Integrative dermatology certainly has a place in the optimizing of care. However, some patients may desire to use only alternative treatments and distance themselves from the more conventional and pharmacologic medications. How the dermatologist responds can be crucial, not only for the desired treatment outcomes, but for their relationship with the patient.

In this second and final part of an interview with HCPLive, Peter Lio, MD, Clinical Assistant Professor of Dermatology & Pediatrics, Northwestern University Feinberg School of Medicine, discussed the importance of balancing integrative approaches with conventional care. He also elaborated further on the reasons for why integrative medicine may appear to come more naturally for the field of dermatology.

HCPLive: Do you ever find yourself having conversations with patients regarding balancing conventional therapies with alternatives? I can imagine there might be some patients who are simply not looking to take a conventional route.

Lio: Honestly, I have. I find myself having that conversation every day. That conversation is very common, especially in my practice, which is sort of touted as being a place where integrative treatments happen.

Many patients come in and begin the discussion claiming they don't want any conventional medicine. And that's really tough, because it'd be like saying to a boxer, 'You can't use your right hand when you punch.'That's half of your game here. So, we do need to use both.

Sometimes I'll start patients on a very gentle approach, and I'll frame it as doing all of the gentler, more integrative approaches first. And then, as a rescue, if they're not doing great in the next weeks or months, we would then use some of the conventional therapies.

Patients would then say, 'Okay, I really like that, I'm on board.'

Whereas, if you say, 'We'll use this convetional therapy, and if you're better, we could try the namby pamby stuff that may not work,'that could ruffle the patient's feathers a bit.

So, if you spin it the other way, I think you can actually meet them halfway. Again, I think it actually can be a viable approach for many patients, too. We can reduce our reliance on more powerful medicines, but use them when we need them.

You mentioned before that you feel integrative approaches come naturally in dermatology. Certainly, in some ways, dermatology can be seen as a much more integrative fieldversus, say, cardiology or rheumatology.Why do you think that is?

I'd like to think it's because the skin is right there. Everybody gets to see their skin, everybody likes to try things.

There is an ancient history of people trying stuff on their skin, from poultices, to different dressings, to different oils. And it's one of those fields where you have to be pretty comfortable with pretty much everybody being the expert.

When I walk into a room, everyone tells me, 'My cousin is really good at skin stuff.' And it turns out that she has a YouTube channel and makes her own home concoctions and there's nothing wrong with that. But that's a little bit different than being a board-certified dermatologist. I mean, everyone has an opinion.

I often say, the antithesis of dermatology is ophthalmology. If something's wrong with your eye, nobody's messing with it. We would just say go see the eye doctor, and don't put anything in your eye.So, these kinds of diametrically opposed systems are fascinating.

As for hepatology, people can't access their liver, they don't know what's going on with it. So, it becomes this vague concept, whereas the skin is right there.

FARIDABAD, India, April 27, 2021 /PRNewswire/ -- ESIC Medical College and Hospital, Faridabad and Nisarga Biotech Pvt. Ltd today announced the final data from a pilot, double-blind, randomized, placebo-controlled trial studying prophylactic activity of a proprietary, patent-pending, Neem-leaf extract (Azadirachta Indica A. Juss) of Nisarga Biotech in healthcare personnel and relatives in daily contact with COVID-19 patients. The study was a planned collaboration of ESIC Medical College and Hospital, Faridabad & All India Institute of Ayurveda (AIIA), New Delhi, and was sponsored by Nisarga Biotech Pvt. Ltd, Satara, MS.

The study was conducted in 190 participants of aged 18-60 years at ESIC Medical College and Hospital, Faridabad, between August to December 2020 under the Principal Investigator Prof. (Dr.) Tanuja Nesari, MD, PhD, Director, AIIA & Co Investigator Dr. A. K Pandey ESIC Medical College & Hospital, Faridabad. The study was approved by the Institutional Ethics Committee and registered at Clinical Trial Registry of India (CTRI) and was conducted in accordance with Good Clinical Practice guidelines and according to all applicable local regulatory and ethical requirements.

Results demonstrated the probability of COVID-19 infection in participants receiving Neem capsules 50 mg twice a day for 28-days was reduced to less than half (a relative risk of 0.45) that of participants receiving the placebo, with the effectiveness of Neem capsules being around 55%. Biomarkers and Quality of life (QOL) remained stable in both groups.

Explaining the significance of the results, Dr. Anil Kumar Pandey, MD, Registrar, ESIC Medical College and Hospital, Faridabad, said, "Ayurveda has immense contribution in prophylaxis of various diseases. These study findings are a leap forward for prophylaxis against the COVID 19 specially when there is no specific treatment available for treatment of Covid-19 infection. I am sure it will work great as a preventive as well as additional protection even during the two doses of vaccination where chances of infection are there. Safety is another critical component for prophylactic measures, and we are encouraged to see that the Neem formulation studied in this study has a better and acceptable safety profile."

Dates, the sweet little jewels that grow on date palms in many desert and tropical areas, are packed full of important nutrients and make a great natural sweetener in recipes.

Dates have long been used in traditional and alternative medicines, as well as in religious traditions, and have a special significance for Muslims. Date palms are mentioned more than any other fruit-bearing plant in the Qur'an, and eating dates in the morning is believed to provide protection against poison and magic, according to Mariam Metwally, Accredited Practicing Dietician (APD).

Eating dates during Ramadan, the religious month (which started on April 12th and ends on May 12th in the US this year) has become a worldwide tradition, especially at the time of breaking the daily fast after sundown, sincedates are quick to raise blood sugar and restore energy levels. Dates are also a healthy source of nutrients, according to the latest research, which indicates that they provide healthy fiber and antioxidants and may even help protect against certain cancers.

Dates are high in minerals such as Magnesium (64.2 mg/100g), zinc (0.5 mg/100g), Iron (0.36.03 mg/100g) which are responsible for a number of body functions like immunity, digestion, and metabolism. They are also high in vitamins like B1 and B6, and when fresh, high in vitamin C. These are all essential nutrients that your body needs daily. This is particularly useful for those fasting as fasting may limit the potential to consume these vital micronutrients, in part due to the reduced hours of eating and also culturally, more indulgent meals enjoyed during this time, says Metwally.

Dates have just under 7g of fiber per 100g, making them great for digestion and maintaining regular bowel movements, which prevents constipation. Metwally adds that this is an added bonus for the month of Ramadan where constipation is a common concern. Eating fiber also prevents blood sugar from spiking by slowing digestion- making them fruits with a low glycemic index. This may be helpful for people living with diabetes when consuming dates in moderation as part of a balanced diet.

Dates are full of cell-protecting antioxidants, higher than other dried fruits. These antioxidants such as carotenoids, selenium, and flavonoids, are useful in preventing a number of diseases, cancers, chronic illness and may even improve fertility. For example, Phenolic Acid is known to be an anti-inflammatory compound. Phenolic acid is suggested to be useful in cancer prevention and heart disease.

Dates have been eaten for easing childbirth for centuries, as mentioned above. A 2017 study found that eating dates in late-term pregnancy aided cervical dilation reducing the need for induced labor and decreasing the duration of labor. They are also a convenient energy source for women to consume during labor to replenish energy stores.

Eating dates regularly has been found to reduce inflammatory compounds like IL-6 (in rodent studies) which causes degenerative function in the brain. This means that eating dates has the potential to prevent things like Alzheimer's disease, although more research is needed.

Eating dates have long been believed to be helpful in cancer treatment in traditional Egyptian medicine. While research is still developing, a 2019 study using Ajwa dates, conducted tests on prostate cancer cells. They were treated with a date extract (ethyl acetate fractions) and cell activity was monitored. The extract showed strong inhibitory signs on the cancer cells, a promising result for prostate cancer treatment. The study concluded that the ethyl acetate fractions from Ajwa dates may have therapeutic properties for prostate cancer.

One of the best things about dates is how many ways you can use them. They have the convenience of being ready to eat whole and easy to carry and store. Their caramel-like flavor is also a compatible flavor with so many foods. Many eat them stuffed with nuts - adding to their health benefits, while others use them in smoothies, or as a refined-sugar alternative in baked goods, sauces, and oatmeal.

Disasters are, unfortunately, a growth business, and the frontlines that were once distant have moved much closer to home. Wildfires, hurricanes, floods, tornadoes let alone a pandemic has forced much of the United States and increasingly large swaths of the world to confront a new reality: few places are existentially secure.

How we respond to crises can radically adjust the ledger of mortality for the people slammed by these catastrophes. Good information, fast response and strong execution can mean the difference between life and death. Yet, frontline workers often cant get the tools and training they need, particularly new innovations that may not wind their way easily through the government supply chain. Perhaps most importantly, they often need post-traumatic care far after a disaster his dissipated.

Risk & Return is a unique venture fund and philanthropic hybrid that has set its mission to seek and finance the next-generation of technologies to help first responders not only on the frontlines, but even after as they confront the strains both physical and mental from missions they undertake.

The family of organizations sees a spectrum from emergency workers in the United States to U.S. military veterans, all of whom share similar challenges and need solutions today solutions that can often be hard to finance for traditional VCs who arent aware of the unique needs of this community.

The group was founded by Robert Nelsen, who made his name as a co-founder and managing director of biotech VC leader ARCH Venture Partners, which last year announced a $1.5 billion pair of funds. Hes joined by board chairman Bob Kerrey, the former co-chair of the 9/11 Commission as well as former governor and senator of Nebraska, and managing director Jeff Eggers, a Navy SEAL who served as senior director of Afghanistan and Pakistan on President Barack Obamas National Security Council.

Nelsen had been thinking through the idea when he met Kerrey, who recalled the conversation happening during a fundraising event for Navy SEALs. There has been a lot of suffering for those who have been on the frontlines, Kerrey said. Bob had this idea, and I thought it was a really smart idea, to try to take a different approach to philanthropic efforts. They linked up with Eggers and the trio brought Risk & Return to fruition.

The venture fund is $25 million, with about 35% of it already deployed. The fund has had a big emphasis on mental health for first responders, with 75% of the companies funded broadly in that category.

The funds first investment was into Alto Neuroscience, which is developing precision medicine tools to treat post-traumatic stress. The fund has also invested in behavioral management startup NeuroFlow; alternative well-being assessment tool Qntfy; Spear Human Performance, which is a brand-new spinout focused on connecting commercial and health data sources to optimize human performance; and Xtremity, which is designing better connection sockets for prosthetics. The fund has invested in another six startups, including Perimeter, which I profiled a few weeks ago.

This isnt your typical venture portfolio, and thats exactly what Risk & Return wants to focus on. Eggers said that We love that type of technology since it has that dual purpose: going to serve the first responder on the ground, but the community is also going to benefit.

While many of the startups the firm has invested in obviously have a focus on first responders, the technologies they develop dont have to be limited to just that market. Kerrey noted that Every veteran is a civilian, [and] these arent businesses targeting the military market. Given the last year, its hard to find a human being in this pandemic that hasnt suffered at least some PTSD, referencing post-traumatic stress disorder. Sales to governments can be incredibly challenging, and the ultimate market for the kinds of specialized mental health services that frontline workers need may not be as commercially viable as one would hope.

While the government does research and innovation in this category, Kerrey sees a huge opportunity for the private sector to get more involved. One thing that you could do in the private sector that is difficult in the public sector is look for alternative therapies for PTSD, he said, noting that areas like psychedelics have intrigued the private sector even while the government would mostly not touch the category today. Risk & Return has not made an investment in that space at this time though.

Half of the returns from the fund will stream into Risk & Returns philanthropic arm, which writes grants to charities along the same thesis of aiding frontline workers both on the job and after it. The organizations hope that by approaching the complicated response space with a multi-pronged approach, they can match potential needs with different sources of capital that are most appropriate.

Weve increasingly seen this hybrid for-profit/nonprofit venture model in other areas. Norrsken is a Swedish foundation and venture fund that is investing in areas like mental health, climate change and other categories from the UN Sustainable Development Goals. MIT Solve is another program that is working on hybrid approaches to startup innovation, such as in pandemics and health security. While disasters are always looming, its great to see more innovation in financing this critical category of technology, such as in pandemics and health security.

Due to its characteristics of clearing congestion and bettering breathing, it is called a 'pocket ventilator' among Unani circles.
