Man denied cystic fibrosis drug by one insurer has it approved by another – CBC.ca

A Halifax man denied a cystic fibrosis drug by one insurance company has had it approved by another.

Stefan Strecko was devastated last month after his approval for Trikafta, what some consider a groundbreaking drug, was revoked.

His partner of three years was able to add him to her insurance plan with Sun Life.

"It's giving me a lot more peace of mind, even though I've only been on it for two weeks, I feel like my health is, like, stabilized almost," said Strecko, 28.

Cystic fibrosisis a genetic disease that causes lung infections and reduces the ability to breathe.

Sun Life is one of the few insurance companies in Canada that covers the drug. Strecko saidhe has not heard anything from Canada Life since it denied him coverage.

"It was much easier with Sun Life than my previous application with Canada Life," said Strecko. "I was added to her plan, and it was a quick couple of phone calls between Sun Life and the pharmaceutical company."

Strecko saidhe was able to pick up the pills shortly after and he saw improvements after his first few doses.

"I saw changes, probably within the first three days of taking it," said Strecko. "I really felt a lot clearer and drier and overall was coughing much, much less, which is a huge difference for me."

Strecko said the cost is about $25,000 per month. Sun Life will cover 80 per cent and the remainder is subsidized by the pharmaceutical company.

Strecko said he's had some side effects, but nothing major.

"I just had some pretty bad congestion, a sore throat, things like that probably lasted three days, and then I felt better after that," he said. "It's not a bad trade-off."

This is the first modulator drug Strecko has taken.

Until now, Strecko has taken drugs that treat the symptoms of cystic fibrosis. Modulator drugs actually seek to improve the faulty protein that people with cystic fibrosis produce.

Trikafta repairs the protein that causes people with CF to have a thick buildup of mucus in the lungs and pancreas.

Strecko saidTrikafta will be part of his treatment for the foreseeable future or at least until something better comes along, and that adding three more pills to the 50 per day he already takes is no big deal.

He said it's a small price to pay for extending his life and bettering his health.

"I'm definitely less worried and less anxious kind of about my health and how things are going to be. And it just gives me a lot of hope for normalcy and in the future."

The Canadian Agency for Drugs and Technologies in Health, a national organization that determines the clinical and cost-effectiveness of prescription drugs,recommendsTrikafta be reimbursed by public drug plans.

However, it states the coverage should extend only to those who have at least one F508del mutation, and they must have a measure of lung functionof 90 per cent or less.

Streckothinks that is unfair.

"It's great for some people that can get it, obviously, but for others, they're kind of going to be left waiting, saying, 'Do I have to get sick or to get on this medication?'"

Additionally, the agency's recommendations aren't binding.

Strecko saidhe's excited to have received Trikafta and happy that some others will be able to get it, too.

However, he's not giving up the fight to get the drug covered for all CF patients by the federal government.

"My heart breaks a little bit to see people still waiting for it," he said. "We're all in this as a team together, and I just hope that all the CF patients can get it too because I think that's the most important thing."

Sun Life was not immediately available for comment.

In case you missed it, the Chicago Fire played Indy Eleven in a friendly on Tuesday, that they won 3-1. Normally something like an untelevised friendly with a lower division opponent wouldnt be a blip on the radar. However, something newsworthy did happen that night. Fire homegrown talent Missael Rodriguez scored twice.

Rodriguez just signed a 4 year deal with the club that technically starts on New Years Day 2022, but he was given a runout with the rest of the reserves Tuesday, and he made the most out of his organizational showcase. After leading the U19s to a MLS Next tournament victory, hes now coming to the senior roster with two goals and a lot of hype. I wish he could come into the setup now, but he cant. And I cant wait until he does.

Montreal is not fading at the end of the season like they historically do. They have only lost once since August 14th, a 1-0 loss in Nashville on the 14th. In that time theyre 3-2-1, with wins over Toronto, Orlando, and the Red Bulls. their draws were against Cincinnati while down a player, and Philadelphia. Theyve been scoring in bunches as well. Their wins against Toronto and Orlando had a combined score of 7-3, with 4 of the goals scored coming against Orlando.

With the season basically over, my lineup dream lineup would look something like this:

Yes, for those looking, its a 4-4-2. The Fires current personnel, and unless a massive amount of roster turnover happens in the offseason, the best way to play for this squad given the talent they have is a central diamond. Sekulic and Navarro can still push up in attack giving the team width, but they dont have any natural attacking wingers in the squad, so they should concentrate on working through the middle of the park.

All of this is possible because of the acquisition of Federico Navarro. His ability to cover so much ground frees up the need for the fire to play with a double pivot. They can push that second player forward and play another attacking-minded player, say, Brian Gutierrez (injury permitting) , instead of Gaston Gimenez who is good at what he does, but his spot is now redundant.

Meanwhile, if Aliseda is healthy again, he should slot into the second striker role where he is most comfortable, handing Luka the main attacking playmaker role. Chinonso Offor is offside less than Robert Beric and baring a Ricardo Pepi-like rise for Missael Rodriguez or a big-time striker signing in the offseason, hes your 2022 starting striker.

Clog the Midfield: At the moment, Montreals best player is Djordje Mihailovic. The ex-Frie homegrown talent has flourished in French Canada. This season, he's second in assists behind Carles Gil. In order to prevent Montreal from getting anything done, you have to shut down Djordje.

The best way to get this done is to clog the midfield to make sure he doesnt have the room he needs to work. The strategy I laid out in the lineup section would be the best way to do this; clog up the midfield and make the passing lanes so tight that Montreal have to go elsewhere to get any offense. However. regardless of how they do it, they must make it hard for Djordje Mihailovic to affect the game.

Figure Out the Offense: The Fire have a big problem. They cant score goals. Luka Stojanovic putting the ball in the back of the net a few times aside, the Fire havent been able to find a consistent goal scorer for any length of time. What they have to figure out now is if they have someone who can score going into the next season, or if they have to go out and get one in the offseason to supplement the trio of young strikers theyll have on the roster in 2022.

Chinonso Offor has been good at everything other than scoring goals. His positioning and timing on runs have been impeccable. However, he hasnt yet gotten that killer instinct that all great strikers have. He needs to gain that, or he could be on the outside looking in once next season rolls around.

Teams are so close in the middle of the eastern conference quality-wise, that the things that separate them are the details, like having the ability to put the ball in the back of the net. For that reason, the Fire are at the bottom of the midfield, and will probably end up with another L in their record. 3-1 to the ex-Impact.

CF Montreal closes stadium section indefinitely after repeated violent incidents – The Athletic

CF Montreal has closed section 132 at Stade Saputo for an indefinite amount of time, starting with the upcoming home game this Saturday, the club announced on Wednesday.

The closure is a result of repeated incidents of "violence, physical and verbal aggression, intimidation and vandalism, as well as numerous reports of unauthorized pyrotechnics, smoke bombs and incendiary devices" in section 132. The team said that section is where ASUPMTL is located, which includes the Ultras Montral 2002 and "affiliated groups."

Those groups are also banned from any activities at Stade Saputo and all official team events and initiatives.

"Several requests, restrictions and warnings have been issued to representatives of section 132 by the organization in recent years, to no avail," the club's statement said. "As recently as August 27, supporters from section 132 were involved in confrontations with visiting supporters who were physically assaulted outside Stade Saputo."

The league has indicated that it could take further action against the club and its fans if this behavior continues.

Minorities With CF Have Worse Lung Function, Study Says – Cystic Fibrosis News Today

Blacks and Hispanics with cystic fibrosis (CF) have worse lung function than white patients, according to a study from New York City.

However, no differences were found amongthese groups of patients in several other parameters, including the number of visits to the doctor, treatments with antibiotics, and hospitalizations.

The findings highlight the need to develop culturally appropriate ways of helping CF patients in minority communities, the scientists said.

The study, Health Disparities among adults cared for at an urban cystic fibrosis program, was published in the Orphanet Journal of Rare Diseases.

Although most CF patients are Caucasian, the number of affected patients with other racial and ethnic backgrounds has been increasing over the years. For example, in 2004, 6.1% of people with CF were Hispanic, but by 2019 it increased to 9.4%. Likewise, the proportion of Black CF patients increased from 3.9% to 4.7% from 2004 to 2019.

Previous research has found that people with CF who have a lower socioeconomic status have worse health outcomes. However, the association between racial and ethnic minorities with CF and health status is still uncertain.

For example, the current study found that Hispanic CF patients in California were found to have a 2.8 higher mortality rate than non-Hispanics. However, similar mortality rates were reported in Texas when comparing Hispanics to non-Hispanics.

This data highlights the fact that there are regional differences in outcomes of minority patients with CF, the team wrote. As a CF center in New York City with a relatively large minority population, we sought to determine if racial and ethnic disparities in health outcomes exist and to further understand possible causes for any existing disparities.

Investigators from Columbia University Irving Medical Center analyzed data from people with CF enrolled in the Cystic Fibrosis Foundation Patient Registry at the Universitys Adult CF Program. Patients were seen at least once during 2019. Two main groups were compared: Black/Latinx (BLx) and non-Hispanic Caucasians.

A total of 262 patients were included in the study. Thirty-nine patients (15%) identified as Black or Hispanic/Latinx (BLx). Mean age was 35.9 years for whites and 32.1 years for BLx patients, a difference deemed not statistically significant.

Among whites, 52% were diagnosed before age 2, compared to 46% of BLx patients. BLx participants had a significantly lower median household income based on ZIP code of residence in comparison to whites ($68,485 vs. $102,492).

FEV1, a measure of how much air a person can exhale in one second, was used to assess lung function. In BLx patients, lung function was lower, with a mean FEV1 percent predicted at 59.50, compared to 70.6 in whites.

Mean body mass index, an indicator of body fat, total number of outpatient visits, hospitalizations, and care episodes in which antibiotics were administered into a vein, did not differ between the two groups.

Results revealed an association between minority status and lower FEV1 that trended toward statistical significance.

The F508del mutation in the CFTR gene is the most common disease cause and is linked with worse outcomes. About the same percentage of patients are homozygous for the F508del mutation, meaning the mutation affects both copies of the CTFR gene, or heterozygous, which means it is present in only one copy.

After adjusting for factors such as sex, age, median household income and insurance type, statistical analysis showed that among F508del heterozygous patients, those in the BLx group trended toward lower FEV1 compared to white patients. No such difference was found in F508del homozygous patients or in those without F508 mutations.

Significant health disparities based on race and ethnicity exist at a single CF center in New York City, despite apparent similarities in access to guideline based care at an accredited CF Center, the researchers wrote.

This data confirms the importance of design of culturally appropriate preventative and management strategies to better understand how to direct interventions to this vulnerable population with a rare disease, they added.

The team noted a few limitations of the study. For example, Black and Hispanic patients were combined in the same category although these are distinct groups. Whether patients were following their treatments as prescribed also was not determined. Also, other factors that may affect health outcomes, including English proficiency and the ability to understand healthcare information, may affect outcomes.

We Think CF Industries Holdings (NYSE:CF) Can Stay On Top Of Its Debt – Simply Wall St

Some say volatility, rather than debt, is the best way to think about risk as an investor, but Warren Buffett famously said that 'Volatility is far from synonymous with risk.' So it might be obvious that you need to consider debt, when you think about how risky any given stock is, because too much debt can sink a company. Importantly, CF Industries Holdings, Inc. (NYSE:CF) does carry debt. But the more important question is: how much risk is that debt creating?

Debt and other liabilities become risky for a business when it cannot easily fulfill those obligations, either with free cash flow or by raising capital at an attractive price. In the worst case scenario, a company can go bankrupt if it cannot pay its creditors. However, a more usual (but still expensive) situation is where a company must dilute shareholders at a cheap share price simply to get debt under control. Having said that, the most common situation is where a company manages its debt reasonably well - and to its own advantage. The first thing to do when considering how much debt a business uses is to look at its cash and debt together.

As you can see below, CF Industries Holdings had US$3.71b of debt at June 2021, down from US$3.96b a year prior. However, it also had US$777.0m in cash, and so its net debt is US$2.94b.

Zooming in on the latest balance sheet data, we can see that CF Industries Holdings had liabilities of US$643.0m due within 12 months and liabilities of US$5.41b due beyond that. On the other hand, it had cash of US$777.0m and US$401.0m worth of receivables due within a year. So its liabilities total US$4.87b more than the combination of its cash and short-term receivables.

This deficit isn't so bad because CF Industries Holdings is worth US$9.77b, and thus could probably raise enough capital to shore up its balance sheet, if the need arose. But it's clear that we should definitely closely examine whether it can manage its debt without dilution.

In order to size up a company's debt relative to its earnings, we calculate its net debt divided by its earnings before interest, tax, depreciation, and amortization (EBITDA) and its earnings before interest and tax (EBIT) divided by its interest expense (its interest cover). Thus we consider debt relative to earnings both with and without depreciation and amortization expenses.

CF Industries Holdings's net debt is sitting at a very reasonable 1.7 times its EBITDA, while its EBIT covered its interest expense just 4.7 times last year. It seems that the business incurs large depreciation and amortisation charges, so maybe its debt load is heavier than it would first appear, since EBITDA is arguably a generous measure of earnings. We saw CF Industries Holdings grow its EBIT by 8.1% in the last twelve months. Whilst that hardly knocks our socks off it is a positive when it comes to debt. When analysing debt levels, the balance sheet is the obvious place to start. But ultimately the future profitability of the business will decide if CF Industries Holdings can strengthen its balance sheet over time. Finally, while the tax-man may adore accounting profits, lenders only accept cold hard cash. So we clearly need to look at whether that EBIT is leading to corresponding free cash flow. Happily for any shareholders, CF Industries Holdings actually produced more free cash flow than EBIT over the last three years. That sort of strong cash conversion gets us as excited as the crowd when the beat drops at a Daft Punk concert.

CF Industries Holdings's conversion of EBIT to free cash flow suggests it can handle its debt as easily as Cristiano Ronaldo could score a goal against an under 14's goalkeeper. But, on a more sombre note, we are a little concerned by its level of total liabilities. Looking at all the aforementioned factors together, it strikes us that CF Industries Holdings can handle its debt fairly comfortably. Of course, while this leverage can enhance returns on equity, it does bring more risk, so it's worth keeping an eye on this one. The balance sheet is clearly the area to focus on when you are analysing debt. But ultimately, every company can contain risks that exist outside of the balance sheet.
