Australia suspends Hong Kong extradition treaty and offers visa extensions – Sky News

Australia has suspended its extradition treaty with Hong Kong, and offered visa holders from the territory a five-year extension with a pathway to permanent residency, in response to China's imposition of a national security law.

The 10,000 Hong Kong citizens in the country on student or temporary work visas are likely to benefit from the move.

"There will be citizens of Hong Kong who may be looking to move elsewhere, to start a new life somewhere else, to take their skills, their businesses," said Prime Minister Scott Morrison.

He also made a pitch for international financial services, consulting and media businesses with regional headquarters in Hong Kong to relocate to Australia.

"If there are businesses that wish to relocate to Australia, creating jobs, bringing investment, creating opportunities for Australia then we will be very proactive in seeking to encourage that," he said.

Acting Immigration Minister Alan Tudge said he expected the numbers of Hongkongers who would come to Australia under the new arrangements would be "in the hundreds or low thousands".

The new security law drawn up by Beijing makes secessionist, subversive, or "terrorist" activities illegal in Hong Kong - as well as foreign intervention in the city's internal affairs - with those found guilty facing punishments of up to life in prison.

Critics fear the law will be used to target protesters and opposition politicians seen as disloyal to Beijing.

Mr Morrison said: "Our government, together with other governments around the world, have been very consistent in expressing our concerns about the imposition of the national security law on Hong Kong.

"That national security law constitutes a fundamental change of circumstances in respect to our extradition agreement with Hong Kong."

Australia has also updated its travel advice for Hong Kong, where around 100,000 Australians live and work, to say they "may be at increased risk of detention on vaguely defined national security grounds".

The action follows similar moves by the UK, which has offered a path to citizenship for nearly three million people in Hong Kong who were eligible for British National Overseas status ahead of the handover back to China in 1997.

Canada has suspended its extradition treaty with Hong Kong and is looking at other options including migration.

New Zealand said on Thursday it was also reviewing its relationship with the territory including extradition arrangements, controls on exports and travel advice.

Relations between Beijing and Canberra have soured since Australia called for an international inquiry into the source and spread of the new coronavirus, which first emerged in the Chinese city of Wuhan.

China has since banned Australian beef imports and imposed tariffs on Australian barley, and urged Chinese tourists and students to avoid Australia.

Though China remains Australia's largest trading partner, the latest announcements are likely to aggravate tensions between the two countries.

Rejected twice but controversial extension will now go ahead ending 18-month saga – Droitwich Advertiser

A CONTROVERSIAL plan to build an extension already rejected by the council and a government inspector will now be allowed to go ahead bringing an end to an 18-month saga.

Worcester City Councils planning committee and the governments planning inspector both rejected an application for a side and rear extension to the existing five-bed HMO in Bozward Street over parking and congestion fears.

The plan for the extension returned after developer Bob Panesar successfully gained a 'lawful development certificate' from the council in January for the proposed work and submitted another planning application in May.

Councillor Jo Hodges said councillors were in a difficult position having already refused the plan but then the council had given Mr Panesar a lawful development certificate which said the work could go ahead, meaning the committee had little option but to approve the plan.

She said it felt like councillors were deciding on something that had already been approved and allowing it would make life a bit more comfortable for the people living in the house.

Cllr Pat Agar said it went "against the grain" for her to approve the plan and she was only saying yes on pragmatic grounds.

"I'll have to vote yes for it, I don't like it, but there you are," she told councillors at a city council planning committee meeting last Thursday (July 9).

The plan was rejected by councillors in February 2019 after it was called in by Cllr Richard Udall, who represents St Johns, over the impact it would have on neighbours and parking in the street.

Mr Panesar appealed to the governments planning inspectorate to reverse the decision but it was thrown out after an inspector said it would make parking worse.

Cllr Udall said again at the planning meeting last Thursday that neighbours were concerned by the plan but he feared he was talking about a retrospective planning application as he believed most of the work had already been carried out.

Boeing Apache Leads The Way – Seeking Alpha

With the travel industry experiencing huge pressure, manufacturers of commercial aircraft also have seen their business shrinking significantly. While around the globe we are seeing countries and states reopening, the travel industry still is under pressure as the recovery is likely going to take years. Among my readers, that has sparked a renewed interest in defense. That is why we launched the Defense Contracts Monitor which we expand along the way as it forms the basis of our expansion toward covering defense and is accessible to subscribers of The Aerospace Forum.

In June, Boeing received nearly $600 million in contracts, but the funds obligated was slightly higher. This might sound weird, but it does happen in some instances that no amount of money changes hands upon contract award and when the contract is modified (for instance due to an extension), the cash from a previous award is being handed over as well. So, that's how Boeing ended up with 101% of funds received during the month.

Nearly 80% of the contract awards was for the AH-64 Apache as Morocco became the 17th country to order the attack helicopter of which Boeing has now delivered 2,500. The contract covers the procurement of 24 AH-64E Apache helicopters as well as Longbow trainers, a simulator to train crews and rehearse missions. Apart from that contract, Boeing received some contracts for the P-8A to perform certification and test flight activities, training devices for the F-15 for Saudi Arabia, upgrades to the Super Hornet and Growler and a contract for the B61-12 Life Extension Program. Unsurprisingly, Morocco accounted for the bulk of the funds during the month: Almost 75%.

Month over month, we saw a decline of $5.6B driven by lower P-8A and Harpoon sales. It's a sharp month-over-month drop, but nothing truly concerning. Year-over-year there's a decline as well as Boeing last year received a $6.5B contract for a five-year extension for the Joint Direct Attack Munition (JDAM), which turns "free fall bombs" into precision-guided munition.

So, we are seeing contract declines but I'm not too worried about that. Overall, we are seeing that some key programs continue receiving contracts providing Boeing with some useful cash at the moment. When Boeing received some big contracts in previous months, we saw some readers claiming that Boeing was being granted contracts by the US government to infuse the company with cash. That's not completely true, as 70% of the contracts granted during the year came from foreign military sales.

Where the difference is made progress payments and from what I can see more funds obliged at the time of contract award. Progress payments have been increased and that provides the biggest boost as the company now receives 90% of the costs it makes on defense contracts instead of the usual 80%.

During the month of June, Boeing received almost $600 million in defense and services contracts marking a month-over-month as well as a year-over-year decline. Noteworthy is that while year-to-date contracts awards fell >$40B to $7.3B, obligated funds have been in line with last year. We don't quite know how much of those obligated funds Boeing keeps and how much is distributed among suppliers, but for Boeing having these funds come in is important given the state of the commercial aircraft industry. Just like last month, the order inflow wasn't carried by the United States to infuse Boeing with cash.

Important to realize is that just like with the commercial aircraft business, defense and associated services have contract time frames spanning multiple years and generally revenues also will be recognized over that time frame. So, the contract awards we are seeing now do not translate directly into revenues nor do they actually offset the declines at Boeing Commercial Airplanes contrary to what many believe.

These days, I cant make out where one activity ends and the next begins. There are mornings when I am stuck between a sink of unwashed dishes, an article that requires editing on the double and a homework submission that is not even mine to finish. I feel like Ive fallen headlong into one long, unbroken activity with neither beginning nor end.

In the initial days of this new mode of existence, my brain very presciently read the hashtag #WFH as #WTF. Now, of course, Ive become an expert at asking friends and acquaintances if they, too, are working from home or simply, WFH.Only, its not very clear to me, and it seems to those I am enquiring after, what this now commonplace term really signifies.

If we turn to the media, we have to be mostly content with advice on the right daily routines or the right furniture to achieve that perfect #WFH life. Online clothes retailers, Ive realised lately, are one step ahead. There are now whole #WFH lines that you can order to wear to that all-important, ten-thousandth Zoom call.

Then, there is a glut of material warning us about the mental health impact of #WFH during these lockdowns and after. Although some of it has the merit of being well-intentioned, its largely uninspiring stuff. Youll sooner find me outdoors in a crowded market area than browsing the internet for tips on how to maintain my mental wellness at this time. I suspect that many like me are unconvinced about mental health being just another internet keyword that can be maxed with listicles about what to do and what to avoid. All the same, it is becoming clearer that all of us need to process what has been happening over the past few months.

As lockdown has progressed into unlock, I sense that there are many more people who are feeling cut-off from themselves. Older people, for whom the act of going to the neighbourhood shop was never just that, want a breather. Every new extension of local containment measures seems to sap their spirits a little more. Children of all ages, but especially those for whom hanging out with friends is the most crucial aspect of their lives at this stage, are unable to understand why things cant get back to normal. Those who are constantly on, whether working or doing something else, find themselves hostage to mind-numbing information loops that cant be turned off.

For many, it may seem to crush their aliveness. Ive caught myself feeling something close to total indifference towards all and everythingon more than one occasion these past few weeks.

With no clear end to the epidemic in sight, it is becoming apparent that the work of tending to our deeper emotional needs is something that each of us must undertake in earnest. I am not speaking here of people who might need help to manage existing mental health conditions or even emerging ones. These need to be looked at by doctors and professionals in the field.

For many, there is the work of asking what it means to be really home and safe. To reflect upon what we mean when we speak of a home. While there is the physical structure and a place where we can shelter in place, what we are also talking about is a refuge. A sense of emotional security that was hopefully built up while being brought up by people who cared for us. Despite the widespread prevalence of the infection and the fear that we, or our loved ones, could be affected, are we able to locate an anchoring place, a home, from where we can operate on a day-to-day basis?

It seems to me this is what really #WorkFromHome means right now. This task of locating our homes and working from there is as important as, say, attending the next Hangout meeting or catching an upcoming deadline. As for me, Ive found that I can cut through my lack of interest by doing what I love best -- writing in my journal.

Running the richest cricket board amid a global pandemic has been one of the toughest challenges in BCCI president Sourav Ganguly's life as an administrator but the former India captain is no stranger to dealing with ups and downs in life which is why he has accepted the current situation and working around the problem to make sure that Indian cricket doesn't suffer.

Ganguly was appointed the president of the Board of Control for Cricket in India (BCCI) in October last year and his tenure will end in September this year. But so far, 4 months out of his 8-month tenure have been hit by the coronavirus crisis.

Whether he will get an extension or not still remains uncertain. Rumours are also doing the rounds about Ganguly looking to run for the president's post in the International Cricket Council.

Talking about his tenure so far in the BCCI, president Sourav Ganguly said he has accepted the situation and doing his best to keep everything on track for Indian cricket amid the Covid-19 pandemic.

"This is unreal. For 4 months we havent been to the officie in Mumbai. This is my 7th or 8th month as BCCI president out of which 4 months have been taken away by Covid.

"So we have been working on video conferencing but thats what it is. I cant change it. I have to accept it and try and do the best I can in this situation.

"I dont know whether we will get an extension or not. If we dont, we dont, I move on to something else. At the moment, the last 8 months as BCCI president, 4 months I have been sitting at home," Ganguly said on the latest episode of India Today Inspiration.

International cricket finally resumed after 117 days with England hosting the West Indies in a 3-match Test series. Other sports like football, tennis and Formula One restarted before that. With things slowly resuming, Ganguly sees light at the end of the tunnel with sport coming back elsewhere.

"I can see light (at the end of the tunnel). All of us want to see light, because we want to get out this and see normal lives back where we can hug each other, shake hands with other without being scared, sit next to each other without being scared. Sports has started, football has started, tennis has started which is good because life cant wait.

"Having said that, the fear is a bit more than what more than what it was in March and April. The numbers are a bit more than what it was in March-April, especially in India.

"This is a unique country, so from that point of view, as much I am optimistic, my optimism comes with a little bit of fear. I go to the office to work. I am scared. Fifty percent of the people I work with have been told to work from home. I still have 3 or 4 people who are working who are still scared. I am exposed, my people are exposed.

"My brother works, my brother runs all our factories. He is the most exposed. All of us are exposed. The problem is there is not one person who is not exposed and knows how not to get exposed," Ganguly said.

Death is part of life, and there is a lot we can learn from it – BBC Focus Magazine

I grew up around dead bodies. In fact, some of my earliest childhood memories are of dead bodies in caskets, and I mean dozens of corpses not the occasional family friend or relative. The reason I saw so many dead people was because my father was a funeral director for thirty-five years in Midwest America.

Fast-forward now through some strange twists of fate, and I am currently the Director of the Centre for Death and Society at the University of Bath, the worlds only interdisciplinary research centre focused on death, dying, and the dead body. Human mortality looms so large in my upbringing and academic career that my younger sister, Julie, is on the record calling me the Overlord of Death.

As a result of these labours, I published a book called Technologies of the Human Corpse in which I cover the history and meaning we living humans assign to dead bodies by using different kinds of technologies: embalming, photography, rail transport, science museums, detention camps, radical life extension, the list goes on.

I have spent many years trying to understand what the bodies of the dead can teach us about the living, and here are some of the lessons I have learnt.

When you see a dead body, you see causation. Some set of events or actions caused that dead body to be in front of you. Dead bodies do not just happen and require either an internal or external force (sometimes both!) to appear. Place a dead body in any situation and that situation automatically becomes far more serious.

One of the great 17th Century human inventions was the autopsy (literally seeing for oneself ), which stressed peering into the dead body to understand causes of death. The autopsys historical success is also one of the reasons we 21st Century humans find it so distressing when a cause of death cannot be determined.

How is an indeterminate cause of death possible, many people ask, with all our advanced bio-medical technology? And it is on this very ship 1,000 different CSI television programmes sailed

But set aside the impossible forensics portrayed on popular television programmes for one minute, since we are living in a historical moment dominated by very real dead bodies with clearly defined causation.

Dead bodies from COVID-19. Dead bodies from police violence. Dead bodies from lack of access to necessary medical care. Dead bodies from interconnected social inequality that accelerates death, which leads me to lesson number two.

Human corpses invisibly surround we the living on a daily basis, so much so that under normal conditions approximately 1,700 people die each day across the UK.

But there are moments when disease and political protest suddenly make these dead bodies far more visible. The current visibility of dead bodies due to COVID-19 and the global protests around George Floyds death in Minneapolis are examples when human corpses become a catalyst for action.

Whether it is the over 550,000 COVID-19 deaths from across the globe or the singular dead body of one black man in Minneapolis these human corpses create new political meanings when answering some fundamental questions: why is this person dead and what political dynamics led to the death?

In many ways we have seen aspects of the current COVID-19 dead body politics before. In chapter 3 of my book I focus on HIV/AIDS corpses and the postmortem political changes produced by that pandemic.

So, for example, a key question during the height of the AIDS epidemic was whether or not it was safe to touch the body of a person killed by the HIV virus. It was safe, but it took many years for that answer to arrive.

Historical examples of dead body politics and race also abound. George Floyds death is part of a much broader US context captured in the book Without Sanctuary: Lynching Photography in America (2000) that documents how white Americans collected photographs of lynched black people and turned those images into collectible postcards. I highly recommend this book to any white person wondering why so many black communities feel such rage and anger about their dead.

I describe lesson three as part of a National Death Infrastructure into which dead bodies are absorbed by any nations very local but also quite global system for managing human corpses.

Any National Death Infrastructure includes systems such as local cemeteries and city morgues alongside international air transfer companies handling postmortem repatriations. It is when those systems overload that we begin to see the dead bodies and cannot stop seeing them since there are simply too many corpses to store. The dead bodies must be moved somewhere.

The recent COVID-19 experiences in many cities, New York and London in particular, demonstrate how pandemics can produce mass fatality events that quickly overload the everyday death infrastructure and create the need for rapid adaptation. In these moments of emergent adaptation, we begin to see how quickly the dead really do impact the world of the living.

But many of us do eventually look away and forget about the dead bodies. In the not-to-distant-future, I have a feeling that the dead bodies created by COVID-19 will be forgotten about, especially by the people who did not lose someone close to them.

Here is a quick test how many people have died from AIDS? The answer is 38 million and counting. That is an enormous number of largely invisible dead bodies.

Virological determinism is the concept I use to describe the current US and UK response to the COVID-19 pandemic, that is, we humans blame the virus for creating all the COVID-19 dead bodies as opposed to recognising human failures (and here I mean government leaders as much as anything) at mitigating the contagion.

This is similar to the way we use technological determinism to explain human problems by saying, the computer did it! as opposed to accepting responsibility for our own actions.

COVID-19 created a whole new linguistic dynamic for 2020s human catastrophes blame the virus. Name a problem and the coronavirus caused it. And while this is correct in some instances, the virological determinist rationalisation only goes so far with dead bodies.

The sudden surge in COVID-19 dead bodies that overloaded National Death Infrastructures everywhere meant hiding the bodies was not possible. Most countries face a real dilemma right now with care homes since the number of dead cannot be easily glossed over.

Governments may try (and some will surely succeed) but here is a key rule: one dead body makes any given situation a tragedy. Twenty-thousand dead bodies make the same situation a mass fatality catastrophe.

Any government that attempts to hide these dead bodies, and here hiding can also mean not acknowledging, faces an immediate problem all attempts at obfuscating the dead will only make their loved ones and advocates work even harder to name the deceased.

There is a parallel here, too, with the George Floyd case. The video recording of his death resonated so deeply because it showed his death in clear-cut terms that meant nothing was going to hide his dead body from public view.

Finally.

I opened these five-lessons with my younger sister Julie calling me the Overlord of Death. Julie died on 29 July 2018 from brain cancer and I wrote at length about her death in the preface to my book. She died in Italy (where she lived), and took her final turn while I boarded a Milan-bound plane at Bristol Airport.

When I arrived at the hospice where she died I immediately asked to see my sister and was taken to her body. I spent a long time talking with Julie about how much everyone loved her and how much everyone would miss her.

I also suddenly found myself next to a dead body, similar in so many ways to my youth, but this time it was my sister. And sitting next to her dead body taught me what loss truly felt like, since I couldnt just call my sister on the phone and tell her what was happening.

She was dead but that experience with her in the hospice meant that Julie would forevermore remain an active presence in my everyday life. And she is.

One of the perks of having trees nearby is that social-distancing rules dont apply you can hug as many as you like without risk of contracting Covid-19. Another benefit, of course, is shade. When the heats on and you need to lie low for a while, its great if some of your friends are shady characters. Especially if theyre tall, mature types with solid builds. Yeah, trees are cool.

When the thermometer spikes, any shade is welcome. If youre lucky enough to have large trees where you live, not only can you get a break from the sun, but the air temperature will be cooler as much as ten degrees compared to out in the open. Its an awesome, natural, and free kind of air conditioning.

On those blistering summer days when you think its too hot to work outside, youre not alone trees share your outlook. Photosynthesis, that amazing process which turns carbon dioxide and sunlight into sugar (thereby keeping the trees alive) and oxygen (thereby helping keep us alive), does not work well above 85 degrees. All that solar energy going to waste! Incidentally, leaves can get too hot in full sun even when the air temperature is moderate, much like the way an asphalt parking lot gets scorching in the sun.

Hopefully youre drinking plenty of water in the summer heat. It might surprise you that trees can run short of water, especially in hot, dry seasons like 2016, 2018, and this year. While we tend to think tree roots dive deep in search of a cool drink, 90% of tree roots are in the top 10 inches of soil, and 98% are in the top 18 inches.

While many shady characters dont take well to a soaking, your tree will appreciate a thorough weekly drench. Forget the lawn it can fend for itself. Please remember your trees, and water them thoroughly if it hasnt rained in more than a week.

National Weather Service announces radar outage during major upgrade in July – UpperMichigansSource.com

MARQUETTE COUNTY, Mich. (WLUC) - Beginning Monday, July 13, the KMQT WSR-88D Radar operated by NOAAs National Weather Service near Marquette, will be down for approximately two weeks for an important upgrade.

Technicians will refurbish and replace the pedestal, one of the most critical components of the radar, which is necessary for antenna rotation and positioning to capture data in all directions. The components are extremely heavy and will require the radome to be removed by crane and replaced when the work is completed.

The radar and pedestal were designed to last 25 years, and this radar has exceeded its life-span. This activity is necessary to keep the radar functioning for another 20 years or more.

The pedestal refurbishment is the third major project of the NEXRAD Service Life Extension Program, a series of upgrades that will keep our nations radars viable into the 2030s. NOAAs National Weather Service, the United States Air Force, and the Federal Aviation Administration are investing $135 million in the eight year program. The first project was the installation of the new signal processor and the second project was the refurbishment of the transmitter. The fourth project will be the refurbishment of the equipment shelters. The Service Life Extension Program will be complete in 2023.

Former Pro Bowl running back Kareem Hunt is hoping to sign a long-term extension with the Browns – CBS Sports

Kareem Hunt considers himself blessed that the Cleveland Browns have given him another opportunity after the former Pro Bowl running back waspulled over in Januarywith an open vodka container and marijuana. Hunt admitted during the traffic stop that it hurt watching the continued success of his former team, the Kansas City Chiefs, who went onto win the Super Bowl at the end of the 2019 season.

Hunt, who signed with the Browns shortly after was released by the Chiefs after a video surfaced of Hunt's domestic incident in February of 2018, expressed his desire to stay with the Browns on a long-term basis during a video meeting with reporters on Monday. In March, the Browns placed a second-round tender on Hunt, who amassed 464 all-purpose yards and three touchdowns in eight games with the Browns in 2019.

"I'd definitely like to be a part of something like this," Hunt said, via Mary Kay Cabot of Cleveland.com. "Everybody is here that I care about. I know everybody in the whole town. I would not mind playing for the Browns for a long time."

Hunt, who grew up less than 20 minutes away from Cleveland, has been working closely this offseason with fellow Browns running back Nick Chubb, who earned Pro Bowl honors last season after rushing for 1,494 yards and eight touchdowns. The duo of Chubb and Hunt in the backfield was one of the things Kyle Stefanski raved about shortly after becoming Cleveland's new head coach back in January.

"Those two guys are jumping off the tape," Stefanski told ESPN 850's Really Big Show. "I could not be more impressed by those two. Nick Chubb, man, wow the tape is so impressive. I can't wait to meet Nick Chubb the person because everyone is raving about him (Hunt) is such a talented player. He's from Ohio, so I know I'm going to get a chance to visit with him at some point."

Hunt, who rushed for a league-high 1,327 yards as a rookie back in 2017, is hoping to be part of a championship team in Cleveland after watching his former teammates in Kansas City hoist the Lombardi Trophy back in February. Hunt added that he has mostly maintained a low profile following January's traffic spot. He said he has also been doing things in the community as it relates to helping people who have suffered significant loss during the COVID-19 pandemic.

"I think we can do something special here," Hunt said of the Browns, who stumbled to a 6-10 last season after many analysts predicted them to compete for the AFC North division title. "I want to get that Super Bowl feeling and I believe we can do it here in my hometown. That would be bigger than anything, for me. That would [be to] bring a championship to Cleveland, especially I have been a fan my whole life. I've been with Cleveland my whole life."

The world continues to be a turbulent and stressful place. With ongoing challenges from COVID-19 and racial division, we may feel our daily lives are out of control. How can we manage the physical, mental and emotional impacts of all of these challenges for ourselves, our families and our communities? One opportunity we all have to face these stressful times is to build our resiliency.

Resiliency is the ability to bounce back from adversity, trauma and threats that we encounter throughout our lives. However, it is not only the ability to survive from these experiencesbut also to thrive. Researchers from Harvard University use the analogy of a balance scale or seesaw. When we are overwhelmed from stress and adversity, the scale will drop to the negative side. However, by increasing our positive experiences and coping skills, we can bring the scale (and our lives) back into balance, and possibly even tilt the scale up to thriving.

What is helpful to know about resiliency is that it can be increased at any age. All of us can learn how to build our coping skills to help strengthen our thoughts, behaviors and overall quality of life. Throughout this summer, UW-Madison Division of Extension will be offering tips and activities on how to strengthen different aspects of resiliency for ourselves and others around us to help build our own capacity for stronger communities. The line between ‘elective’ and ‘essential’ is often hazy – Bryan-College Station Eagle

Late February was the last time I saw Mr. Fields, a man in his sixties with debilitating nausea, bloating and regurgitation. (To protect his privacy, I have changed his name and other identifying details.) His symptoms made it difficult for him to eat, and he was losing weight visibly from one clinic visit to the next. His last upper endoscopy - a procedure to inspect the top of his gastrointestinal tract - was aborted because there was a pile of food sitting in his stomach, despite his having fasted since midnight the day before. We had discussed repeating the endoscopy after a prolonged liquid diet to determine what was holding up his digestion (a tumor, an ulcer, or a general sluggishness of the muscle) and to use this information to guide his treatment.

Then the pandemic hit, and priorities changed. On a Friday afternoon in mid-March, I was asked to cancel the bulk of the procedures I had scheduled for the following Monday. From higher up, we received instructions to review every patient's chart, to separate the emergent from the urgent from routine. Mr. Fields certainly wasn't the only one perched on a borderline - it's the nature of my gastroenterology practice, as it is for most diagnostic proceduralists, not to know what I'll find until I go looking. My colleagues and I turned to each other for guidance. How worried would you be about an 80-year-old with iron-deficiency anemia? How about a 40-year-old with rectal bleeding? A 20-year-old unable to swallow? There was no question that they merited procedures, but when? Could they stand to wait four weeks? Eight? Twelve?

What does "elective" mean, really? It's intuitive to think about illness as varying along a spectrum of urgency: On one end, a hangnail; on the other, a heart attack. The term also implies that treatment can also vary along a spectrum of need, from cosmetic surgery to cardiopulmonary resuscitation. Most of medicine sits in the middle, asking us to balance potential health benefits against potential costs, such as missed diagnoses, procedural complications or, more recently, a covid-19 infection after visiting a medical building.

While the country debates whether to prioritize reopening the economy or minimizing preventable deaths, we face our own complicated trade-offs within the practice of medicine. The pandemic logic of triage - a process, borrowed from wartime, of prioritizing sick individuals according to the severity of their prognosis - exerts novel pressure on a system not used to such resource scarcity. Individual clinicians are grappling with what is truly necessary and urgent.

During my first week back to work in the endoscopy suite last month, I had trouble convincing any of my originally scheduled patients to come in for their procedures.Their episodic abdominal pain, refractory heartburn, and unrelenting diarrhea had seemed pressing just a few months before, but now the threat of infection loomed larger. Alongside clinicians, patients make their own judgments about what constitutes essential health care - about how to value the reassurance of resolving old symptoms against the risk of getting sick with something new and even more uncertain.

The calculus quickly gets complicated. We have to weigh not only the patient's health but that of the physicians, nurses, technicians and custodians who keep a procedural space operational. Concerns about infection also ripple from patients and providers to all their family members who might be affected. The masks and gowns protecting one cohort of health care workers could always be set aside for a needier cohort later. And staff relieved of their nonessential duties in a procedural suite could theoretically be reassigned to other settings where they might be of greater service.

On the other side of the equation, there are certainly diseases that become more dangerous when left unattended. A few of my colleagues in other cities worry vocally about how many gastrointestinal malignancies they'll find once routine practice picks up again. Less vocally, they worry about the legal implications of not having made the diagnosis earlier, wondering if they'll be liable for not having pushed more procedures into hospital settings even as federal or professional society guidance and state executive orders limited elective procedures. The potential hazards of clinical lag time also hold true for other specialty areas - blurry vision can herald impending strokes, and unattended orthopedic injuries can settle into contractures. In the background is the unsettling awareness that interventions deemed essential in one state may be elective in the next, particularly for politically sensitive services like abortion.

The line dividing essential from elective care is always a subjective one, because risk-benefit calculations tend to shift over time. The past few months have occasioned especially dramatic fluidity, but larger-scale changes have also unfolded over the past several decades. In the 1960s, specialties began lobbying for new interventional environments like the ambulatory surgical center, while emergency rooms arose as distinct hospital spaces for managing acute concerns. Meanwhile, chronic disease prevalence and pharmaceutical incentives inspired increasing professional interest in the idea of risk states (like high cholesterol as a precondition for heart disease, or age and gender as preconditions for breast cancer), which in turn entrenched preventive maneuvers (from annual physicals to mammograms to a daily aspirin) as vital elements of routine care. The insurance industry overlaid these developments with thorny questions about medical necessity.

In a sense, it's been remarkable how successfully patients have avoided clinical settings in these first few months of the pandemic. Early anecdotal reporting noted surprising drops in hospital admissions for common medical emergencies like cardiovascular events, appendicitis and strangulated hernias, and international data have begun to confirm these trends. In the United States, emergency catheterization procedures for life-threatening heart attacks were down 38 percent in the early phase of covid-19, despite predictions that those events would be, if anything, more prevalent during a viral pandemic. Fewer cirrhosis patients are being admitted to Veterans Health Administration hospitals, according to a study recently published by my colleagues at the University of Pennsylvania, and those who are admitted have been, on average, measurably sicker than baseline.

Contemporary biomedicine certainly prioritizes procedures financially, but also emotionally and culturally. Objective observations gathered by diagnostic maneuvers like CT scans, tissue biopsies and cardiac catheterizations have become a linchpin of clinical certainty. They are vital to a confident diagnosis and treatment plan and, when edged with the promise of life extension, serve as a kind of salve for the threat of mortality. These assumptions are so foundational to modern medicine that it can be shocking to question their validity.

Well before covid-19, the writer Barbara Ehrenreich detailed her decision to turn away from screening tests like colonoscopies, contending that they delude us into approaching death as a problem to be solved while at the same time invading the body to the point of violation: "if mammography seems like a refined sort of sadism," she notes, "colonoscopies mimic an actual sexual assault." It's a provocative argument, easier to accept in the abstract than when I recall patients with cancers I've found on years-overdue colonoscopies. Our infrastructure of high-tech diagnostic intervention can seem particularly bloated in the context of a pandemic, but patients and clinicians alike are often deeply and justifiably invested in it.

Ultimately, clean distinctions between essential vs. elective care are rhetorically convenient but clinically reductive. Sacrifices are entailed on both sides, and the messy work of negotiating the pros and cons will persist as the pandemic recedes. The same might be said for all-or-nothing debates pitting human life against a solvent economy - a compelling thought exercise, but less relevant in practical terms. Accumulating job losses are leaving more and more people uninsured, for example, forcing them to continue deferring elective medical care once it picks up again. Progressive dreams of radical reform notwithstanding, the medical and financial priorities of our healthcare system are deeply enmeshed, and for now, grappling with either means grappling with both.

Every day provides new data, and with it another opportunity to reweigh our priorities. In Philadelphia, where I work, our projected peak was at the end of April, and the first surge of covid-19 cases appears to have crested. Stocks of personal protective equipment are no longer dangerously depleted, and opportunities exist for systematic testing. These developments will afford us more space to evaluate those uninfected patients with separate, slow-burning complaints.

I recently re-connected with Mr. Fields; I hope he doesn't hold this interruption of his care against me. I'm ready to pick up where we left off, but covid-19 has made it harder to pretend that medicine operates in a vacuum. I marvel at the confidence of our pre-pandemic therapeutic encounters, which so often left implicit all the indelicate questions of financial incentives, workers' well-being, and the capacity of medicine to harm as well as to heal. Once the crisis lifts, it's hard to imagine returning to such thoughtless certainty.
