients had severe respiratory disease at hospital admission, and about three-quarters received nutritional support through a feeding tube and required at-home ventilation support. Nearly half (48%) were on a waiting list for a lung transplant.

All needed oxygen therapy in the hospital, with 92% being given non-invasive ventilation (i.e., an oxygen mask) and 10.3% invasive ventilation (i.e., a breathing tube). None were using CFTR modulator therapies, either because these effective treatments werent yet available or the childs disease-causing mutation wasnt eligible for their use.

Overall, five children (17%) died while in the ICU of respiratory failure, 14 (48%) received a lung transplant, and 10 (35%) were discharged without a transplant. Of those 10, three required a transplant within three years.

Mortality rates for the entire group were 31% at three months of follow-up, 34% at one year, and 42% at three years. Seventeen of the 29 children (58%) were alive at three years of follow-up, 12 (70%) of whom underwent a lung transplant. Of a total of 17 patients given a transplant, five died within three years of that surgery.

Most importantly, our results show very low rates of survival without lung transplantation during the 36 months following PICU admission for children with chronic respiratory insufficiency, the scientists wrote.

Lung transplantation should therefore be considered early after PICU admission and children referred to lung transplant centers immediately after their discharge, they added.

Statistical analyses uncovered risk factors associated with mortality or the need for a lung transplant. At the time of hospital admission, female sex, and higher carbon dioxide and lower chloride levels associated with a greater likelihood of poorer outcomes.

The probability of death was significantly increased in females, the scientists wrote.

A need for respiratory or nutritional support at the home, prior registration on a lung transplant list, or a lung infection with Stenotrophomonas Maltophilia bacteria prior to hospitalization also linked with a mortality or transplant risk.

Each of these factors could be considered markers of more severe underlying diseases, highlighting the importance of longitudinal disease control and proactive early care, they noted.

Study limitations include a retrospective design that precludes conclusions regarding causality between the factors identified and the risk of mortality or lung transplantation, the scientists wrote.

They added that study findings need to be confirmed in children at other centers, as their hospital typically treats severely ill patients.
