UP gets largest Covid facility with 400-bed capacity in Noida – The Indian Express

Uttar Pradesh Chief Minister Yogi Adityanath Saturday inaugurated a 250-bed Covid-19 facility operating from the premises of Noida Sector 39s District Hospital Building. The facility, which will soon be expanded to 400 beds, is the states largest Covid facility and has been developed by Tata Trusts and Melinda Gates Foundation, in collaboration with the state government.

The state government has been making efforts to ensure timely treatment to Covid patients. The CM inaugurated the Covid facility which, in the days to come, will have a capacity of 400 beds. The hospital has been built at a cost Rs 344 crore, in coordination with Tata Trusts and Melinda Gates Foundation. At present, the hospital has 250 beds along with 10 ventilators, said Noida administration in a statement.

The inauguration was attended by Gautam Budh Nagar MP Mahesh Sharma, Noida MLA Pankaj Singh and other senior administration officials. The CM reached Noida district on Friday evening and a meeting was held with the DM, CMO and other administration and medical officials to take stock of Covid situation.

Known as the Noida Covid Hospital, the facility spreads across eight floors and is equipped to provide both L1 and L2 facilities with dedicated wards each. The state government had earlier emphasised on ramping up L1 facilities across districts for patients with mild symptoms.

According to a Tata Trusts spokesperson, the hospital has its own diagnostics and pathology lab, which are equipped for processes such as haematology, biochemistry and serology.

Presently, the ICU has a functional capacity of 20 beds, along with eight beds in the emergency rooms equipped with ventilators and central monitoring stations. A patient occupying the bed will be provided with a medication drawer, an overbed table, an IV stand, and a bedside stool, medical oxygen pipeline system for a continuous supply of oxygen.

Till Friday evening, Noida reported a total of 5,868 cases with 4,888 recoveries and 43 deaths.

But what is safe to do when socializing and how large should those gatherings be? Jason H. Yang, assistant professor and chancellor scholar in the Department of Microbiology, Biochemistry and Molecular Genetics at Rutgers New Jersey Medical School and co-chair of the COVID Research Alliance, a part of the Center for COVID-19 Response and Pandemic Preparedness, offers some advice, along with some caution, for meeting up safely with friends.

What are some suggestions for allowing families to get together safely?

Although COVID infections have been steadily declining in New Jersey and Governor Murphy has moved the state into Phase II of reopening, community transmission continues to be a problem, and COVID is staging a resurgence in many other states. It might be safe for some families to gather and socialize if all individuals were to be vigilant in wearing face masks, washing hands and maintaining physical distance. However, this may be difficult to enforce for families with young children, and having children playing together closely introduces new risks. Everyone should continue to use their best judgment to protect themselves and each other from infection.

What is safe to do while socializing this summer? Are backyard playdates OK? Should you refrain from going into other people's houses to use their bathroom or for activities?

Any activity in which individuals are in close physical proximity, touching similar objects, and/or bringing objects close to their faces introduces risk of transmission and should be minimized. Outdoor socializing may be safer than indoor socializing, and all socializing is safer with face masks and physical distancing. Individuals should wash their hands frequently if possible.

What should families take into consideration if they decide to expand their social bubbles with select friends? Do you think its safe to reestablish close contact with a small group of people? If people do, should they develop agreed-upon terms with theirfriends, and what should they be?

Engaging in close physical contact with other people will always increase the risk of infection, but it may be possible to manage risk through conscientious and vigilant behavior. If individuals decide to enter into a bubble with some friends, they should discuss terms that they all agree to protect the group from infection. In general, individuals should continue to try to keep these types of groups and social interactions small and limited until community transmission stops, or until there is an effective vaccine.

What about hosting a party for summer birthdays? Should drive-by celebrations that have become a hallmark of the pandemic continue, or would it be safe to have an outdoor gathering to celebrate in the summer?

Everyone should exercise good judgment and make their best decisions to minimize risk. It would in general be best to avoid dense and large social gatheringssuch as birthday parties.Outdoor gatherings may in general be safer than indoor gatherings, but any large social event still poses a large risk for transmission.

What precautions should you take when you come together with another family? If you are exclusively spending time with one or two other families, is it necessary to wear masks? What is your recommendation when children refuse to wear a mask in a social setting?

Socializing with other people introduces risk because asymptomatic individuals are capable of transmitting COVID-19. When coming together with another family, everyone should make their best effort towardprotecting themselves and each other with face masks, hand washing and physical social distancing. If children or other family members refuse to wear a mask, it would be best to postpone the social event to another time when community transmission has stopped. Care should be taken so that asymptomatic children do not unknowingly transmit to older family members.

Is it safe to bring a babysitter into your home or are there precautions you should take? Does having a babysitter preclude socializing with other families?

If a babysitter has recently tested negative for COVID-19 and does not exhibit any symptoms of infection, in principle it should be safe to bring them into your home. In general, babysitters and all other guests should follow the same guidelines that Rutgers adheres to from the State of New Jersey and the Centers for Disease Control to wear face masks, wash hands and maintain physical distancing.

Purpose: Hypertension is a growing public health problem with a remarkable contribution to morbidity and mortality. It is a common condition which usually coexists with diabetes and aggravates its complications. The objective of this study was to assess the prevalence and determinants of hypertension among diabetic patients attending their follow-up at Jimma University Medical Center (JUMC) from June 1 to August 30, 2019.Methods: A hospital-based cross-sectional study was conducted in diabetic patients attending their follow-up at JUMC. Systematic random sampling and a pretested interviewer-administered structured questionnaire were used to collect information. Data were entered into EPI data 3.1 and exported to SPSS version 20. A variable having a p-value of < 0.25 in the bivariate model was subjected to multivariate analysis to avoid the confounding variables effect. Adjusted odds ratios were calculated at the 95% confidence interval and considered significant with a p-value of < 0.05.Results: A total of 366 diabetic patients were included in the study. Their mean age was 50.1 14.28 years, and the mean duration of diabetes was 6.8 5.3 years. The study finding showed that the prevalence of hypertension among diabetic patients was 37.4%. According to the multivariate logistic regression analysis, age of 50 years [AOR = 4.79; 95% CI: 1.4, 16.4], having body mass index (BMI) of 25 [AOR = 3.11; 95% CI: 1.58, 6.12] and khat chewing [AOR =19.34; 95% CI: 10.26, 36.44] were independent predictors of hypertension among diabetic patients.Conclusion: Our study found that there is high prevalence of hypertension among diabetic patients. Age of 50 years, having BMI of 25 kg/m2 and khat chewing were associated with hypertension among participants. Early detection and appropriate interventions should be an important action among patients with age 50 years, having BMI 25kg/m2 and khat chewers.

UHS approves proposals of reforms body – The News International

LAHORE:The 156th meeting of Advanced Studies and Research Board of the University of Health Sciences (UHS) was held here on Thursday with UHS Vice Chancellor Prof Javed Akram in the chair.

The board approved the recommendations of MD/MS/MDS Reforms Committee with regards to intermediate and final examination. It also gave a nod to the revised curricula of various specialties proposal by concerned Specialty Advisory Committees constituted by the varsity for that purpose. It was told to the members that the committee had thoroughly updated the course contents, rotational plan and table of specification for intermediate and final exams.

He remarked that the activation of NTC and enforcement of Act will enhance opportunities for the technologists.

The minister directed for formulating recommendations with regard to NTC Act and stressed that the future of Pakistan is linked with technical education. The minister underscored that the Punjab government has adopted a revolutionary strategy to promote technical education in the province. He said Tevta was playing a pivotal role in the promotion of technical education. He emphasised that the targets of progress can be speedily attained by promoting latest technology.

Study by U of T researchers reveals how bacterial toxins evolve to cause new illnesses – News@UofT

The coronavirus pandemic is a daily reminder of the far-reachingconsequences of apathogens successful invasion of human cells. And, as a new University of Toronto study on bacterial toxins shows, it does not take much for these encounters to turn deadly.

The research found that two almost identical bacterial toxins cause distinct illnesses diarrhea and fatal toxic shock syndrome by binding unrelated human receptors. It also highlights a mechanism by which pathogens have evolved distinct receptor preferences to infect different organs.

I always think of bacterial toxins as fascinating machines of death in how they find new ways to enter host tissue, saysMikko Taipale, an assistant professor of molecular genetics at the Donnelly Centre for Cellular and Biomolecular Research.

Taipaleco-led the study,published recently in the journalCell, withRoman MelnykandJean-Philippe Julien, both senior scientists at the Hospital for Sick Children and faculty members in U of Ts department of biochemistry.

Many are familiar withClostridium difficile, a gut-dwelling bacterium that can cause diarrhea. Lesser known is its close relative,Paeniclostridium sordellii, which also lives in the gut and in the female reproductive tract. Infections are rare but fatal and can occur when the bacterial toxin escapes into the bloodstream, during birth for example, and spreads into the lungs and other organs.

Both species are thought to be part of the microbiome, the bodys resident bacteria, but its not clear why they harm some people and not others.

The toxin released byC. difficileacts through Frizzled receptor proteins, which play a role in tissue regeneration. Yet, whileP. sordelliiproduces a similar toxin, it does not bind in the same way so the researchers set out to investigate.

They took an unbiased approach by systematically switching off every gene in human cells and exposing them to theP. sordelliitoxin. Cells that survived turned out to lack genes encoding cell surface proteins called semaphorins, and other experiments confirmed that two members of this class, Semaphorin6A and Semaphorin6B, are indeed the receptors for the toxin. Both receptors are present in the lungs, as expected, though their role there remains unclear.

Knowing the receptor opens the door to finding treatment. The researchers were able to halt infection in mice by co-injecting the toxin with purified semaphorin fragments, which bound and neutralized the toxin before it could reach the real receptors.

But the finding led to more surprises.

Like Frizzled, semaphorins play important roles in the body most notably in developing the nervous system, where they help guide projecting nerve fibers. Even more surprising was that it binds a receptor with no structural resemblance to Frizzled.

Here we have two toxins that are so similar to each other, but they use completely different receptors, says Taipale. We did not expect to find that.

The reason is a tiny differencebetween C. difficileandP. sordellii in particular, the surface though which both toxins contact their receptors, as revealed by cryo electron microscopy. Each toxin protein is composed of about 2,500 amino-acids and the researchers were able to pinpoint those that directly engage with the receptor. Swapping a mere 15 of these amino acids between the two toxins was sufficient to switch receptor preference. In other words, they created a P. sordellii toxin that targeted Frizzled and vice versa.

We were floored when we saw that the toxins shared a surface that each evolved to uniquely interact with distinct cells, says Julien.

It appears that, while the rest of the toxin is under strong evolutionary pressure to remain unchanged, the receptor-binding surface is free from such constraints. This allows toxins to evolve into variants that bind new receptors to invade other tissues and hosts.

Receptor switching is not unique to bacteria, however. SARS-CoV-2 and coronavirus strains that cause common cold use the same part of the now famous spike protein to bind to diverse receptors, which might explain differences in disease severity.

This is a nice example of how viruses and bacteria from completely different domains of life have found similar molecular tactics to change their receptor targets in human cells, says Taipale.

It also reminds us how much cool biology one can find in the microbial world.

Infectious diseases block inaugurated at Srinagars SKIMS – Hindustan Times

Sher-i-Kashmir Institute of Medical Sciences (SKIMS) Soura on Monday further strengthened advanced patient care services with the inauguration of Infectious diseases block (IDB).

The inauguration was done by advisor to L-G of Jammu & Kashmir, Baseer Ahmad Khan.

It is the first centre of its kind in north India which is equipped with superlative quality facilities. The services that will be provided include 40-bed isolation ward, l5-bed high dependency isolation ward and intensive care unit (ICU).

The centre will also have four-bedded RO and dialysis unit, independent portable x-ray, USG machine and ECG, lab facilities for haemogram coagulation studies arterial blood gas and electrolyte analysis, biochemistry etc.

SKlMS director Dr AG Ahangar dedicated the IDB to the patient care, especially the engineering department, and other supportive management services for making this possible in short span of time against all odds.

Ahangar expressed satisfaction about the safety measures taken at all levels to ensure protection of frontline health workers.

AbstractINTRODUCTION: Vitamin B12 deficiency is widely prevalent across many low- and middle-income countries, especially where the diet is low in animal sources. While many observational studies show associations between B12 deficiency in pregnancy and infant cognitive function (including memory, language and motor skills), evidence from clinical trials is sparse and inconclusive.METHODS AND ANALYSIS: This double-blind, multicentre, randomised controlled trial will enrol 720 vegetarian pregnant women in their first trimester from antenatal clinics at two hospitals (one in India and one in Nepal). Eligible mothers who give written consent will be randomised to receive either 250 mcg methylcobalamin or 50 mcg (quasi control), from enrolment to 6 months post-partum, given as an oral daily capsule. All mothers and their infants will continue to receive standard clinical care. The primary trial outcome is the offsprings neurodevelopment status at 9 months of age, assessed using the Development Assessment Scale of Indian Infants. Secondary outcomes include the infants biochemical B12 status at age 9 months and maternal biochemical B12 status in the first and third trimesters. Maternal biochemical B12 status will also be assessed in the first trimester. Modification of association by a priori identified factors will also be explored.ETHICAL CONSIDERATIONS AND DISSEMINATION: The study protocol has been approved by ethical committees at each study site (India and Nepal) and at University College London, UK. The study results will be disseminated to healthcare professionals and academics globally via conferences, presentations and publications. New COVID-19 test kit being rapidly developed using algae – CTV News London

LONDON, ONT. -- The number of COVID-19 cases across the country could be higher than we know, according to a group of scientists at Western University.

"We are underestimating the number of people in the population who have been exposed to the virus," says Dr. Dave Edgell in the Department of Biochemistry at Western's Schulich School of Medicine and Dentistry.

"There are some estimates that there may be 10 to 20 times more people infected that are actually being reported as confirmed cases of COVID."

Edgell says thats why his team, in collaboration with Suncor, are working at rapidly developing COVID-19 test kits that would determine if someone has been previously infected and has antibodies for the virus.

"The test will tell you if you have antibodies that your immune system has created to combat the virus."

Dr. Greg Gloor, also from Schulich's Department of Biochemistry says these kits could help researchers investigate herd immunity.

"In order to contain this outbreak we need to know who is currently infected and who has been infected. People who have been infected and have developed a high enough immunity probably have less to fear than people who have not been infected."

The research team is developing these kits by using algae. A new concept that they say would allow tests to be made more cost-effective with the goals of it reaching more people.

"This is where algae comes in, because it can be grown in sea water and sunlight and it doesnt cost anything really compared to the way people make the viral antigens now," says Edgell.

The hopes are to roll out these tests kits by late fall, once production and testing of efficacy is complete.

Pubs need to be on the same reopening timeline as restaurants and cafes, Cork City chairperson of Vintners Federation of Ireland Michael ODonovan has said.

Mr ODonovan said hospitality establishments share customers and it would be an unfair advantage to have certain sectors of the industry opening over others.

We are hoping all hospitality outlets will be on an equal footing, Mr ODonovan said.

If people have choice, it is good for the consumer. All we are asking for is a level playing field.

Mr ODonovan said many of the pubs that offer food service are possibly better able to provide enough space to socially distance.

There are a lot of pubs doing food that are bigger than restaurants and better able to physically distance.

He said any issues in relation to a lapse of social distancing was something that would be experienced across the sector regardless of the outlet. Most restaurants serve wine, spirits and craft beers, they are in direct competition with food bars, the same issues apply.

Mr ODonovan said the most important thing they would like to see come out of todays announcement is clear guidelines on what they need to do in order to reopen.

The Vintners yesterday revealed that 60% of Dublin pub owners say they will go out of business if they have to stay closed until next year.

The cocooning elderly are crumbling under the pressure of confinement and loneliness due to the current restrictions, according to older persons advocate Paddy OBrien.

Mr OBrien said he would like to see some concessions given to the elderly going forward, such as allowing them to go for a spin in their cars and walk within an extended radius of 5km or 6km.

He is very concerned about the elderly getting depressed.

People are collapsing under the pressure, it is becoming too much for them.

The loneliness is becoming depression.

It is very serious, a lot of sad, depressed people are definitely affected by this. I am concerned about people living with no contact.

Contact is vital and I am very concerned about the effect it is having on people.

They are confined and must adhere to the guidelines, as we all have a role to play, but if there were some changes made it might be easier to manage.

Mr OBrien said a month ago, people were saying it would be grand, now the gardening is done, the house is cleaned, the presses are organised, there is nothing left to do. People know they are negative, they have no zest, their heart is gone out of them.

A large number of people over 70 are driving and they are telling me they would give anything to be able to go for a spin.

Ireland must double its Covid-19 testing capacity and ensure reliable results are available within 24 hours to safely reopen society, an expert in biochemistry at University College Cork (UCC) has said.

Tom Cotter has been professor of biochemistry at UCC since 1995.

He says for society to safely reopen, there must be a dramatic increase in testing capacity for Covid-19 and an effective antibody test to determine whether people had the virus and have now recovered.

When society is opened up once again, we have to be able to test people very, very quickly, he said.

There are two things the Government needs to do before they start opening up everything again theyll need a very good antibody test and theyll need a PCR [polymerise chain reaction] test for the virus, and they will need to be available within a 24 hour period.

When lockdown is released and people are out and about, if anyone gets sick they need to be tested along with everybody theyve come into contact with, within 24 hours, he added.

PCR testing and antibody testing are the main methods of testing being used across the globe in the fight against Covid-19.

The antibody test will tell you whether youve had the coronavirus before or not, and are therefore probably immune or not, said Prof Cotter.

The PCR test will tell you if you have the virus and are infected. These two tests need to be streamlined with a turnaround time of 24 hours so that if there is another outbreak of the virus, we can catch it straight away, he added.

Some of the antibody tests that are available now are good while others are bad we need to make sure we have a really good one to tell if people have been infected and have recovered.

This test tells you if you have antibodies from the virus which means you had it and are now OK.

Then youre immune to it but for how long, we actually dont know at the moment.

Prof Cotter said that the Government is likely to release some parts of industry in Ireland on a step-by-step basis.

Theyll loosen low-risk areas like construction and hardware stores, and then theyll watch to see what happens, he said.

If the infection rates dont go up, theyll release another sector or group of jobs, and theyll watch again then theyll keep going like that.

Its a watching and waiting game because we cant afford to have another outbreak of this, he added.

CLARITY and financial support are what the hotels of Cork want to hear from Taoiseach Leo Varadkar today as the roadmap to a new normal is announced.

Chairperson of the Cork branch of the Irish Hotels Federation, Fergal Harte, said for the duration of the pandemic, the landscape of the hospitality sector will be significantly altered.

Social and physical distancing in its current guise is extremely difficult, there is no question about it.

All of the things that we want to promote in our hotels, in terms of a nice atmosphere, enjoyable ambience, and a social element are all becoming difficult to create if people are confined and cant mix, and even from the perspective of a restaurant or a bar, you have to restrict the numbers and space people out.

All the things we associate with hotels will be changed for this period anyway.

Mr Harte said there were a number of challenges to face, but that the Irish Hotels Federation was looking at it with an open mind.

We are making plans and looking to the future, hotels are anxious to reopen, but we need clarity and support, financial supports to make the industry viable.

In terms of financial assistance, the Mr Harte said the organisation was calling on the Government to waive local authority rates and water charges for 12 months as well as a reduced Vat rate of 0% for 12 months, business interruption grants, and work to provide support from the banks.

The managing director of Blackwater Motors, Denis Murphy, says he is frustrated and angry by the ongoing lockdown.

Mr Murphy said he did not understand why they could not go back to work next Tuesday, May 5.

As it goes on, it is becoming more difficult, people are becoming more and more disillusioned, they are not afraid of the virus anymore, they are afraid for their livelihoods, said Mr Murphy.

He said his staff are worried about what is going to happen in the industry if they do not get up and running soon.

Eighty-five per cent of businesses are closed or substantially reduced. That is colossal, he said.

The guys making the decisions dont own businesses, they are still getting paid, they still have their pensions.

Mr Murphy said he would like to see more transparency in the decision-making process.

He said that the reason for the lockdown was to prevent a surge and if hospitals are managing at the moment, surely then businesses should be allowed to recommence in line with strict social distancing and other guidelines.

I cant see why we are not opening next Tuesday, he said. They must think we are totally irresponsible and unable to follow these social distancing guidelines.

CORK developer Michael OFlynn believes now is the right time for construction work across the country to restart.

He said that if social housing has been deemed safe to be built, private housing can be dealt with similarly.

His company was in the process of delivering around 200 houses in Cork and Dublin at the time construction was deemed non-essential, and he had to lay off around 500 people who were either directly employed or subcontracting at a number of sites around the country.

However, with an updated announcement expected today on restrictions, Mr OFlynn said he sees no reason why construction couldnt restart with adequate procedures in place.

We have all embraced the restrictions for five weeks now. I absolutely understand that the health issue has to come first, but I think part of our industry can restart in a safe way, he said.

Mr OFlynn outlined a number of steps the industry has taken, including the development of new operating procedures and manuals being circulated, in preparation for a safe return to work.

Im very hopeful, especially given that social housing sites have been back for the last week now.

Private housing is the same method of construction as social housing, so if someone could form a conclusion that social housing is safe, then private housing must be safe, but only on the basis that theres total adherence to the HSE guidelines, Mr OFlynn told.

I think housing is a special case. In a lot of countries housing never closed. I appreciate the decision here was to do that, and for all the right reasons, he said.

Having said that, everything has to be considered, and I think now is the time to consider easing back, but only if it can be done safely.
