On a quiet block in Scarsdale, a physician-turned-consumer-superhero has spent two decades helping keep the $36 billion U.S. supplement industry honest. And COVID-19 hasnt slowed him down.

Tod Cooperman, MD, 58, is founder and president of the Westchester-based ConsumerLab.com, LLC (CL), a leading provider of independent testing and information about health and nutrition products. For 21 years, CL has reviewed and rated more than 100 categories of such supplements; over 6,000 products.
Cooperman has also testified before Congress; raised alarms about tainted products; pinpointed manufacturing deficiencies; and become an unofficial consumer watchdog for the 84 percent of Americans who regularly take supplements. This year, some vitamins, herbal remedies, and treatments touted as COVID-19 preventives were added to that list, with reviews available in CLs Coronavirus Information Center.

The guiding question when ConsumerLab began was the same as today; what would I tell my mother if she asked which specific supplements were safe, says Cooperman. Based in White Plains, with an office in Coopermans home, and a Vernon, N.J. laboratory, CL maintains an updated A (acai) to Z (zinc) directory of products sold in vitamin stores, pharmacies, supermarkets, mass merchandisers, discount clubs, catalogs, health care practitioner offices, and other venues. The companys 12 staffers occupy the space between federally-approved pharmaceuticals and largely unregulated over-the-counter remedies. There, Cooperman knows, pretty much anything can and does go.
The most surprising thing has been realizing how often a manufacturer is simply unaware its product has a problem, he says.
Weve seen vitamins that have up to three times the correct amount of an active ingredient and weve seen the opposite, where the ingredient isnt there at all.
He cites a popular cooking oil that contained none of the oil it advertised; a womens multivitamin with high levels of lead; and a leading probiotic that offered only half the listed number of goodbacterial colonies. Then, there was the undigestible vitamin C tablet.
We tried to test it but couldnt break the pill with a hammer or anything else, Cooperman says. It couldnt be metabolized, so it wouldve been completely useless in the body.
CLs online-only findings are supported by 90,000 member-subscribers, 84 per cent of whom are consumers, says Cooperman. The rest are universities, libraries, and other institutions. Members can access daily-updated news, major probes that appear several times a month, 600 answers to supplement-related questions, over 1000 recall notices and warnings, and regular explanatory videos. A twice-weekly newsletter reaches over 200,000 people many, quite vocal.
And we really listen to what they have to say, says Cooperman.
During the pandemic. readers asked about products touted as immunity boosters, including vitamin D, vitamin C, magnesium, zinc, and elderberry. CL also examined COVID-19-related best-sellers, including masks, pulse oximeters (to measure blood oxygen), air purifiers, and hand sanitizers (which can be deadly).
Cooperman twice brought his concerns about dietary supplement quality to Capitol Hill, testifying before the House of Representatives Committee on Government Reform in 2006 and at the Senate Special Committee on Aging in 2010.
Hes served as a columnist, expert, and industry opinion leader, but never seen patients. At Boston University Medical School, he became so fascinated by the biopharmaceutical industry that, by graduation in 1987, his business aspirations had trumped any interest in a clinical practice. Instead, he launched a company to track consumer satisfaction with some aspects of managed care, while seeking the perfect entrepreneurial opportunity.
It arrived in 1994 when the Dietary Supplement Health and Education Act (DSHEA) allowed manufacturers to make limited claims about their products ability to maintain good health. The law sent vitamin sales soaring but left consumers without reliable information about what they were buying.
The FDA (US Food and Drug Administration) would occasionally come in like a firefighter to douse flames when there was an obvious problem, but they were otherwise hands-off, Cooperman says.
That bothered him.
He sold his company, hired a former FDA tester, established his lab, and launched the first independent, scientifically-based /consumer-focused evaluator of supplements. Where the renowned Yonkers-based Consumer Reports sometimes assessed a single supplement, CL would monitor all products, continually and in depth.
Our goal was to go beyond the best or worst products to drill down to detailed clinical information about whether a product would work or not including safety concerns and drug interactions, says Cooperman.
CL (like Consumer Reports) ensured totally objectivity by refusing advertising, payments, or donations, and by purchasing products for testing itself, from neighborhood stores. Manufacturer samples were not allowed. Subscriptions covered operating costs. (Although CLs voluntary Quality Certification Program assesses products for a fee, permitting those that pass to display a certification badge. Trusted nonprofit U.S. Pharmacopeia or USP also offers certification but reviews fewer products.)
In the years since DSHEA, the FDA has become somewhat tougher on non-prescription drugs, but Cooperman still considers their efforts weak and inconsistent.
Thats not true in the Cooperman home, where his wife and three grown children are routinely drawn into his supplement probes.
My familys acutely aware of the pros and cons of every test. Its common dinner table conversation, he says.
Cooperman regularly takes Vitamin D when sunshine is scare. Hell occasionally add vitamin B12 and protein powder, a necessity for a mostly non-meat-eater. Hes such an ardent fan of dietary fiber (in food) that his college-aged kids call him Fiberman.
Every morning I eat two bowls of hot steel cut oats with kasha, fruit, nuts, and Greek yogurt, he says.
The Fiberman diet helps sustain his energy, but the grateful feedback he gets from subscribers is just as galvanizing.
The thing we most commonly hear is that CL is their go-to source before buying these products, says Cooperman. That makes me and my CL colleagues feel great; motivated, and focused.
And, he adds, its reassuring to moms, too.

Dr Sotirios Karathanasis discusses the history of cannabis as a treatment and delves into the process of turning plant-based medicines into clinically proven drugs.
Over the past few years, cannabidiol (CBD) has entered mainstream use as an ingredient with a range of proposed and proven health benefits. While the first US Food and Drug Administration (FDA) approval came in 2018, the history of CBD as a treatment for neurological disorders goes back millennia.1 Below, we delve into the history of this controversial substance and why it has taken so long for western medicine to tap into its therapeutic potential. More importantly, we discuss ways to expedite the process of turning historically used plant-based medicines into clinically proven drugs by harnessing the complexity of the natural world to tackle todays biggest healthcare challenges.
The introduction ofcannabisinto western medicine came in the 19thcentury. In 1843, Irish physician William OShaughnessy published a pre-clinical and clinical study on the effectiveness of Indian hemp extract. Several other physicians wrote on cannabis formulations and their efficacy in various conditions. Shortly after, in 1881, renowned neurologist Sir William Gowers published a book called Epilepsy and Other Convulsive Disorders, where he described how cannabis successfully treated a bromide-resistant case.
Despite scientific recognition of the benefits of cannabis, western societys use of the plant decreased significantly in the first decades of the 20thcentury. Cannabis became illegal in many countries just as the era of scientific study began in earnest. Despite its potential, knowledge of its efficacy and value remained limited to anecdotal experience and small clinical studies. As a result of new laws and regulations, many synthetic medications which aim to mimic the effects of illicit drugs such as cannabis were developed as part of western medicine. These drugs became the first-line treatment for various diseases, leaving cannabis behind.
Between 1930-1940, 9-tetrahydrocannabinol (9-THC) and CBD were isolated. Since 9-THC has psychoactive effects, researchers looked to CBD as a potential anti-seizure treatment. However, the lack of known molecular targets in the central nervous system hampered the characterisation of these molecules until the discovery of cannabinoid receptors (CB1 and CB2) and the identification of an endogenous cannabinoid system in the brain (endocannabinoids) in the late 1980s and early 1990s.
Despite decades of hesitation, the efficacy of cannabis paved the way for it to become more widely used, especially in cases for which no existing drugs worked.
In 2012, a patient suffering from seizures due to Dravet syndrome, was administered a low-9-THC cannabis oil, which brought about a sharp decline in her seizures.Shortly after this, GW Pharmaceuticals initiated a Phase I trial in 2013 to test Epidiolex (CBD) in epileptic children.3 The Phase III clinical trial concluded in 2016, leading to approvals by US and European medical agencies.
Despite a long history of successful use, effective drug candidates like CBD would have been missed with the traditional drug discovery process. Scientists at Enveda use a process called reverse translation, where they start with plants that have a successful history of use, then identify leads that can be optimised into drug candidates. Utilising both omics data and laboratory assays, aids in the identification of novel targets and disease pathways addressed by these molecules.
Typical high-throughput drug discovery processes start from druggable protein targets, such as neurotransmitter receptors. Scientists then find molecules that bind to and influence a given receptors activity. Since epilepsy is caused by overexcitability of neurons, anti-epileptic drugs typically target receptors that control neuronal excitability, such as GABA and glutamate receptors. Interestingly, neither of these are bound by CBD.4 Unlike 9-THC, CBD also does not interact with cannabinoid receptors at concentrations typically achieved with oral intake.5
It is unclear how CBD reduces seizures exactly, but it does not prevent the compound from being a highly effective medicine for drug-resistant epilepsy. One likely target is the Transient Receptor Potential Vanilloid 1 (TRPV1), which is generally present in high levels in the temporal lobe of the brains of epileptic patients. CBD also interacts with many important neurotransmitter receptors, including serotonin and opiates.5 In fact, CBD might be effective due to its combined effects on multiple targets both inside and outside the brain rather than on one target alone.6
In spite of recent successes, the potential of even cannabis-based medicines remains largely untapped. Our studies, for example, suggest the presence of several novel cannabinoids and related phytochemicals. Hemp-derived CBD oils are now widely sold over the counter and used for a variety of ailments from chronic pain to insomnia, psychiatric disorders and autoimmune diseases. Existing medications for these conditions often have limited efficacy and/or harmful side effects. Cannabis plants, by comparison, are well tolerated. Robust scientific studies are just beginning to answer these initial questions. Beyond cannabis, there may be value in thousands of plant species with a prior history of medicinal use, but with unknown mechanisms of action.
We recognise the limitless opportunities in systematised reverse translation to find effective natural products, especially for therapeutic areas that are not well addressed by current medications.

So, how did CBD affect me?
Im not the best sleeper. Scratch that. Im not the best rester. What does that mean? Well, while I do get my eight hours of sleep every night, I cant exactly say that these are quality hours.
Oftentimes, I wake up more tired than I was the night before. As such, I was looking forward to seeing how the CBD was going to improve my sleep patterns especially since research has shown that CBD is an effective sleep aid.

I began taking 20 drops of my daily CBD Oil Drops (making sure not to exceed the recommended daily dosage of 20 mg CBD) before I went to bed. Per the instructions, I placed the dropper under my tongue as this is the fastest way for your body to absorb CBD, and I was soon off to bed.
The next day, I woke up feeling quite refreshed and even chuckled at the extremely vivid dream I had the night before (featuring a very handsome Sebastian Stan). Well, I forgot to take CBD for one night. The next day, I woke up feeling more tired than I had in weeks (I also didnt have memorable dreams). Coupled with the fact that Ive managed my anxiety levels and that Im on top of my fitness game, CBD is definitely the tool for longevity.
