Bioelectric Medicine Ready for Prime Time as Companies Advance Therapies – BioSpace

Bioelectric medicine is coming into its own as a combination or stand-alone therapy for a growing cohort of conditions. Big pharma has funded programs, international coalitions have formed and the National Institutes of Health (NIH) and the Defense Acquisition Research Agency (DARPA) each are making sizeable research investments. The field, panelists at a recent Demy-Colton Virtual Salon agreed, holds huge promise.

Targeted delivery of energy biomedicine can be used to treat chronic diseases of the nervous system, inflammatory conditions, autonomic disorders and metabolic diseases. It may even be possible to treat heart conditions, oncology, pain, and gastric, urologic, and ophthalmic conditions such as chronic dry eye.

Key benefits of this approach include the lack of side effects and possibly expedited regulatory pathways. As panel moderator Beth Rogozinski, CEO, Oncoustics, noted, Were seeing payers with expedited pathways and payers paying for telemed and telepsych at the same rates as in-office visits.

For biomedicine, this is the age of innovation.

At Massachusetts General Hospital, Paolo Cassano, M.D., Ph.D., co-founder Niraxx, and his team use photons of light to modulate brain function.

This can be applied to any organ of the body. We absorb light, make energy from it, reenergize cells, and thus reestablish cellular function, Cassano said. The most obvious target for light is skin, but other applications include muscles, fatigue, and general health, so sports medicine is another big application.

Some of the treatments have effects beyond their neuromodulatory effect, for systemic and local effects, Cassano said.

He specifically noted neuro-oxidative, anti-apoptotic and anti-inflammatory effects. This suggests that bioelectric therapies may control the excess of reactivity of the body and contain the most lethal consequences of an infection and its processes.

Cala Health, in another example, is developing a wearable, watch-like device to address essential tremor in Parkinsons and other neurodegenerative disease.

This is found in the hand, voice or legs of patients and affects their ability for self-care, Renee Ryan, CEO, Cala Health, explained.

The therapeutic options today are surgery or off-label drugs.

We believe a lot of the disease is influenced by the median nerve, so we have a watch-like device that provides stimulation to the radial and median nerves at the wrist. A 40-minute treatment resulted in an average of more than 94 minutes of tremor relief, she added. We usually see the onset of relief after about 10 minutes, and one patient achieved seven hours of relief after an early morning session.

By relieving tremors, she said she expects to also reduce patients depression and anxiety.

Improving cognition is another area of great need. This includes improving patients memory and retention as well as executive function.

With an aging population, we need to keep workforce performance up. Theres also the issue of preventing dementia and mild cognitive impairment, Cassano said.

For COVID-19, biomedicine offers the ability to modulate inflammation, and thus may help control or prevent the cytokine storm and caustic inflammation. Developmental disorders, such as Downs syndrome can be addressed, too.

If we can intervene early, some of these patients may develop their cognitive function to enjoy a fuller, productive, more joyful life.

This is a very exciting time for this industry, Casanno said. Patients are desperate for new solutions, for diseases that have no treatments or no disease-modifying treatments.

Bioelectric medicine differs from biofeedback, panelists pointed out. Neurofeedback is about sensing whats happening in the brain so people can modify their behavior. Bioelectric therapy, in contrast, temporarily modifies the brain function.

That safety profile lends itself to a potentially faster commercialization pathway.

If there is good scientific data and regulatory support, and the therapy isnt invasive, theres an advantage, said Juan-Pablo Mas, partner, Action Potential Venture Capital.

Bioelectronic therapies will have various places in the treatment paradigm, such as being used in combination therapies, replacing drugs, filling therapeutic gaps, proving unique mechanisms of action and catching patients earlier in the disease when surgery isnt an option, Mas said. All will have effects in terms of how disease is treated.

They have a place in wellness also, where they are not regulated.

Thats a big opportunity, Cassano said. But, Once you know a person actually is suffering from a disorder, the disease itself requires monitoring, he cautioned.

Medicine is shifting towards prevention. That should be our goal, Cassano said. So capitalizing on the wellness side is critical, preventing wear and tear of brain that lead to onset of diseases.

As in so many things today, The new normal wont be the old normal, Rogozinski concluded.

For I+I’s Selam Kelati, Natural Remedies Are an Ages-Old Family Tradition – CBD Today

After a 20-year career in the corporate world, Selam Kelati was ready for a change. Born and raised in Ethiopia, she grew up in a culture where plants and herbs were part of everyday life and utilized in an array of food, medicines, and cosmetics. So, when she decided to build a business of her own, she constructed the frame around her knowledge of natural remedies and homeopathic plants.

Launching a startup in the competitive U.S. cosmetics-and-skincare sector was no small endeavor. The industry is dominated by powerful multinational brands. But with estimated revenues of $645 million in 2019, the CBD segment of the market still offers plenty of room for startups. Kelati, along with business partner Jennifer Culpepper, seized the opportunity and created I+I Botanicals. (The name is pronounced I and I.)

The company takes an eco-conscious, holistic approach that upholds the traditions and ethics Kelati learned at an early age. Our core message is good for the earth, good for the soul and, of course, good for the body, she said. I and I comes from the Rastafarians, who use the words to mean you and I are the same. The message is that the love you give to yourself, try to give it to another.

Kelati and her eight siblings were raised by Eritrean parents and an extended family that passed down recipes for plant-based foods and medicine. Her grandmother was known in her village as an herbal medicine expert who had a deep knowledge of plants in the region and used them to create concoctions for health, beauty, and wellness.

People would come from near and far to get some kind of something that she would mix for medical purposes, so this was in our house and our family, Kelati explained. Anything to do with herbs or medicine or cooking was just a natural thing in our household. You sit with your grandmother and your aunties and your mom and you learn, but you dont even understand you are learning at that age, because its part of your life, right? So thats where my passion for natural ingredients comes from, and its something Ive carried with me throughout my life.

The prickly pear cactus grows abundantly in Eritrea and is utilized in a number of different ways, from making jams to feeding livestock. Pressing the seeds creates an oil that is beneficial for skin and hair, Kelati said, and it also has anti-aging and anti-inflammatory properties. Another ingredient widely used in her native region is moringa oil. The substance is created by processing the seeds and leaves of the moringa tree, which has been dubbed the miracle tree due to its bountiful medicinal and nutritional attributes.

Moringa oil mimics the natural oil we create in our body, so it does help with your hair shine and keeping it healthy and not breaking, Kelati said. And for the skin as well. Its nutrient-dense, which helps reduce the appearance of wrinkles.

There are a lot of people who dont understand that its not just about working hard. Its also about having a starting point.

Kelati has been experimenting with her own natural plant remedies for most of her life and enjoys sharing her creations with friends and family to gather feedback about what works and what does not. Soon after she heard about CBDs anti-inflammatory, antioxidant, and skin-soothing properties, she immersed herself in research. She went to trade shows and networked with operators in the industry. During her investigation, one product, in particular, stood out to her. It was an under-eye cream that [my business partner] Jennifer shared with me, and I remember seeing the puffiness under my eye being reduced within three or four days of using it, she said. That made me a believer.

I told Jennifer I wanted to have my own beauty line based on natural ingredients, because this is what I know and have been taught since childhood, she continued. Theres some kind of magic we can create using CBD. So, Jennifer and I brainstormed and I+I was born in the process.

As working mothers, Kelati and Culpepper understand the unique pressures women face on a daily basis, juggling roles from caregiver to breadwinner and trying to squeeze in time for themselves when and where they can. As such, one of their goals is to give women an easy way to incorporate CBD products into their daily routines and enjoy a relaxing moment or two amid the hustle and bustle.

The global CBD skincare market is expected to reach $1.7 billion by 2025, according to a report by Grand View Research Inc. North America, which accounted for more than 40 percent of the market in 2018, will lead the way, Grand View projected. For brands like I+I, which has a relatively small footprint at this point, establishing a presence in the lucrative space early is crucial. The effort starts with creating unique, compelling products that will stand out from the crowd.

The process Kelati uses to formulate new products is a combination of her homespun plant knowledge and ad hoc testing, with some modern science and technology to clinch the deal. [Jennifer and I] experiment with [a new formulation] ourselves to see how it feels, to get the texture and viscosity we want and the result we want to get, she said. Then we have chemists that we work with, and they produce the product at their facilities.

I+Is original CBD products include moisturizing Dry Oil Body Mist (jojoba oil, moringa oil, and a blend of citrus essential oils), hydrating Face Serum (prickly pear seed oil and hyaluronic acid), and relaxing Bath Tea Blend (essential oils). One of the most popular products, Kelati said, is Coffee Body Scrub, a foaming exfoliator that contains a combination of seven plant extracts, CBD, and fair-trade Ethiopian coffee. This is something we actually use back home after we finish drinking coffee, Kelati said. Using the leftovers, you can scrub it around your feet to make them smooth.

Other ingredients in traditional Ethiopian and Eritrean health and beauty regimens happen to be on-trend in Western markets right now. Called adaptogens, the materials comprise nontoxic plants and plant derivatives that are believed to help the body resist stress. Scientific research and validation for adaptogens is scarce, but adaptogenic concoctions have been used for centuries in Chinese, African, and other cultures. I realized when I looked at the list of ingredients that I knew growing up, they are actually considered adaptogens, although we didnt call them that, Kelati said. So we decided it makes sense to follow up and create products using those ingredients.

The importance of sourcing products from sustainable, eco-conscious farmers and manufacturers cannot be overstated, she said. All the CBD and most other essential ingredients in I+I products are sourced in the U.S., but Kelati also maintains several key partnerships back home. The main ingredients that represent the product are coming from somewhere we actually source ourselves, she revealed.

I+I primarily operates in the beauty segment, but Kelati wants to grow the business into other verticals and develop products that appeal to a wider audience. One thing we are working on is beard oil so we can give something to the guys, she said.

As legalization has swept across the U.S., social equity has become one of the hottest of the hot-button issues in the cannabis and hemp industries. People of color and their advocates are fighting for a bigger stake in a space that is overwhelmingly controlled by White investors and operators. Despite legislation and other efforts designed to promote ownership and participation by Black, Indigenous, and people of color (BIPOC) stakeholders, very little progress has been made thus far.

As a Black female entrepreneur, Kelati understands the challenges. She is all too familiar with the discrimination and other issues people of color face as they try to build brands from the ground up with scarce access to investment or business loans. But none of this has tempered her enthusiasm, and Kelati is eager to build I+Is momentum across the country in 2021, one mist and scrub at a time.

Being a Black woman, I did recognize at an early stage that were going to have a lot of challenges, Kelati said. People are going to have doubts about everything we say. But I do see theres a lack of mentorship and representation, and when we go to trade shows, we dont see a lot of buyers who are minorities. So, its justI have to prepare myself more than my colleagues or my competition [must], because I just want to make sure I am representing myself fully so I can capture the audience.

On a more optimistic note, she said Black women are one of the fastest growing groups in business, and in the beauty industry she believes the situation is becoming more favorable for BIPOC and women. She cited the 15-percent pledge as one example of the changing tides. In June, Sephoraa French multinational chain of personal care and beauty storesannounced its U.S. unit would pledge to grow the share of Black businesses it helps empower to at least 15 percent. The company created an advisory group that includes brands owned by people of color to help make changes. Sephora currently works with about 300 brands in the U.S., where it has more than 400 retail storefronts plus kiosks in J.C. Penney stores.

One of the most critical factors for young businesses is raising capital, and Kelati said that has been a difficult hurdle to surmount. When it comes to attracting investors, or even getting grants or any kind of loan, its not that easy for women of color, she said. Weve been trying to get investors to be attracted to our message and product, but it has been very, very challenging. Definitely, if there is more mentorship and representation, those can make a big difference.

Kelati also noted most Black business owners are new to the business world and dont have the same level of experience, capital, or the connections enjoyed by many White business owners. Everybody does not start from the same beginning, and Black business owners dont have that foundation. We start everything from scratch, she said. So, to expect to go the same speed as the competition who actually have a good jumpstart, its really not fair. When you go networking, you dont see someone who looks like you or someone who understands your struggle. There are a lot of people who dont understand that its not just about working hard. Its also about having a starting point. The experience is not the same.

Kelati has seen some signs over the past year that give her hope for a brighter future for BIPOC-owned businesses. In the meantime, I+I is doing its part to highlight a proud legacy with roots in East Africa being carried forward by modern-day female entrepreneurs.

To be honest, it makes me comfortable now to say that, yes we have a Black woman business owner, and we always promote ourselves as a woman-owned business and my face is out there, she said, explaining that even in the cosmetics industry, women-owned businesses face a steep climb. It just makes us more comfortable now because we feel like the [equity] movement [in cannabis] is genuine, and people are trying to make a difference and inviting diversity into this industry.

Many studies into the effectiveness of resveratrol focus on its antioxidant, anti-inflammatory, and anti-microbial benefits. The research team found that resveratrol significantly inhibits the virus and prolonged cellular survival, concluding that the natural polyphenol is a potent anti-MERS agent in vitro. Given the similarities between the viruses, its possible that resveratrol could offer some protection against SARS-CoV-2. Indeed, several research studies are already ongoing in this area.

Taking Resveratrol Supplements

The possible health benefits of resveratrol are seemingly endless, with new studies being published every week on its links with improving a multitude of conditions and diseases. However, it should be noted that resveratrol is as yet unregulated and is not prescribed by medical professional. Healthspan & Longevity: What Are The 10 Primary Mechanisms of Aging? – Longevity LIVE

Its an undeniable truth that aging is inevitable. Isnt it? What if almost everything weve been taught about aging is wrong? What if we could change how long we live? In other words, could we choose our longevity by having better healthspan?

The most rapid increase in the 60+ population is occurring in the developing world. It is expected to show a 225% jump between 2010 and 2050.

In our youth, the issue of aging wasnt something we lost sleepover. Our energy levels could withstand anything. We woke up feeling fresh and ready to take on the day. We even indulged in a double-cheese pizza didnt result in our picking up a very resistant 3kg, seemingly overnight.

However, over time, the reality of aging kicks in. Now, science reveals that our lifespan will be determined 70% by our lifestyle and 30% by genetics.

Scientists such as Australian-born David Sinclair, a leading researcher in the field of aging at Harvard Medical School in the US, believes youthfulness can also be reset in the body, using epigenetic reprogramming. He says: Soon we will be able to tweak cells in a particular way that the central nervous system behaves as if it were young again. This is an exciting moment in time for the science of longevity. Researchers are on the verge of understanding the process of aging and how to create drugs that promote extended healthy longevity.

Lifespan refers to how long a person lives, from birth to death. Healthspan, on the other hand, is about how many of those years were lived in complete health. It describes how long we are able to function without the assistance of others, in the absence of chronic disease and frailty.

Oxidative stress occurs when there is an imbalance of free radicals and antioxidants in the body. Examples of free radicals include hydroxyl radical, nitric oxide radical, and superoxide. This can be countered with good-quality antioxidants in the diet and supplements, as well as specific lifestyle changes.

Glycation takes place between reduced sugar and proteins, lipids and nucleic acids, wherein reactive carbonyl groups of a reducing sugar react with free amino groups of proteins to form an unstable Schiff base. Advanced glycation end products are formed through this reaction.

Telomeres are the caps at the end of chromosomes that protect the cells. These wear down as you age until they are so short that they cant protect your cells.

Golding explains that, in his practice, there is a test called a telomere link or teloyears. The longer the link, the younger you are biologically and the longer you will probably live.

So lets say youre 60 years old. Youre significantly overweight. You have early signs of Type-2 diabetes. Youre eating poorly, youre not exercising, and youre stressed out. Generally, in that scenario, your telomere length is going to be very short. This means your lifespan is probably going to be shortened significantly. Once your telomeres disappear, your cells can no longer divide. You enter a state called cellular kinesthesis, where you cant replicate your cells anymore. Thats one marker of what you can do to know your telomere length, and with lifestyle interventions such as proper diet, taking good supplements, sleep, exercise, and managing your stress, you can actually extend your telomere length, whatever your age. Your hormones, genes, social interactions all of these things play a role, and the lifestyle components are very important.

Astragalus membranaceus (AST) is a plant-based compound that demonstrates, in labs, immunomodulatory, antioxidative stress and anti-aging effects which are associated with extending longer telomeres, thus extending lifespan.

By the time one is middle-aged, levels of NAD+ (nicotinamide adenine dinucleotide) have fallen to 50%, with resulting loss of sirtuin and PARP activity.

PARP inhibitors are a group of pharmacological inhibitors of the enzyme poly ADP ribose polymerase (PARP). They are developed for multiple indications, including the treatment of heritable cancers. Sirtuins are a family of histone deacetylases that seem to regulate oxidative stress and damage to DNA, as well as organismal lifespan. Compelling research shows that NAD+ has a unique ability to protect tissues, induce DNA repair, and increase lifespan. It has long been known that NAD+ plays an important role in transferring energy released from glucose and fatty acids to the mitochondria, so that it can be converted into cellular energy.

Recent studies have revealed that the decline in NAD+ levels causes aging-related disorders, and therapeutic approaches increasing cellular NAD+ prevent these disorders in animal models.

Nicotinamide mononucleotide (NMN) has been shown to mitigate many aging-related dysfunctions.

Senescent cell production occurs throughout life and plays beneficial roles in a variety of physiological and pathological processes. This includes embryogenesis, wound healing, host immunity, and tumor suppression.

Meanwhile, throughout life, the steady accumulation of senescent cells in the body with age also has an adverse consequence that leads to age-related death. Senescent cells dont divide or support the tissues of which they are part. Instead, they emit a range of potentially harmful chemical signals that encourage nearby cells to enter into the same senescent state.

Autophagy (the natural process in the body of removing damaged cells, and thereby regenerating newer ones) was originally thought to suppress cellular senescence by removing damaged macromolecules or organelles. Now, recent studies also indicate that autophagy promotes cellular senescence by facilitating the synthesis of senescence-associated secretory proteins. As a result, a different strategy to remove the accumulation of senescent cells is needed.

A new class of drugs, known as senolytics, focuses on the destruction of these stubborn death-resistant cells from the body to reduce inflammation and improve tissue function.

While scientists are looking for more powerful synthetic drugs, Golding incorporates two of the most powerful natural senolytics in his senescent cell removal program. These are fisetin and quercetin. Fisetin is a plant flavonoid found in fruits, mostly in strawberries. Quercetin is a flavonol pigment found in fruits and vegetables, with its highest concentration found in capers. It has antioxidant properties that work to protect the body from free radicals and oxidative stress and is considered to be a more powerful antioxidant than vitamin C, vitamin E or beta carotene.

Resveratrol is an everyday maintenance program against senescent cells. Its a known activator of SIRT1, which slows aging, and has been demonstrated to inhibit mTOR activity and cellular senescence.

Mammalian target of rapamycin (mTOR) is a signaling pathway heavily linked to the aging process.mTOR is stimulated by nutrients and growth factors, and inhibited by stress, to ensure that cells grow only during favorable conditions.

As with most healthspan-related advice, caloric restriction is advised. Fasting and a reduction of calorie intake is implicated with mTOR inhibition. The two most popular options are a small feeding window (four hours per day) or 24-hour fasts two to three times per week.

The SIRT1 gene is a multifunctional protein deacetylase that plays important roles in cellular pathways including longevity, metabolism, cellular senescence, genome maintenance, DNA repair, and inflammation. Thus, the loss of NAD+ has direct and indirect consequences on multiple cellular endpoints. Ultimately, depletion of intracellular NAD+ alters the NAD+/SIRT1 axis and leads to defects in mitochondrial homeostasis, ROS production, DNA repair, and cell survival.

It has consistently been shown that the loss of activity from the NAD+/SIRT1 axis also inhibits mitophagy (the selective degradation of mitochondria by autophagy).

Nicotinamide mononucleotide (NMN) and NR, a key NAD+ precursor intermediate which decreases with age in mammals, is an efficient therapy against age-associated diseases. Research has further demonstrated that NMN activates Sirtuin1 (SIRT1), which is a NAD enhancer.

Proteostasis is the concept that there are competing and integrated biological pathways within cells that control the biogenesis, folding, trafficking, and degradation of proteins present within and outside the cell. One of the results of a loss of NAD+ as a person gets older is a loss of proteostasis, causing havoc on the system.

Once again, NMN and/or NR is an efficient therapy against age-associated diseases, including loss of proteostasis.

Inflammation drives all diseases, and it can ignite a long list of disorders. This can includearthritis, asthma, atherosclerosis, blindness, cancer, diabetes, and, quite possibly, autism and mental illness.

In clinical practice, we can do a simple test, called a C-reactive protein, on the blood, says Golding. Then we can intervene with simple supplements, such as curcumin.

This causes cancers, senescent cells and tumors, and accelerates aging. mTOR is the master anabolic regulator that organizes many other nutrient sensors, such as insulin, leptin and insulin-like growth factor (IGF-1), a hormone that supports growth development, healing, muscle-building and bone strengthening, depending on what the body needs.

Because mTOR is stimulated by insulin, growth factors, blood factors, phosphatidic acids and oxidative processes, when we eat, its best to avoid animal proteins and lots of processed sugars.

Dietary additions such as resveratrol and metformin have been shown to inhibit mTOR upstream. Metformin isnt available in stores, so ask your medical doctor for a prescription. Dont forget that resveratrol should betransandwhitein color.

Rapamycin supplementation has also shown promise with mTOR, but due to the serious side-effects, its not recommended.

This results in sarcopenia, loss of skeletal muscle mass and strength. It can also lead to catabolism of lean tissue and muscle disorders. This can accelerate premature aging and make you prone to other metabolic diseases. Because the body and all the cells consist of protein, which requires the presence of anabolic precursors and building blocks, you need mTOR and amino acids for existence and DNA repair.

About Dr. James Wu

Originally from Canada, Dr. James Wu graduated from the University of Toronto in 1992 and then went on to Boston University and Boston Medical Center to complete his specialty surgical training in oral and maxillofacial surgery in 1997. Dr. Wu is a board-certified oral and maxillofacial surgeon in the U.S. and Canada, and since completion of his specialty training, he has been a clinical assistant professor in the Department of Oral & Maxillofacial Surgery at Boston University School of Dental Medicine.

In addition to Diplomate status with the American Academy of Aesthetic Medicine, Dr. Wu is certified in plant-based nutrition from eCornell and the T. Colin Campbell Center for Nutrition Studies. He is also a NESTA-certified Lifestyle and Weight Management Specialist.

Dr. Wu has a particular interest in facial and aesthetic services, performing many reconstructive surgeries due to facial trauma as well as corrective facial and jaw surgeries since 1997.
