Whole-genome sequencing was found to provide rapid and accurate genomic profiling for patients with acute myeloid leukemia (AML) or myelodysplastic syndromes (MDS), and this technology may be a potential replacement for conventional cytogenetic and sequencing approaches to risk stratification, according to the results of a study published in the New England Journal of Medicine.

Implementing whole-genome sequencing for clinical testing can provide a unified, stable, and extensible platform that minimizes laboratory-specific bias and that can be standardized throughout the world, study researchers stated. Although our study focused on myeloid cancers, many of the advantages of whole-genome sequencing that we observed will directly apply to patients with other cancers, the researchers concluded.

Patients who have AML or MDS often undergo genetic profiling for diagnostic classification and risk assessment. This study tested whether whole-genome sequencing could be streamlined and applied to diagnostic clinical samples in real time. The study included genomic profiles from 263 patients with myeloid cancers, including 235 who had undergone cytogenetic analysis.

In this study, whole-genome sequencing identified all 40 recurrent translocations and 91 copy-number alterations that were detected using cytogenetic analysis. In addition to these mutations, whole-genome sequencing identified new clinically reportable genomic events in 40 of the 235 patients (17%).

Prospective sequencing was performed on samples taken from 117 consecutive patients in a median of 5 days. These 5 days included 2 days for library preparation, 2 days for sequencing, and less than 1 day for analysis. Results provided new genetic information in 29 patients (24.8%), and risk category changed in 19 patients (16.2%).

According to the researchers, risk prediction based on the whole-genome sequencing results correlated with outcomes. Significantly longer overall survival was observed in the 21 patients with intermediate or favorable risk compared with the 6 patients with adverse risk.

This survival difference was superior to that resulting from the assignment of patients to risk groups on the basis of gene mutations alone, the researchers noted. Although larger studies involving more patients will be required to firmly establish the clinical

performance of whole-genome sequencing, our proof-of-concept study shows that this method has the potential to add prognostic value by expanding risk stratification to more patients, especially for those with inconclusive results on cytogenetic analysis, where whole-genome sequencing could have an immediate effect on treatment decisions.

Genome Sequencing: An Alternative for Risk Stratification in Myeloid Cancers - Cancer Therapy Advisor

PLYMOUTH MEETING, Pa., April 15, 2021 /PRNewswire/ --Inovio (NASDAQ:INO), a biotechnology company focused on rapidly bringing to market precisely designed DNA medicines to treat and protect people from infectious diseases, and cancer, today announced the results of a study focusing on the human immune responses induced by INOVIO's DNA vaccine candidate for COVID-19, INO-4800, against variants of concern. The results showed that INO-4800 induced a robust T cell response against all spike protein variants tested, which the company believes will be key in providing protection against SARS-CoV-2 variants, in addition to providing similar levels of neutralizing activity against both the UK and Brazilian variants as those against the original strain. The study, entitled "INO-4800 DNA Vaccine Induces Neutralizing Antibodies and T cell Activity Against Global SARS-CoV-2 Variants," has been submitted for peer review and can be viewed at https://www.biorxiv.org/content/10.1101/2021.04.14.439719v1.

Dr. J. In the study, clinical samples were collected at varying timepoints post-immunization from subjects in INOVIO's Phase 1 US-based INO-4800 clinical trial. Antibodies capable of neutralizing activity were measured against the spike protein variants tested, including B.1.1.7 (UK variant), B.1.351 (South African variant) and P.1. (Brazilian variant). The study showed the T cell responses induced by INO-4800 vaccination were fully maintained against the UK, South African and Brazilian variants when compared to the T cell responses to the original Wuhan strain. The neutralization levels of INO-4800 against South Africa and UK variants were reduced to the levels similar to the previous reports of mRNA or viral vector vaccines. Furthermore, despite recent reports showing a reduction in neutralizing activity against the Brazilian variant by the mRNA or viral vector vaccines, INO-4800 generated robust neutralizing antibodies at levels against Brazilian variant which were comparable to those against the Wuhan strain. Taken together with the data showing the maintenance of T cell activity, the results reported in this study provide a comprehensive overview of cross-reactive cellular and humoral immune responses against SARS-CoV-2 variants for INO-4800 vaccinated individuals that may be important for protection against variant strains of SARS-CoV-2.

Collectively, the studies showcase how the combination of robust T cell responses in conjunction with neutralizing antibody responses provide an important defense against both emerging and potential future viral variants, exhibiting broad protection against the virus and less impacted by the mutational changes a virus may undergo.

ABSTRACT

OBJECTIVES: Despite the high prevalence of coronavirus and various treatment approaches, including complementary and alternative medicine (CAM), there is still no definitive treatment for coronavirus. The present study aimed to evaluate the effect of CAM interventions on COVID-19 patients.

CONTENT: Four databases (Web of Science, PubMed, Scopus, and EMBASE) were searched from the inception of databases until July 16, 2020. Keywords included complementary and alternative medicine therapies and Coronavirus.

SUMMARY AND OUTLOOK: Of the 1,137 studies searched, 14 studies performed on 972 COVID-19 patients entered the systematic review final stage. The results showed that different CAM interventions (acupuncture, Traditional Chinese medicine [TCM], relaxation, Qigong) significantly improved various psychological symptoms (depression, anxiety, stress, sleep quality, negative emotions, quality of life) and physical symptoms (inflammatory factors, physical activity, chest pain, and respiratory function) in COVID-19 patients. The results showed that various CAM interventions have a positive effect on improving the various dimensions of coronavirus disease but since there are few studies in this regard, further studies using different CAM approaches are recommended.

You might be surprised to learn that a cancerous tumor is not a single set of cells and some of these tumor cells are more prone to spread to different parts of the body. They are the reason for cancer metastasizing. That subset of tumor cells are known as cancer stem cells and they also are more resistant to anti-cancer medications and radiation therapy.

Recent medical studies have shown cancer stem cells seem to be sensitive to common medications and some dietary supplements. With these therapies, their ability to metastasize may be reduced by as much as 90%. As a result, some researchers suggest cancer therapy should also include therapies that target cancer stem cells.

Cancer stem cells, or CSCs, are a small subpopulation of cells within tumors with capabilities of self-renewal, differentiation and tumorigenicity. The clinical relevance of CSCs demonstrates that CSCs quickly develop resistant to chemotherapy and radiation treatment. CSCs were first described by John Dick in 1994. He discovered that CSCs are slower growing than other tumor cells. Traditional anti-cancer therapies focus on fast-growing cells so it is not unusual for CSCs to "escape" and travel to other parts of the body.

Dick stated that "to get rid of the cancer, you have to find ways of eliminating the stem cells." CSCs may be susceptible to common antibiotics.

All cells contain a unique organelles called mitochondria. It is in the mitochondria that much of the energy used in the cell is made. Both the cell and its mitochondria independently make needed proteins through a protein-based "machine" termed a ribosome. Ribosomes of the cell and the mitochondria are quite different. Indeed, mitochondrial ribosomes are more related to bacterial ribosomes than cellular ribosomes. Some antibiotics kill bacteria by inactivating their ribosomes and there is some research indicating these antibiotics, by the same mechanism, affect the mitochondrial ribosomes of CSCs.

One study published in the medical journal Aging demonstrated that two antibiotics and vitamin C reduced propagation of CSCs by 90%. This was a study done in test tube (in vitro) but the results are compelling. The two antibiotics, doxycycline and azithromycin, bind to CSCs ribosomes at different sites. Vitamin C enhances the effects of the antibiotics. The end result dramatically reduces the metabolic activity of the CSCs, reducing their ability to metastasize.

An additional benefit of azithromycin is that, by a different mechanism, it may reduce the ability of CSCs to survive. Another, less well-known antibiotic, salinomycin (Chemistry, 2020), induces CSCs death by forcing the cells to accumulate toxic levels of iron.

The clinical trials for antibiotics and CSCs is limited but positive. However, given the low toxicity associated with these antibiotics and the potential gain for cancer patients, antibiotic therapy should be considered in combination with chemotherapy, radiation therapy and even surgery. In the fight against cancer, no stone should be left unturned.
