Breakthroughs in Prior Authorization Automation: An Expert Offers Insights – Becker’s Hospital Review

With new breakthroughs happening in electronic prior authorization technology, industry stakeholders are curious what CMS may ultimately mandate in its forthcoming final rule and what types of solutions will help facilitate the changes.
A resident expert from MCG Health answered questions about potential approaches the federal government might take as well as new technologies that could support the evolution of authorizations. Rajesh Godavarthi, the Associate Vice President of Technology and Interoperability at MCG Health, who also serves on the WEDI Board of Directors and the ONC HITAC, discusses these important topics below.
Question: With regard to the healthcare industrys efforts to automate prior authorization decisions and pending rules from CMS, are there any new updates on that front?
Rajesh Godavarthi: As it pertains to the federal government, I feel that the establishment of electronic standards around prior authorization is among their highest priorities. It ranks up there with efforts to promote health equity and a couple of other initiatives. They know the burden of prior auth is real, and they understand how it can impact patient outcomes.
Earlier this year, the Office of the National Coordinator for Health Information Technology (ONC) put out a request for information (RFI) to gather data from a wide array of healthcare stakeholders, like the American Medical Association (AMA), the American Hospital Association (AHA), and the Council for Affordable Quality Healthcare (CAQH) a lot of prominent organizations to look at ways to develop a criteria for the best ways to approach EMRs (electronic medical records). And by that, I mean how can they enable the EMRs to support the necessary data elements and workflows that can be expose to the payers (or the technology partners) to automate the process. Theyre currently looking at the best ways to start a certification process. However, they do acknowledge that the HL7 Da Vinci Project standards are not fully matured to impose as a strict criteria. They will probably take a more measured approach that allows the industry to mature a little bit and have some pilot studies or connectathons. I think once they start seeing more industry progress, they will start creating specific certification criteria around the question, Does this require prior auth? And then theyll look at the next piece. So, instead of saying health plans and health systems need to have an end-to-end automation in place (according to the specs), I think theyll take a more modular approach.
Q: There was a lot of buzz at HIMSS 2022 in Orlando about MCG Healths advances in interoperability and automating authorization decisions. Your company appears to be ahead of the curve in this area. Can you tell us a little about that?
RG: Yes. MCG has been working with a large payer and two hospital systems based in Ohio. Weve started automating their inpatient workflows which are pretty complex when compared to elective procedures and prior auth. The work that MCG has done with all sides to facilitate the communication between the payer and provider systems helps enable a clean exchange of data for authorizations. What we have learned since we rolled out our Cite for Collaborative Care solution last year, is that you need to make the statuses of the authorization transparent at every point of communication so that providers know (1) their auth request has been delivered successfully to the payer, (2) it has been reviewed by the payer, and then (3) the determination is transmitted back. We have seen a tremendous success and rapid adoption as these hospitals asked us to add more facilities and end users. Today, 70% of the authorization decisions from these hospitals to the payer are being automated through this platform.
Q: Integrating a solution like this into the hospital EHR platforms and the payers utilization management (UM) system sounds challenging. What were some of the top learning moments for this type of project?
RG: On the EMR side, we have worked with CarePort and Allscripts for this project. They control the actual provider workflow, so we collaborated with them to make sure the APIs (application programming interfaces) are implemented properly so that the hospital staff can know their request has been submitted and they can seamlessly receive the status. The MCG team worked very closely with CarePort to make sure everything worked, and this dovetails into everything MCG is also doing with the HL7 Da Vinci Project. We are working with other industry leaders to develop the standards such as How do you create a CDS hook? so that things can be initiated from the provider. Much of the work weve done as a part of the HL7 Da Vinci Project is to make the providers workflow much more seamless so that they dont have to do any additional actions to get complete the authorization request. With the payers medical management system (MMS), we learned that there is a lot of information on their side, but not everything is exposed. At least to the point where we can take it and communicate it to the provider. In those cases, we initiated certain extensions to get the information about the status whether it is approved, denied, or pended and relay that back to the provider. We just had to work with the different systems to learn what information needed to be shared and then we built the pipes that could deliver it between the payer and provider.
Q: What has the feedback been from these hospitals systems using the platform?
RG: As of today, one of the hospital systems is sending all emergent cases to the payer via this platform which is anywhere from 45-70 cases per day. Since they are receiving authorization determinations via their EHR, this has reduced the faxes they receive from the payer by about 800 per month. This is a 57% reduction in faxes for their hospital system, and it has saved about 20 hours per week of manual, administrative burden. They also reported significant increase in employee satisfaction (especially around auto approvals), and their UM staff felt the streamlined workflow. We are currently in discussions with them to expand the use of the platform in other use cases.

Toyota has been named the official car and truck of the Pacific Office Automation 147, the upcoming NASCAR Xfinity Series event at Portland International Raceway (PIR) on June 3-4, 2022. With the support of the Pacific Northwest Toyota Dealers Association, Toyota has been added as another major partner for the big stock car racing weekend in the Rose City, showcasing the exciting return of a NASCAR national series to the Pacific Northwest for the first time in 22 years.
The automaker will provide Toyota Tacoma trucks to serve as the parade vehicles for pre-race driver introductions as the starting field goes around the 1.97-mile permanent road course before the green flag. Additionally, Toyota will provide the official pace car featuring the Toyota Supra for the Pacific Office Automation 147. Toyota will also receive significant track signage and onsite exposure via video and public address announcement and event activation space during the event weekend at PIR.
"NASCARs much-anticipated return to Portland with the Pacific Office Automation 147 is a historic event that the Pacific Northwest Toyota Dealers Association is proud to partake in as the official car and truck sponsor," says Russ Humberston, president of the Pacific Northwest Toyota Dealers Association. On Saturday, June 4, the Pacific Office Automation 147, a 75-lap race covering 147.52 miles, will get underway at 1:30 p.m. PDT, followed by an ARCA Menards Series West 51-lap feature to cap off the two-day event.

Fetcher raises $27M to automate aspects of job candidate sourcing – TechCrunch

Reflecting the growing investor interest in HR technology startups, Fetcher, the talent acquisition platform formerly known as Scout, today closed a $27 million Series B funding round led by Tola Capital with participation from G20 Ventures, KFund and Accomplice. The new money $7 million in debt and $20 million in equity brings the startups total capital raised to $40 million, which co-founder and CEO Andres Blank says is being put toward international expansion and building out the Fetcher platform with new applicant tracking system (ATS) integrations and customer relationship management capabilities.
Fetcher was co-launched in 2014 by Blank, Chris Calmeyn, Javier Castiarena and Santi Aimetta as a professional networking app called Caliber. After a few years, the founding Fetcher team decided to pivot into recruitment, leveraging some of the automation technology theyd built into Caliber.
Hiring high-quality, diverse candidates had always been a pain point for me. At one of my prior startups, I personally experienced this issue, and after bringing on a recruiting team to help scale hiring efforts, I saw that their time was also too valuable to be spent on the manual, repetitive tasks that come with sourcing candidates, Blank told TechCrunch in an email interview. Rather than relying on expensive staffing fees, I thought there must be a better way to keep sourcing in-house, without it taking up too much time and energy on the talent acquisition teams and hiring managers.
Through a Chrome extension, Fetchers platform ties in with ATS products as well as Gmail and Outlook to allow recruiters to source candidates directly from LinkedIn. Fetcher filters jobseekers into prebuilt email workflows, offering analytics including progress toward diversity goals at the individual, team, position and company levels.

Fetcher also performs predictive modeling, automatically gauging the interest of job candidates from their replies, and automated sourcing, which runs in the background to push applicants through vetting processes via automated emails.
A great candidate experience is essential for any company, and part of that experience comes from building long-term relationships with candidates over time. Fetchers candidate directory allows companies to remarket to qualified candidates, set up reminders for future connections, and add additional outreach emails to the automated sequences, Blank said. Overall, the goal is to make it simple for companies to store, update, and connect with great candidates over time, messaging them about future job opportunities, milestones at the company, and more.
The reliance on algorithms is a bit concerning, given the potential for bias Amazon infamously scrapped a recruitment algorithm that favored male engineers and New York City recently placed restrictions on the use of AI in hiring. When asked about it, Blank asserted that the platforms automation technologies allow for a more diverse group of prospects to push through the hiring funnel. He also highlighted Fetchers outreach policy, noting that people who dont wish to be contacted about opportunities via Fetcher can send data deletion requests.
[O]ur secret sauce here at Fetcher is combining both machine and human intelligence in order to minimize the biases that exist on both sides, Blank said. Beyond this, we also have diversity metrics on each search (visible on our platform to the client too), which keeps us in check. If were over- or under-indexing anywhere on the gender or demographics front, the platform can course correct. Finally, we remove selection biases from the client. The way we do this is that once a client trusts that the search is heading in the right direction (after vetting a handful of candidates upfront), they place the search on full automation. This means that going forward, they are no longer vetting every candidate, but simply reaching out to all qualified candidates that are found for [a given] open role.
Blank linked to case studies from customers like Frame.io, which recently used Fetcher to hire employees mostly from underrepresented groups. But biases can enter at many different, often unpredictable, stages of the pipeline. As Harvard Business Reviews Miranda Bogen writes: For example, if [a] system notices that recruiters happen to interact more frequently with white men, it may well find proxies for those characteristics (like being named Jared or playing high school lacrosse) and replicate that pattern. This sort of adverse impact can happen without explicit instruction, and worse, without anyone realizing.

The risk doesnt appear to be dissuading recruiters. Fetcher currently has over 350 customers (growing 10% month-over-month) including Behr Paint, Albertsons, Foursquare and Shutterstock., and annual recurring revenue tripled in the last 12 months.
Beyond the strong top-line numbers, Fetcher is benefiting from the broader boom in the HR tech segment, which has seen high venture capital activity over the past few months. According to PitchBook, HR tech startups collected more than $9.2 billion in venture capital funding globally from January 2021 to October 2021 a 130% jump from 2020s total.
Fetcher is uniquely positioned as one of the only software-as-a-service recruiting platforms to automate both candidate sourcing and email outreach efficiently, Blank said. Rather than using a straight database model, Fetcher is the only sourcing solution that can truly automate the sourcing process for companies, based on its unique combination of machine learning with human intelligence. This model allows for what feels like a 24/7 sourcer to work in the background for each client. By automating both the sourcing and outreach sides of recruiting, Fetcher can reduce the number of internal sourcers and recruiters a company needs, as well as significantly reduce the budget being spent on outside recruiting firms, agencies, or consultants.
Fetcher employs 45 people, currently, and plans to double that number by the end of the year.
