Treating Major Depressive Disorder: A Lesson through the Ages – News & Features

The earliest records of depression date back to B.C.E. in Mesopotamia. Back then, this mental health disorder was considered a spiritual condition, attributed to satanic elements. This pervasive belief at the time resulted in oddball treatments such as physical restraint, severe beatings, and even starvation. Of course, these treatments failed to rid the patients of their depressive disorders.

During the common era, between 25 BCE and 50 CE, treatments for depression were equally savage and included incarceration, starvation, and severe beatings. During the common era, a doctor from Persia named Rhazies understood that depression was a mental health illness and recommended a variant of behavioral therapy to treat the condition.

During the Renaissance period, depression was generally perceived as an intractable weakness. Treatment options varied between medication, diet, exercise, and even music. Apothecaries encouraged vomiting, dietary modifications, riding horses, and enemas.

By the 19th and 20th century, the esteemed psychiatrist, Emil Kraepelin from Germany, distinguished schizophrenia from bipolar disorder. He also advocated for psychotherapy based on psychoanalysis.

Sigmund Freud furthered more in-depth studies of depression, with his theories associating melancholy with loss. He explained how loss resulted in unconscious anger and self-loathing. Freud advocated psychoanalysis be used to resolve these internal conflicts and to reduce the negative thought processes.

In the 19th and 20th century, various treatment regimens for depressive disorders became available. Unfortunately, these treatment options were ineffective or overkill.

Lobotomies, a surgical procedure that involves severing connections in the brain's prefrontal cortex, were commonplace but they resulted in severe personality changes, improper judgment, and an inability to make decisions; enter the era of ECT (electroconvulsive therapy).

The invasive ECT surgical procedure requires patients to undergo shock therapy with a psychiatrist, nurse, physician's assistant, and an anesthesiologist at the ready. It is believed that ECT induces seizures which has a positive effect on combatting depression. Further studies suggested that depression was the result of endogenous and reactive causes. Doctors also discovered various treatment options for depression in the form of isoniazid.

By the time cognitive behavioral therapy came around, it proved to be a highly effective alternative to the other depression treatments known as psychodynamic theory. Various SSRIs (selective serotonin reuptake inhibitors) such as Paxil, Zoloft, and Prozac are highly effective at targeting the brain serotonin levels with far fewer side-effects than other medications.

Tricyclic antidepressants were touted as treatments for depression, but they oftentimes came with serious side effects. Major Depressive Disorder (MDD) was coined by American clinicians during the 1970s and despite a large body of work surrounding MDD, there isn't a definitive answer to the root cause of this depressive disorder.

Psychiatrists, mental health counselors, and clinicians believe that depression is the result of social elements, biological factors, and psychological components. While the root cause of depression may be somewhat murky, the effects of depression are well known.

People who are suffering from major depressive disorder often struggle with major disturbances in activity levels, poor sleep habits, and gastrointestinal issues. Major depressive disorder is associated with high levels of anxiety.

Given the explosion in interest in depressive disorders and anxiety disorders, psychiatrists, psychologists, MDs, and mental health professionals now have a much better understanding of the disorder and how to treat it.

The medication used to treat depression Paxil, Prozac, Zoloft, divers targets neurotransmitters in the brain. However, medication may cause serious side effects which need to be monitored and addressed. For example, antidepressants may cause more problems than they are worth, with a meta-analysis of 17 studies that was published in 2017 in the journal Psychotherapy and Psychosomatics finding a 14% higher risk of heart attacks and strokes and a 33% greater risk of death for people who used antidepressants over a long time.

For patients who do not respond well to antidepressants and traditional therapy regimens, there are alternative options available. These include the aforementioned ECT (electroconvulsive therapy) and modern treatment options such as TMS (transcranial magnetic stimulation).

It should be noted that these modern procedures and treatment regimens are prescribed only when patients do not respond well to traditional depressive disorder treatments such as medication, therapy, and are at serious risk of hurting themselves or others.

While ECT is invasive, painful, and may have severe side-effects, the safe alternative TMS does not even require the patient to be sedated or anesthetized prior to the procedures.

Transcranial magnetic stimulation is a cutting-edge solution for major depressive disorder. It involves the passage of electromagnetic waves into the depression center of a person's brain, just beneath the skull.

The level of intensity of the TMS treatment depends upon the severity of the depression. By reconfiguring the neural activity in the brain through TMS, it is possible to alleviate many of the symptoms that cause depression to begin with.

TMS treatments have minor side-effects including dizziness, headaches, tingling, or pain on the scalp. These symptoms can be alleviated with over-the-counter pain medication such as Advil, Tylenol, or Aspirin.

With TMS, patients who don't respond to medication and therapy have another shot at a life free from depression. The treatment protocol requires patients to consult a licensed and regulated mental health counseling service where TMS is offered.

The TMS sessions range from 30 minutes to 45 minutes, 5 days a week for 4-6 weeks. The severity of the condition determines the intensity of the TMS treatment.

TMS will often be used in concert with therapy and medication for optimal effectiveness. The majority of patients who complete a TMS treatment regimen report significant improvement in general disposition and mood. Many patients also attest to a complete remission of their depressive symptoms.

Business in 2020.scattered, smothered, covered, diced and topped. For those unfamiliar with the terms those are but a few of the options available for hash brown orders at Waffle House. As apt as they are for a plate of shredded and griddle-fried potatoes, theyre just as apt to describe the chaos that descended on the freight-hauling business in 2020.

This photo from a Quality Transport driver shows a reported 200-truck line at a Costco distribution center part of the initial supply chain logjam as retailers like Costco, grocery store chains and retailers like Walmart and Target hustled to restock in the wave of panic buying and pantry stuffing in March and April.

AsCCJ reported in the first weeks of the pandemic, an initial surge of grocery and retail restocking (post panic buying and pantry stuffing) was in some ways masking a sudden and dramatic slowdown in freight movement, and fleets large and small felt the sharp bite of that economic slowdown. A few large publicly traded fleets, for example, reported voluntary temporary pay cuts for high-level executives. Many other carriers reported feeling the pinch, with abysmal rates and loads hard to find.

More than 100,000 trucking companies turned to federal assistance, according to data compiled byCCJ, either for a few thousand or a few million to help keep their operations afloat.

Those bleaker freight days led to a public spat over the rates brokers were offering at the time a spat that eventually spilled into the offices of the U.S. DOT, with the Federal Motor Carrier Safety Administration accepting comments on rule changes around broker transparency.

Those rock-bottom rates were mostly short-lived, though, with a dizzied economy rebounding in large part on the back of a wave of e-commerce and retail, with consumer habits shifting dramatically. Food consumption tilted away from restaurants and toward grocery stores. Housing and home improvement boomed, creating big demand for materials at places like Home Depot and Lowes. Demand for bikes, kayaks and exercise equipment soared. Lastly, manufacturing and other industrial areas of the economy were slow to come back. All of these changes upended the supply chain, causing many shippers and carriers to throw existing contracts out the window and opt for shorter-term deals.

Also, a lot of freight was diverted to the spot market, where rates saw a dramatic reversal in just the span of six months. There are a few lingering issues that could cause freight to slow or shift again heading into the new year.

DOT enacted a few key changes to drivers 14-hour on-duty clocks and added leniency to the use of the 30-minute break.

Hours of service reforms finalized and enacted.When midnight hit on Sept. 29, fleets and drivers had new tools at their disposal for managing their daily and weekly on-duty hours the first changes to hours of service since 2013 and the first significant overhaul since 2004. Use of the new flexibility options was somewhat metered in the first months as there seemed to be initial confusion among fleets and drivers as to how to utilize the new regs initially. In January, due to ongoing technical issues, many fleets and drivers had to wrestle with the Clearinghouse platform to register and use the system. Since then, and with updates by FMCSA to the platform, its operated smoothly.

Clearinghouse rule takes effect, sidelining nearly 50,000 drivers in its first year. Starting Jan. 6 this year, fleets were required to not only report instances of drug and alcohol violations to the U.S. DOTs new Drug & Alcohol Clearinghouse, but also were required to ping the Clearinghouse for every new driver hire and at least once for all of their current drivers. Through November, FMCSA reported that more than 50,000 drivers had hit the Clearinghouses rolls for violations of either drug or alcohol regs. Marijuana violations so far have made up more than half of those, with cocaine, amphetamines and opioids together accounting for another large block of violations.

When the Clearinghouse rule went live, the system faced repeated outages and downtime as a swell of new users logged on. FMCSA has since beefed up the system and it has reportedly worked well since.

Nikola Founder and Executive Chairman Trevor Milton resigned amid a firestorm of criticism sparked by a skeptical stock short-seller and subsequent inquiry from the Securities and Exchange Commission.

Truck makers turn headline makers. One owns a nearly 120 year old trucking legacy. The other is a six-year-old upstart that has yet to commercially produce its first truck. Despite their positions on opposite ends of the trucking timeline, Navistar and Nikola each underwent signifiant boardroom changes this year. Navistar in November finally OKd the its a matter of when, not if scenario of a long-speculated Volkswagen buy-out. Traton Volkswagens commercial truck unit previously held 16.7% of Navistar, which it acquired via a procurement joint venture and strategic technology and supply collaboration in 2016, and bought the rest for $3.7 billion in November.

The transaction will create the global automotive and trucking behemoth that Volkswagen has longed for, pitting the company firmly against the likes of Volvo and Daimler.

Nikola this year has been dragged through the highest of highs and lowest of lows, all in the span of just a few months. The Phoenix-area based hydrogen-electric upstart was briefly the darling of the NASDAQ stock exchange when the company went public in June.

Just weeks later, the stock cratered and Nikola Founder and Executive Chairman Trevor Milton resigned amid a firestorm of criticism sparked by a skeptical stock short-seller and subsequent inquiry from the Securities and Exchange Commission (SEC). By November, several employees and board members had received subpoenas from both the SEC and Department of Justice.

Driverless trucks come roaring back. After taking a backseat to electrification for most of 2019, autonomous truck technology reared its head again in 2020.

Navistar and Traton each picked up minority stakes in autonomous truck retrofitter TuSimple, which operates a fleet of 40 trucks in the U.S. Navistar bought into the company after two years of an ongoing technical relationship through TuSimples self-driving technology. Traton and Tusimple launched a developmental program to operate the first Level 4 autonomous hub-to-hub route in Sweden using Tratons Scania trucks.

Jim Mullen, the former Acting Administrator of the Federal Motor Carrier Safety Administration, joined TuSimple earlier this year to serve as its Chief Legal and Risk Officer.

Daimler Trucks and Buses, parent company of Daimler Trucks North America, took another step toward its goal of delivering commercially viable self-driving trucks in 2019 by acquiring a majority stake in Torc Robotics, a Blacksburg, Va.-based autonomous driving solutions provider and developer of the Asimov self-driving software. This year, the company doubled down in partnership with Google sister company Waymo.

Under the terms of the partnership, Waymo will integrate its driving technology with Freightliners Cascadia to enable autonomous driving. Freightliner who with the 2020 Cascadia became first trucking OEM with a Level 2 autonomous driving-enabled truck expects its autonomous Cascadia, equipped with the Waymo Driver platform, to be available to customers in the U.S. in the coming years. Waymos technology has logged more than 20 million miles on public roads in 25 U.S. cities, and 15 billion miles in simulation.

Autonomous trucking tech firm Locomation in late-Fall completed an on-road pilot program that showcased autonomous trucking technology for the SmartBelt Coalition, a collaboration of transportation agencies in Michigan, Ohio and Pennsylvania. Over the course of one week, two Locomation trucks performed three separate interstate routes under close observation from the involved organizations. One route started at the Pittsburgh Community Food Bank and delivered food donations to families in need in Toledo, Ohio.

Locomations Autonomous Relay Convoy (ARC) platform allows one driver to pilot a lead truck equipped with technology augmentation while a follower truck operates in tandem through Locomations fully autonomous system.

Putting AI in the cab. Throughout the year, a number video telematics platforms added new sensors that use artificial intelligence (AI) to detect and alert drivers to distraction, fatigue and other unwanted driving behaviors.

Lytx added a new configuration option that lets fleets disable or limit in-cab video recording for driver privacy yet benefit from greater risk detection for cell phone use, eating and drinking, smoking, no seatbelt, speeding, and inattentiveness.

Omnitracs acquired SmartDrive in September to drive the technology convergence of telematics and video-based driver safety systems.

Samsara created newdriver safetyoptions for commercial and private fleets that use its mobile platform and Samsara AI Dash Cams.The options include real-time detection and alerting for tailgating and distracted driving.

Video telematics provider SmartDrive has developed new SmartSense intelligent driver-assist sensors to detect unsafe speeds in bad weather conditions and sitting duck events.

New driver safety tools were introduced by weigh station bypass providers PrePass and Drivewyze. Both started programs that give carriers more than a lower pull-in rate for inspections, as their drivers can get a heads-up warning through apps and telematics systems for upcoming safety risks along their routes.

Both companies have extensive relationships with government agencies to collect road and traffic data to supply location-based safety information.

In September, ELD and fleet mobility provider Omnitracs acquiredSmartDrive, maker of in-cab driver- and road-facing camera systems. The companies plan to drive technologyconvergence of telematics and video-based driver safety.

The timing happened to be right for both of us to move in this direction, said Ray Greer, chief executive of Omnitracs.

In September at McLeod Softwares virtual user conference, load board and freight-matching platform Truckstop.com discussed a new TMS integration for brokers that use McLeods PowerBroker to post their Book It Now loads.

Real-time freight matching. Another major acquisition was made by Trimble to expand its Transportation division beyond technology used by asset and non-asset transportation providers. In January, Trimble acquired Kuebix, a cloud-based TMS for shippers.

Collectively, the commercial and private fleets that use Trimble products manage more than 1.3 million commercial assets in North America. Trimble said it is now able to offer those customers access to freight matching opportunities from more than 20,000 shippers and freight intermediaries that use the Kuebix platform.

Freight-matching also benefitted from new breakthroughs in speed and transparency of information. Companies that offer load boards and freight-matching platforms introduced new features that give freight brokersand carriers instant visibility of market rates, as well as ratings and reviews to develop instant trust.

Load boards, rating engines, transportation management software (TMS) systems and other databases quickly integrated these and other capabilities to speed freight transactions.

When freight brokers post loads to the DAT network, for instance, they have the option to designate them as Book Now. When carriers see this designation, they can lock in the rate and terms of the load using DATs app or website.

This allows the carrier to make the first handshake with a broker in a digital way, said Ken Adamo, chief of analytics for DAT.

In September at McLeod Softwares virtual user conference, load board and freight-matching platform Truckstop.com discussed a new TMS integration for brokers that use McLeods PowerBroker to post their Book It Now loads.

The integration allows brokers using PowerBroker to tap into capacity from thousands of carriers that login daily to Truckstop.com, said Brian Sowul, senior product manager for Truckstop.com.

Scientific articles in the field of physics are mostly very short and deal with a very restricted topic. A remarkable exception to this is an article published recently by physicists from the Universities of Mnster and Dsseldorf. The article is 127 pages long, cites a total of 1075 sources and deals with a wide range of branches of physics from biophysics to quantum mechanics.

The article is a so-called review article and was written by physicists Michael te Vrugt and Prof. Raphael Wittkowski from the Institute of Theoretical Physics and the Center for Soft Nanoscience at the University of Mnster, together with Prof. Hartmut Lwen from the Institute for Theoretical Physics II at the University of Dsseldorf. The aim of such review articles is to provide an introduction to a certain subject area and to summarize and evaluate the current state of research in this area for the benefit of other researchers.

In our case we deal with a theory used in very many areas the so-called dynamical density functional theory (DDFT), explains last author Raphael Wittkowski. Since we deal with all aspects of the subject, the article turned out to be very long and wide-ranging.

DDFT is a method for describing systems consisting of a large number of interacting particles such as are found in liquids, for example. Understanding these systems is important in numerous fields of research such as chemistry, solid state physics, or biophysics. This in turn leads to a large variety of applications for DDFT, for example in materials science and biology.

DDFT and related methods have been developed and applied by a number of researchers in a variety of contexts, says lead author Michael te Vrugt. We investigated which approaches there are and how they are connected and for this purpose we needed to do a lot of work acting as historians and detectives, he adds.

The article has been published in the journal Advances in Physics, which has an impact factor of 30.91 making it the most important journal in the field of condensed matter physics. It only publishes four to six articles per year. The first article on DDFT, written by Robert Evans, was also published in Advances in Physics, in 1979. This makes it especially gratifying that our review has also been published in this journal, says secondary author Hartmut Lwen. It deals with all the important theoretical aspects and fields of application of DDFT and will probably become a standard work in our field of research.

Let's admit it: It's been a pretty rough year for our neck of the solar system. But it's been a great year for scientists studying more distant reaches of the universe. From a colossal explosion to mystery burps deciphered, here were some of the top stories in physics in 2020.

What might have been the universe's most powerful known explosion was detected back in 2016 but it really happened over 390 million years ago. While the first four-legged critters crawled onto land, a supermassive black hole in the Ophiuchus cluster launched a jet that blew a gargantuan cavity in the surrounding gas. In 2020, astronomers revisited the old data and realized just how powerful that explosion was: five times 10^54 joules of energy. For perspective, that's enough energy to literally rip apart all 300 billion stars in the Milky Way and a hundred more galaxies.

If you want to navigate among the stars, you're going to need a map. And that's exactly what the European Space Agency's Gaia space observatory created, using data on over 1.8 billion cosmic objects. The haul includes stars near and far, asteroids, comets and more. Want to know the position, velocity, spectrum and more for 0.5% of the population of our galaxy? You're in luck. Over 1,600 papers have already been published with Gaia data, and astronomers will be sure to mine the database for years to come. And here's the best part: There's even more data to come.

In 2020, the world lost one of its foremost and celebrated supersmart folks, Freeman Dyson. A man of unbounded imagination, he is perhaps best known in popular science circles for his conception of the Dyson sphere. (He didn't name it after himself; that came later.) A Dyson sphere is a hypothetical megastructure that completely encloses a star to harvest 100% of its solar energy exactly the energy a hyper-advanced civilization might need to do hyper-advanced things. So far, astronomers have not detected any Dyson spheres in our galaxy or any others, but Freeman's dream lives on.

It was too good to be true: claims of solid evidence for life in the cloud tops of Venus, an otherwise hellhole of a world. The reasoning was based on phosphine, a peculiar (and stinky) chemical emitted on Earth by anaerobic bacteria. To get as much phosphine in the atmosphere as was claimed, scientists proposed, Venus would need a large population of airborne microbes. Alas, further analysis reduced the observed amount of the stinky stuff (to levels barely considered noteworthy, let alone a sign for life), and in some analyses, removed it altogether as just another noisy signal. Don't worry, alien life: If you're out there, we'll keep looking.

Everyone loves a good fast radio burst (FRB), right? The source of these enigmatic, energetic signals has been an annoying puzzle to astronomers for more than a decade. FRBs are fast, high-powered, frequency-hopping radio signals coming from all over the sky, which makes it hard to pinpoint their origin. But finally, in 2020, astronomers got lucky: They found an FRB source in our own cosmic backyard. Follow-up observations revealed the culprit: an exotic star known as a magnetar (a super- magnetized dead stellar core). Apparently, magnetars sometimes burp out a tremendous amount of pent-up energy, which appears to Earthbound observers as a quick blast of radio emission.

Mars has liquid water. No, it's bone-dry. No, wait; it sometimes has water. No, nope, never mind. The Red Planet has been teasing astronomers for decades on the vital question of whether it's home to any liquid water at all. Astronomers care because, where there's water, there's a potential home for life. Earlier this year, astronomers claimed that there isn't just one, but four lakes of liquid water on Mars. The catch? They're incredibly salty more like a briny sludge than something to take a dip in and buried under a mile of frozen carbon dioxide at the southern polar cap. Not everybody is convinced, though, so don't pack your Martian swimsuit just yet.

2020 was surely the year of the solar system. Three independent spacecraft have successfully acquired samples and sent them on their way back to Earth. NASA launched its OSIRIS-REx mission to the asteroid Bennu, which collected so much material that its sample container leaked. The Japanese Hayabusa2 mission took a poke at the asteroid Ryugu and landed the material safely back to Earth. And the Chinese Chang'e 5 lander went on a mission to the moon, managing to launch a sample back to Earth before the lander broke down.

Astronomers have used gravitational waves (ripples in the fabric of space-time) to observe so many black hole collisions that by now, it's hardly newsworthy. But in 2020, astronomers announced the discovery of the biggest collision yet: a titanic merger of an 85-solar-mass black hole and a 66-solar-mass black hole. Post-merger, the resulting black hole tipped the scales at 142 times the mass of the sun. (About nine suns' worth of mass was converted into pure energy.) In other black hole news, the universe's ultimate Pandora's box was the subject of this year's Nobel Prize in physics.

Superconductors are super-neat. Due to the weirdness of quantum mechanics, under very special conditions, electrons can buddy up, with the pairs traveling together without losing energy. That means a game-changing technology where electricity can flow forever without resistance. Unfortunately, to make superconductors work, physicists have had to make everything super-cold. But in 2020, researchers announced the discovery of a superconductor at nearly room temperature, just 59 degrees Fahrenheit (15 degrees Celsius). The catch? You need to re-create the pressures found in Earth's center.

The novel coronavirus SARS-CoV-2 has devastated humanity, reaching pandemic levels in only a couple of months and washing across the globe. But we're fighting back with one of our most powerful weapons: vaccines. The current vaccines target a very specific part of the virus, a "spike" protein that it uses to invade our cells. One of the first steps in the war against COVID was to identify and map that protein, which researchers accomplished earlier this year, using a physics-based technique called cryogenic electron microscopy. Using this map, drugmakers could target this feature of the virus for vaccines to mimic, giving our immune systems a fighting chance.

The Schrodinger Equation appears in Criminal Minds – Looper

In popular culture, the physicist Erwin Schrdinger is most well known for his Schrdinger's cat thought experiment in which a cat exists in a closed box with a poison that has a 50/50 chance of killing it. At this moment, the cat is said to be both dead and alive at the same time. Thankfully, he did not actually subject cats to this, as it was merely a way to think about small particles like atoms, which are what quantum mechanics is all about.

Schrdinger's equation therefore looks at the probability that a particle is in a particular state, or, sticking with the cat, the probability that the cat is dead or alive. However, there's a gap of knowledge between that state of limbo and the moment when the particle is observed (aka when we find out the fate of the cat). The "many-worlds" theory supposes that all options do actually happen, sprouting off into different versions of the universe. There would then be a world where the cat is alive and one where it is dead. So, according to this complex physics theory, there is another world where the unsub's son isn't dead. However, sacrificing a bunch of people is not what's going to get him there.
