Analyzing CBD studies: Health concerns, health claims, populations … – Nutritional Outlook

Vedic Lifesciences is a contract research organization based in Mumbai, India. We recently examined clinical trials conducted in the hemp and cannabinoid space. We analyzed a total of 27 studies whose investigational product included cannabinoid or other hemp products. This report is intended to cover nutraceutical-centric studies that incorporate cannabidiol (CBD) and its derivatives like hemp and CBD oil (different extracts and forms) as the investigational product.
This report describes our findings regarding:
Our findings will interest a broad range of industry professionals:
By attempting to identify the innovative health claims being investigated for specific forms of CBD, this report aims to help R&D teams formulate their functional ingredients or branded supplements using various CBD types and doses, and design unique trials with trending objectives. It also aims to help company managers make informed decisions about marketing their products and planning their growth and to provide companies looking to enter the CBD market with more information on results of recent clinical and preclinical studies.
In terms of our search methods, we used the clinical trial registry http://www.clinicaltrials.gov instead of a search engine like PubMed, which would only have yielded trends from published studies initiated more than two years ago. By contrast, a clinical trial registry provides more current data, including data on studies not yet completed.
Here are the filters we used in our analysis:
Health Area
Which health concerns is CBD being studied for?As Figure 1 shows, the majority of CBD research is on cognitive support. The primary objective of more than one-third of the studies we analyzed was some aspect of cognitive health. Secondary outcomes included changes in sleep, quality of life, physical activity, and behavioral patterns at baseline and end of the study. Figure 1 also shows that CBD is now being studied for metabolic health (weight management, glucose levels, lipid profiling).
Study Endpoints
Which study endpoints are these CBD studies focusing on? Table 1 provides a detailed look at some of the most common endpoints for health concerns ranging from cognition and metabolism to immunity, heart health, joint health, sleep, sports performance, and more.

Which tools and biomarkers did these CBD studies use? Table 2 provides a detailed look at some of the most common tools and biomarkers used in studies for health concerns ranging from cognition and metabolism to immunity, heart health, joint health, sleep, sports performance, and more.
Sample Size
Interest in CBD has led to more than 150 proof-of-concept studies. Approximately 58% of the studies we analyzed had a sample size of fewer than 50 subjects. (See Figure 2) More companies and research institutes are initiating proof-of-concept/pilot studies on CBD. This would be a space to watch out for. Large-sample-size studies should follow suit based on the data obtained in the studies.
Completion Status
Interestingly, while more than 42% of the studies we analyzed had been completed, no results were highlighted in the clinical trials registry. (See Figure 3) Another fact we noticed is that close to 31% of the studies are currently recruiting. This means that the research on CBD has gained momentum in the last five years. Finally, both types of studiescompleted and in progressencompass a range of health areas, meaning several health claims are substantiated.
Study Duration
More than 53% of the total studies we analyzed had a duration of less than 12 monthswith some still in the recruitment phase in terms of their completion date. (See Figure 4) This fact, combined with a trend observed of smaller study sample sizes, indicates that several companies are interested in faster substantiation of their health claims. This may then lead to large-sample-size studies in the next couple of years and faster market inclusion of their products.

While there are more of proof-of-concept studies, close to 77% of studies are efficacy studies rather than safety studies. This indicates that companies may be directly jumping into substantiating health claims rather than performing the quintessential safety assessment of different extracts of CBD.
Other Interesting Insights
When we analyzed the studiesexcluding those whose status was either withdrawn or unknown, two studies caught our attention, for different reasons.
First, in the study NCT05108220, 1350 female subjects aged 24-44 (working population) were recruited in order to investigate the effect of CBD on subjects anxiety levels. The dose-ranging study included CBD oil and capsules with 0.0%-0.3% THC content. In-Home Usage Tests (IHUT) were done to understand the different CBD compounds effects on anxiety in women.
Second study NCT04777981 was interesting. The focus was the effects of CBD, in combination with red algae, on symptomatic improvement in COVID-19 patients.
Other key takeaways is that interest in CBDs benefits is not just limited to cognition. While cognitive support is a known area of CBD research, interest in other health areas like heart health (hypertension), cancer (symptomatic improvement), sleep quality, immune health, and metabolic health demonstrate broad interest in CBD inclusion in the nutraceutical industry.
Recommendations for Research from a Regulatory Perspective
According to certification and testing firm FoodChain ID, both preclinical and clinical studies are mandated for regulatory submission for any novel food. These include the bacterial reverse mutation test, in vitro micronucleus test in mammalian cells, and a 90-day study. Also for CBD, global standardized guideline tests to be done would be OECD 451,452, 443, and 422.
After preclinical studies are concluded, a safety study in healthy individuals is needed. Safety can be analyzed by assessing the comprehensive metabolic panel, which tests the levels of electrolytes and kidney and liver parameters. Tests can also include lipid profile, hormone profiling for men and women, semen analysis, and inflammatory biomarkers.
Another good practice is understanding the sleep quality, cognitive health, and gut parameters using questionnaires. This data can then be studied in conjunction with the safety tests to draw final conclusions about the effect of CBD in humans.

USDA Doubles The Number Of Hemp Professionals Serving On … – Marijuana Moment

The U.S. Department of Agriculture (USDA) and the Office of the U.S. Trade Representative (USTR) are again expanding representation of the hemp industry in federal trade advisory committees, doubling the number of officials who will help bolster cannabis commerce.
On Thursday, USDA and USTR announced the appointments of two more hemp officials: one a top regulator for the crop in Oregon and the other a member of the National Industrial Hemp Council of America (NIHC). That raises the total number of federal hemp trade advisors to four.
Eric Pike, a member of the Oregon Hemp Commission and founder of the CBD company Root Origins, and NIHC board member Patricia Sheikh will be joining the Agricultural Technical Advisory Committee (ATAC) for Trade in Tobacco, Cotton, Peanuts and Hemp. Its one of six ATACs that offer the government technical advice about specific agricultural commodities and products.
USDA and USTR have been adding hemp officials to committee rosters for the past few years, starting with Kevin Latner, a former marketing executive at NIHC, who was made part of an ATAC focused on processed foods in 2020. In 2021, NIHC President Patrick Atagi was appointed to the ATAC dealing with hemp. All together, NIHC members have been selected for three out of four of the hemp appointments to date.
Now, the number of members of the Tobacco, Cotton, Peanuts and Hemp panel with hemp in the name of the organization they representthreeis the same as the number from groups with peanut or cotton in their names and is one more than the number of representatives from tobacco organizations.
The latest announcement shows the commitment the federal government has in hemp, Atagi told Marijuana Moment on Thursday. He added that the latest NIHC addition to the committee, Sheikh, has extensive experience in agriculture and trade, including a long career at USDA in many roles, including USDA deputy administrator.
The newly appointed members will serve until August 2027 and the committees will be supplemented by additional appointments over the next four years, the government agencies said. People are encouraged to apply for committee membership at any time and applications will be reviewed periodically.
In further recognition of hemps growing role in the agriculture sector, USDA and USTR formally renamed one of the ATACs in June to include the name of the crop. Previously, the first hemp appointees served on what was then called the ATAC for Trade in Tobacco, Cotton and Peanuts.
While USDA has worked to promote the hemp industry since the crop was federally legalized under the 2018 Farm Bill, the sector has faced a downturn in recent years as the production value of hemp has plummeted. Stakeholders largely attribute the problem to an ongoing lack of Food and Drug Administration (FDA) regulations to allow for the sale of derivatives like CBD in food products or as dietary supplements, as the Congressional Research Service (CRS) noted in a recent report.
As lawmakers work to resolve the issue, USDA has sought to support the industry in other ways, such as releasing an informational video encouraging hemp growers to construct a backpack vacuum that allows them to collect up to 10 grams of cannabis pollen in less than a minute. Thats part of an effort to build a national genetic repository for hemp that will be available to international researchers and breeders.
Earlier this month, USDAteamed up with the Pennsylvania Department of Agriculture to host farmers for an educational eventfocused on cultivating cannabis in a bio-based economy.
The department also recently shared the success story of an Indiana-based hemp farm, touting how the agency offered financial and technical assistance to help the business to maximize its output of high-quality CBD oil.

AFTER winning two National Championships and spending six seasons in the NFL, a former Florida Gator has kickstarted his own business.
One of the stars of the long-awaited Netflix Untold docuseries on Urban Meyers' Florida Gators, titled Swamp Kings, former linebacker Brandon Spikes has lived a remarkably varied post-NFL life.

Originally from North Carolina, football offered Spikes a path toward a brighter future.
In the documentary, Spikes discussed his tough childhood, explaining how "Football was my way out. Out of it all."
In 2006 he headed for Florida, joining a Gators program that was on the rise under their young head coach.
Spikes went on to solidify himself as a legend down in Gainesville, winning two BCS National Championships while being named a Consensus All-American twice, and an All-SEC First-Teamer three times.
Come 2010, it was time for Spikes to take the next step, ultimately being selected in the second round of the NFL Draft by the New England Patriots.
He spent four seasons with the Patriots, starting 39 of his 51 appearances, racking up 286 tackles, twelve defended passes, and four forced fumbles.
In 2014, Spikes headed for the Buffalo Bills where he'd go on to play in all sixteen games that season, starting ten.
He subsequently returned to New England, although this proved to be an ill-fated move as he was released less than a month later after running into trouble with the law.
Spikes eventually pled guilty to having left the scene of an accident, as per The Athletic, and went on to miss the 2015 season.
The following year, while working as a certified credit investor, he reached out to his former head coach Bill Belichick for help.
"I wanted to give up. I really did until I reached out to Bill," he told The Athletic.
"I reached out to everybody, people I trusted and loved, and they just couldn't give me an answer why I wasn't in the league.
"So I was like, 'F**k it, I'm about to call Bill.' So I called Bill Belichick, and of course, he answered on the first ring."
Belichick put Spikes in touch with an agent, David Canter, who managed to get him a one-year deal back in Buffalo.
Eleven appearances later, and his time in the NFL came to an end.
Spikes' next career venture took him to Canter's agency where he worked for the post-career affairs department.
Meanwhile, he also founded SpikesCBDX, which produces CBD-infused creams, oils, and gels.
"Throughout my career in the NFL, I always wanted to help myself and other athletes avoid injuries and recover quickly, without medications and their side effects," Spikes wrote on their website.
"This was the driver behind the Spikes CBDX product line, creating products all athletes could benefit from."
Fast forward to 2023, and Spikes has gone on to enter a new chapter of his post-NFL career, as a student assistant coach for the Gators.
Speaking to FloridaGators.com, he explained: "I just want to bring value and basically get this thing back to how we left it. I've seen how it should be done.
"You don't come to Florida to be mediocre. The bar is set, and I will do everything in my power to enforce what is set here."

The ABCs of CBD: Cannabinoids in Dermatology and Skin Care – Dermatology Times

Cannabinoids (CBDs) are gaining popularity in both the skin care industry and the dermatology drug market and are becoming increasingly available to physicians and consumers. Cannabis-based personal care products suddenly seem ubiquitous all over the market, where CBD is a featured ingredient in everything from moisturizers and lotions to makeup.1 In medical dermatology, CBD has been studied in acne, eczematous disorders, lichen planus, melanoma and nonmelanoma skin cancer, melasma, prurigo, pruritus, psoriasis, scleroderma and systemic sclerosis, and seborrheic dermatitis.2
Although some of these studies indicate a potential therapeutic benefit in skin disease, existing studies are, unfortunately, underpowered and lack rigorous design. Making matters worse, the federal government lists cannabis as a schedule I substance (defined as drugs with no currently accepted medical use and a high potential for abuse), which makes it very difficult to study CBD for medical purposes. In its last communication on CBD products in the current market, the FDA stated that the existing regulatory framework for CBD is not appropriate and that it will work with Congress to develop a safe and effective regulation strategy. Despite these issues, CBD may offer a promising new area of pharmacology for skin disease and skin care. Even if dermatology providers choose not to use or recommend these products, patients are asking about them during their office visits. Therefore, it is important to review the current evidence on CBD use in dermatology to understand its potential for treating skin diseases. It is also important to learn about the potential benefits of CBD in dermatology to better counsel patients in the office.
What Are CBDs?
CBDs are chemicals that exert their effects through cannabinoid receptors (CBRs). Multiple ligands exist for CBRs including endogenous endocannabinoids (such as AEA, 2-AG, PEA, and OEA), phytocannabinoids (produced in plants), and synthetic CBDs. The most notorious and well-known phytocannabinoid is delta-9-tetrahydrocannabinoil (THC), which is the psychoactive component of cannabis responsible for the feelings of euphoria, relaxation, heightened sensory perception, altered perception of time, and increased appetite.
Most CBDs are not psychoactive in the traditional sense. For example, many beauty products contain hempseed oil (cannabis sativa seed oil), which contains no THC. This is an important distinction to make because skin care products labeled cannabis can be misleading.
Not only are consumers confused on this topic, but health care providers are as well. A survey of 531 dermatologists found that 64% of respondents did not know that CBD is not psychoactive and 29% did not know that THC is psychoactive.3 Only half of the dermatologists on the Medical Advisory Board for the National Psoriasis Foundation believed that medical cannabis is safe for patients.4
CBRs have been discovered to exist on cutaneous nerve fibers, mast cells, and keratinocytes, and some evidence has shown that CBDs may directly stimulate keratinocytes and act on other important receptors such as PPAR-g (which regulates fatty acid storage and glucose metabolism).5 CBD has also been found to enhance adenosine A2A receptor activity, which downregulates hyperimmune states and decreases tissue inflammation.6
These basic science findings are encouraging and suggest that CBDs may be of benefit in a number of dermatological diseases, as well as for use in personal care products.
Cannabinoids continue to be of interest as they possess many characteristics that would seemingly make them a great fit for multiple dermatologic conditions: They are potentially anti-inflammatory, anti-pain, and anti-itch; can alter hair growth; can regulate epidermal proliferation; and can even decrease sebum production. In atopic dermatitis, acne, psoriasis, and hidradenitis suppurativa, there is a potential role for them, at least in theory, said Peter A. Lio, MD, clinical assistant professor of dermatology and pediatrics at Northwestern University Feinberg School of Medicine in Chicago, Illinois, and also the author of the article Cannabinoids in Dermatology: Hope or Hype? featured in the Journal of Cannabis and Cannabinoid Research.
CBD and Acne
The pathophysiology of acne is a permutation of follicular hyperkeratinization, microbial colonization, sebum production, and inflammation. The endocannabinoid N-arachidonoylethanolamine glycerol (AEA) has been shown to cause sebocyte apoptosis at higher levels, thus targeting one of the key pathophysiological mechanisms of acne.7 Several in-vitro studies indicate that CBD could be a possible treatment option for acne through its regulatory effects on sebum production and sebocyte proliferation.8
One key study, conducted by Olah et al, investigated the effects of CBD on sebaceous glands in vitro.9 The researchers found that AEA (a type of endocannabinoid) was able to inhibit lipogenic actions of sebaceous glands in a dose-dependent manner by inhibiting lipogenic compounds such as arachidonic acid and a mixture of linoleic acid and testosterone. CBD was also found to prevent pro-acne mediators from elevating TNF-a mRNA expression and normalize the lipopolysaccharide-induced expression of IL-1 and IL-6. Furthermore, in a small, split-face study, male subjects applied 3% cannabis seed extract cream to 1 side of their face twice daily for 12 weeks. At the end of the study, researchers found skin sebum production and erythema were significantly decreased compared with placebo, with no notable adverse effects.10
Although larger-scale human studies and randomized, controlled clinical trials are needed in the treatment of acne with CBDs, these preliminary studies show promise in the use of CBD as an individual or adjunctive therapy for acne vulgaris. This may prove popular with patients who are seeking alternative, complementary, or natural therapies.
CBD and Atopic Dermatitis/Eczema
CBDs have been studied for the treatment of eczema and atopic dermatitis, which affects about 10% of the US population.11 Eberlein et al studied more than 2000 eczema patients in a large, multinational cohort study and found that applying a cream containing palmitoylethanolamide (PEA) resulted in a significant reduction in dryness, excoriation, pruritus, and erythema across the population. Impressively, 56% of patients stopped using topical corticosteroids by the end of the study.12 It is important to note that this study lacked a placebo group. Therefore, it is difficult to ascertain the therapeutic benefit of the PEA vs the emollient effect of the vehicle. Nevertheless, this study is promising for the use of PEA in the treatment of eczema, as another randomized, double-blind, comparative trial of 60 patients with eczema found that PEA/AEA emollient was associated with improved scaling, dryness, and itching at day 28 of therapy.13
Oral CBD has also been studied for this inflammatory skin disease. Callaway et al found that oral hempseed oil improved AD in a randomized crossover study demonstrating a significant decrease in skin dryness, itchiness, and use of topical medications compared with olive oil.14
CBD: Emollients and Skin Care
CBD has been gaining popularity as a cosmetic material because of its reported emollient and anti-inflammatory properties. Through an examination of online search trends, a 2019 study found that consumer interest greatly increased in topical CBD products from 2015 through 2019.15 CBD has been shown to increase dermal water content and skin turgor.16 CVS.com lists nearly 50 personal care products with CBD, including muscle relief creams, moisturizers, massage oils, facial cleansers, facial masks, antiwrinkle creams, bath salts, and eye serums.17 Sephora.com lists a $125 full-spectrum CBD luxury beauty serum, whose description states that whole plant cannabis molecules penetrate quickly to visibly calm redness, clear blemishes, and deeply restore.18
Research may support its use here, as well. A mouse model investigating the emollient effect of CBD found that a 1% CBD solution applied to the skin increased the dermal water content through upregulation of aquaporin-3.19 An in vitro study using normal human epidermal keratinocytes found that CBD is able to penetrate keratinocytes and reduce the oxidative stress caused by UVB irradiation and hydrogen peroxide.8
An ideal property of a moisturizer is to treat the pruritus associated with xerosis, and multiple studies have found that anandamide, an endocannabinoid, possesses antipruritic properties via inhibition of TRPV1 (an ion channel expressed mainly in nociceptive neurons of the peripheral nervous system).20
Future Directions
CBD represents an exciting area of pharmacology in the treatment of skin disease and for use in skin care and cosmetics products. CBD has been studied for the treatment of eczematous eruptions, skin cancer, pruritus, fibrotic skin diseases, acne, seborrheic dermatitis, psoriasis, and skin rejuvenation.20 However, most studies are either in vitro studies or clinical studies with only a small number of patients. Currently, double-blind, randomized, placebo-controlled clinical trials for the use of CBD in dermatology are lacking. Preliminary studies show potential benefits but more, well-conducted trials are needed before we can offer definitive recommendations to patients. It is certainly possible that formulations of cannabinoids can be highly effective, and I remain cautiously optimistic about this. However, that means for nowwe are in a tough place and are not really able to recommend cannabinoid products routinely, in my opinion, Lio concluded.

When the night is done at her familys Turkish restaurant Seyda Coskun goes home, washes off the kebab oil from her shift and frets. Its like this most nights and has been for some time.
As one of the remaining stores on Downtown Taurangas Devonport Rd, this is the harsh reality for those trying to make ends meet with a struggling business in a cost of living crisis.
While all passer-bys might see are shopfronts among For Lease signs, and construction sites there are whole worlds of financial stress, waning hope and family burdens that toil within these businesses.
Turkish restaurant and family business Abrakebabra opened in 2008 on Devonport Rd.
Abrakebabras manager Seyda Coskun says: Back then the street was a lot more profitable.
We actually existed back then. Years ago the environment was a lot more lifted. It was happy and people were more willing to spend money.

More hours, no paychecks
Now, the business is taking it week-by-week. Seyda says the businesses is struggling with a lack of customers due to ongoing construction sites and lack of CBD parking.
Our family is working far more hours and were not taking home pay cheques anymore because our staff need the pay cheques.
She says restaurant losses are 40-45 per cent some weeks. With financial stresses and personal bills to pay, Seyda says its impossible to switch off from the business.
It never goes awaymost nights Im just worrying because its a family business and it means a lot more than just working for another company. We are all we have. You go home, take a shower, and you wash all the oil and the kebab off you and just worry. Its awful.
Construction continues
Ongoing construction work on Devonport Rd and Elizabeth St is a huge problem for downtown businesses, says Seyda.
Were not getting any notices when parts of the road are all blocked off. There is no compensation available...so were out of pocket.
Devonport Rds construction sites include private developments and council projects. Tauranga City Councils future office block at 90 Devonport Rd is expected to be complete early-2025.
Seyda says it would help customer foot traffic if construction work took a staggered approach.
Itd be great if they could tackle one part of the city first, even one half of the street, then the other.
While we will try to mitigate disruption where we can, we do need to balance it with making sure the transformation can take place as quickly as possible so we can get the heart of our city pumping again and local businesses can start benefiting from that," says Tauranga City Council's general manager of city development and partnerships Gareth Wallis.
Carpark spots and costs
Seyda says the CBD parking situation is also damning business on both customer and staff fronts.
If customers can even find a park, theres then the parking charges themselves and theyre quite expensive. Cost of living as you know is going up. Our staff are leaving because they cant one, find a park; and if they can its too expensive which is just damning all of our businesses again.
Gareth says TCC has been closely tracking city centre parking availability.
Recent data from June 2023 shows, on average, only 50-60 per cent of on-street carparks are being used Monday-Friday. In parking buildings, during peak hours, no more than 70 per cent of carparks are used at any given time.
Short-term plans are also progressing to develop a temporary carpark at 160-176 Devonport Road."
Business strategy
Tauranga Business Chamber CEO Matt Cowley met with struggling downtown businesses last week.
Some businesses need help with business strategy to diversify their revenue sources, some need help with cash flow forecasting as their liquidity run-way is month-to-month, others need help renegotiating leases with landlords. The two most common issues are worsening lack of parking and the growing antisocial behaviour.
Seyda says antisocial behaviour in the CBD, making customers and staff feel unsafe, isnt helping business either.
So is there enough support for these CBD stores who have been in our community for years? Matt says if youre city centre business, the answer is: No.
If youre a ratepayer in other parts of Tauranga whod be paying for the support, then maybe. Ultimately, if you ask a CBD business owner what support they want the most, its more carparking.
Gareth says council has been implementing several short to medium term solutions to support local businesses at this time, through the City Centre Development Incentive Fund.
Some of these initiatives include: partnering with Tauranga Business Chamber to provide free targeted support and mentoring to each businesss unique situation; and funded access to the Business Navigator programme managed by Mentoring NZ to ensure city centre businesses have expert advice and resources to get through this period of development," says Gareth.
"The unfortunate part of that is that it takes us away from the business and we cant leave the business theres just no staff so it makes it a bit tougher on families," says Seyda, in regards to business mentoring support.
"I appreciate it takes time to do anything. I just think they [TCC] need to appreciate weve already given them a lot of time and theyve caused us nothing but worry. No reassurance. No support. No real change.

Downtown Taurangas Chair Ash Gee says the community is encouraged have the Covid-19 support local mind-set towards downtown businesses.
Continue to support your favourite businesses in the city centre that may be just a little bit harder to get to presently. Community support is required to help get our city centre businesses through this transformation phase.
Abrakebabra doors remain open, while other businesses of Devonport Rd have shut up shop.
Its really sad, and its kind of a reminder that that could happen to us which is really hard driving past all the closed businesses every day. It makes you think that really could be us, and who knows in however much time. Its quite scary.
