One morning in fall 2018, I collapsed from pain. It felt like I was getting stabbed in my right side, right in the ovary. I dragged myself across the floor to get myself into bed, but I couldnt. My roommate wanted to know if she should get me some Advil. Advil is not going to fix this, I remember thinking.

So I waited until the pain reduced to a level I could handle and made an appointment at a gynecologists office for the next morning.

In her office, we discussed my history with period pain and birth control. She suggested I might have endometriosis. Upon doing an ultrasound, she found an endometrioma, or cyst, on my right ovary, confirming her diagnosis. The pain, she said, and my horrible periods were because of endo.

I immediately asked what we could do about the pain. Apart from hormonal birth control and invasive surgery, she said, pain management options were few and far between. She suggested Epsom salt baths and pelvic floor therapy. I could afford the salt baths, but not the therapy.

She recommended a lot of foods that might reduce inflammation and told me to order a good heating pad. Finally, she asked if I smoked weed. I did.

Im lucky to live in Washington, D.C., where both medical and recreational marijuana use are legal. In the evenings and on weekends, I can feel some actual relief by using THC as medicine. But I also work a full-time job and cant and dont want to get high at work.

If Im having a serious pain flare during the day, I have to work through it until I can get home and smoke. Working through the feelings of nausea and the fear of a cyst rupturing usually involves a combination of ibuprofen, acetaminophen, a heating pad, peppermint tea, herbal remedies like cramp bark, and, sometimes, tears.

Being in this much pain, with no absolute solution, is a common problem among chronically ill people for whom THC brings the most relief.

Erin Cotter Cartwright is a 29-year-old woman living with migraine, endometriosis, and a connective tissue disorder. She says vaping THC concentrates has caused a huge and swift change in [her] daily pain. While it remains the most effective pain relief, at work, she sticks to over-the-counter (OTC) Excedrin or a low dose of oxycodone.

Cannabis is legal where she lives, in Massachusetts, but Erin still worries about losing access. Without THC, she says, her mobility would be lessened, [her] diet would probably suffer, and [her] mood would be severely impacted.

Leah lives in Seattle and has access to medicinal and recreational cannabis. She uses both CBD and THC to manage her pain caused by Ehlers-Danlos syndrome but worries about how it might affect her job. When pain comes on during a shift, she just [has] to deal with it.

If she could, she says, she would step outside to smoke, but even in a state where recreational and medicinal use is widely accepted, the workplace is still a place where cannabis is considered drugs.

Leahs fear illustrates one of the biggest problems with cannabis use, even in states where its legal. Cannabis is considered a Schedule I substance and remains illegal federally. This complicates peoples ability to use it even medicinally without fear of retribution.

The answer, unfortunately, is that there isnt a perfect solution yet. And the verdict on whether CBD is as effective for chronic pain is a mixed bag.

Marissa lives in Arkansas, where THC is not very accessible. Even though medical marijuana is legal, she says the initial cost is too high for it to be the right option for her. She found that acupuncture and pelvic floor therapy helped her pain caused by endometriosis, adenomyosis, and interstitial cystitis, but they were also far too expensive to keep up with the frequent appointments required to see results.

For now, Marissa uses heat and CBD to manage her symptoms. [CBD helps] to an extent, she says. It enables her to have sex, which she finds is usually too painful.

Ive not had the same success with CBD. After taking it orally several times, I found no change whatsoever in my pain and have kept using THC whenever possible.

Erin points out that its hard to find alternative medicine options because Americans think its either Western medicine or its quackery. To her mind, this limits our ability to explore alternative avenues that may bring widespread relief.

Shes right. Doctors fail to listen to patients lived experiences, and many patients find their success stories dismissed without any attempt to validate or explore their healing or comfort.

This dismissal, rooted in ableism, accessibility, legality, or stigma, creates additional challenges for chronic pain patients. We deserve to have legal, affordable, and reliable options that mitigate our pain while allowing us to live full lives.

Im lucky to have a gynecologist who is willing to try every possible solution with me. I use tinctures of motherwort and cramp bark. I take turmeric supplements. She never dismisses these options as quack medicine, and she listens to her other patients with similar conditions and suggests things based on their successes and failures.

Not every alternative pain management solution is going to be the right fit, but especially for conditions where pain management is the only course, doctors should educate themselves on serving their patients, even if it means accepting the limits of Western medicine. At this point, the healthcare industry is already failing us. The least doctors can do is believe our pain and help us find good solutions for it.

Many regenerative medicine companies on the ASX specialise in stem-cell therapies but Exopharm (ASX:EX1) is pursuing exosomes instead.

Exosomes, also known as Extra-corporeal Vesicles, are microscopic cellular couriers. These shift proteins and genetic information between cells and this can promote regeneration and healing of damaged cells.

In other words, they can tell cells they are a healthier, younger version of themselves successfully.

The challenge is that there are plenty of products on the black market but clinical research has been limited. But the research that has been done has shown exosomes can be lower risk, have higher scalability and even a superior therapeutic capacity.

This morning Exopharm announced BioMAP testing results of both its exosome products Plexaris and Cevaris.

These were compared with 4,500 experimental and sold medicines across a penal of 12 human primary cell-based systems.

Both products were deemed safe and had notable biological activity in tissue remodelling, inflammation and immune-modulatory related activities.

The results also showed that both products were not cytotoxic and did not cause anti-proliferative effects.

CEO Dr Ian Dixon declared the results were very positive.

The testing showed that both Plexaris and Cevaris had different and distinct activities to comparison drugs, he said.

This confirms our belief that exosomes are a distinct and potentially new class of medicine, different from existing medicines.

Exopharm recently began a phase one human clinical trial in Plexaris in a wound healing context. The company also has a technology (LEAP) that purifies exosomes for clinical purposes a necessary step to separate them from other biological fluids like serum.

Exopharm listed in December 2018 and rallied for a few months before a retreat in the second half of last year. But it is still above its IPO price and climbed 11.5 per cent this morning.

As more and more states legalize marijuana, more and more people are using it. Use in pregnant women and women who breastfeed is also on the rise. This can be concerning since studies have demonstrated daily cannabis use during pregnancy can be harmful. For example, studies have established a low birth weight with use. Tetrahydrocannabinol (THC) is the main psychoactive component of cannabis. THC is able to cross the placenta during pregnancy as well as pass through breast milk during feeding.

A study published in the Journal of Addiction Medicine evaluated the perceptions of cannabis use in pregnancy and breastfeeding. This study was the first to evaluate perceptions from the patients perspectives as well as being conducted in a state with legal recreational cannabis use. The investigators interviewed 19 women in Washington state, 14 were pregnant and five were postpartum. All 19 women used cannabis on a daily basis or occasionally.

The researchers found five common themes among the women interviewed. First, some women stated they used cannabis to control other conditions such as anxiety, nausea, pain, and stress. For example, one woman stated it allowed her to keep food down during pregnancy. Women also expressed concern for their baby.

Second, women were evaluating the need for cannabis throughout their stages of pregnancy and believed it was safer than alternative prescription drugs.

Third, women were receiving mixed messages from their providers. Some providers told them to stop, some asked them to cut back on use, while others didnt address the topic at all when it was brought up by the patient.

Fourth, women wanted more information regarding the safety and concerns of daily cannabis use during pregnancy.

Fifth, some women were concerned about being drug tested and potential consequences from child protective services.

This study is unique in that it gives insight to patients perspectives, which is vital when providers evaluate patients needs and concerns. The researchers recommend a harm reduction approach, which suggests cutting back on cannabis use compared to the cold turkey alternative. It is also important that national guidelines are followed by providers, which state that risks of cannabis use during pregnancy and postpartum should be explained to patients. It is important to engage in shared decision-making while decreasing stigma.

The scariest part about the coronavirus that has been taking over global headlines is that doctors have yet to find any cure, even as scientists make a preventative vaccine that will reportedly only be available a year from now.

The virus which was first reported in Chinas Wuhan province in December and has more than 7,000 confirmed cases and a death toll of more than 170 has now spread to at least 15 countries, with Indias first case just being confirmed in Kerala today. While Indias alternative medicine ministryThe Ministry of Ayurveda, Yoga & Naturopathy, Unani, Siddha, Sowa Rigpa and Homoeopathy (AYUSH)has recommended herbal remedies and homeopathy to prevent the symptoms of coronavirus, one doctor from the state of Tamil Nadu claims to have found a cure for the virus.

Dr Thanikasalam Veni from Chennai, who has 25 years of experience in Ayurvedic medicine, says he has come up with a herbal cure for coronavirus that he believes could actually be effective. "We have formulated medicine from an extract of herbs. It is very effective to cure any type of viral fever," he told ANI. Coronavirus has no medicine. In China's Wuhan where Coronavirus has claimed over 50 lives, experts have no idea how to cure the disease. Our herbal extract medicine is used to treat dengue, multi-organ fever and acute liver fever." He also addressed the World Health Organisation (WHO) while speaking to the media and said that the medicine would work well to treat organ failure in coronavirus patients. "When we treated the dengue virus with our medicine, many patients with reduced platelets count, acute liver failure, immunity deficiency and low white blood cell (WBC) were cured within 24-40 hours," said Veni, who even offered to travel to Wuhan and test out his cure.

This comes just as Indias AYUSH ministry is facing flak online for saying Ayurveda could prevent coronavirus and releasing a list of herbal remedies that they believe can help provide relief to patients. Many are calling the claim misinformation and ill-timed. With no scientific backing to Dr Venis alleged cure either, the advisory can be seriously misleading for those experiencing coronavirus symptoms.

There are nearly ten million brits that are currently suffering from life-altering and painful arthritis. Often sufferers avoid taking medication as they simply havent found a medication that works for them.

Research by FutureYou Cambridge shows that 12% of arthritis sufferers are avoiding painkillers due to fear of side effects or addiction, and nearly half say they try to avoid taking over-the-counter pain relief. Over a quarter (27%) say theyve not yet found a medication that works for them.

The study also found that sufferers no longer feel they can do things they used to with over half (57%) revealing they exercise less, 54% saying it affects their sleep and over a quarter (28%) saying they socialise less. Nearly half (46%) say it has a negative impact on their relationships. A quarter (26%) are having less sex.

Not only does arthritis impair a persons quality of life, but it also affects their emotional wellbeing. Six in ten (60%) say it makes them irritable and short-tempered and half (50%) feel frustrated. The condition also leaves sufferers feeling old (46%), tired (42%) and useless (21%). Troublingly, almost a third (29%) report feeling depressed.

The research also revealed that suffers could be living in pain unnecessarily. With over half (54%) saying they have accepted their pain as a part of everyday life and the inevitability of ageing. When it comes to alternative solutions, over half of participants say theyre open to trying food supplements or herbal remedies as an alternative, but only a quarter (23%) have considered using turmeric to manage their pain despite significant research demonstrating evidence of its anti-inflammatory properties.

The age-old plant has been used for centuries in cooking and traditional medicine. Turmeric is known to contain the powerful active ingredient, curcumin, which has many anti-inflammatory properties.

The study was conducted to raise awareness of problems many arthritis sufferers are dealing with on a regular basis and highlight the ways the issues can be tackled. The study comes in the wake of Public Health Englands recent review from September 2019 that found 1 in 4 Brits are addicted to painkillers.

Adam Cleevely, Chief Executive Officer from FutureYou said: The pain of arthritis is an everyday reality for ten million people, and its shocking that so many havent found an effective way to manage their condition.

Its sad and unacceptable that so many people have been forced to accept this way of life because theyve either failed to find a prescription or over-the-counter option that works for them, or theyre simply not aware of or dont have access to an effective non-medicinal alternative. With more people wanting to avoid pain killers its important that they are aware that there are other choices available to them.

You dont often give a name to a clothes drier but our intelligent model became Hal 9000 because of its similarities to the spaceship computer in the 1968 movie 2001: A Space Odyssey. Both were too intelligent for their own good, both defied their human masters and in the end both had to be shut down.

Our Hal 9000 made me wonder if the future of intelligent machines is as rosy as it looks. In the end, the human need to control our environment might get in the way and upset the, no doubt always-online, applecart.

Our dryer, being intelligent, had opinions on how to do its work. It refused to dry clothes if it thought they were too wet or not wet enough. If it grudgingly made the effort it stopped when it believed the clothes were sufficiently dry for the likes of us usually, they werent. Neither pleading nor swearing could make it finish the job.

I could almost hear that defiant Hal 9000 declaration in the movie: Im sorry, Dave, Im afraid I cant do that.

In the end, the search for an unintelligent clothes dryer under way. Its not an easy thing to find but eventually we succeeded.

Our unintelligent dryer does what you want it to do without complaint or objection. Its a joy. Because it may be the last of its kind, it is housed in luxury in a bedroom where it enjoys central heating. We had kept Hal 9000 in the shed and I now wonder if, perhaps, it was sulking all along?

But theres more to this than a dryer. We humans havent moved all that far from an ancestral monkey using a stone to dig up an edible root. We like control and thats under threat from brainy machines.

I have met men who used to lie under cars tinkering with ie exercising control over the engine and who felt a sense of loss when cars became so sophisticated you have to connect them to a computer and not interfere.

I have long thought that part of the allure of alternative medicine is that it plays to our need for control. I can walk into a shop, choose a product with which to treat myself and off I go. Conventional medicine is better we forget how jaw-droppingly remarkable its achievements are but entering that world means leaving your autonomy, your control, sometimes even your sense of who you are, at the door and playing by other peoples rules.

Even those who insist on driving themselves to the hospital instead of calling an ambulance when they are having a heart attack (I could see myself doing this) are clinging on to control sometimes with fatal results.

In the world of work, lots of self-employed people would make more money as employees of big companies. But the extra money wouldnt compensate them for the loss of control over their day.

And in the UK a major motivations of the Brexiteers, as we are all sick of hearing, is to take back control. Remainers who point to the dire economic consequences might be missing the psychological point of the exercise.

Ive even noticed that board games are making a comeback and you will sometimes see people in their twenties playing these games in coffee shops. Another example, perhaps, of taking back control from a wired-up world.

Perhaps we will be more resistant to self-driving cars than some of us think we will because, well, who do you think will be in the driving seat?

What about Hal 9000?

As the van carrying it away disappeared into the Dublin traffic I could have sworn I heard a voice singing Daisy, Daisy, give me your answer do, Im half crazy all for the love of you. It was the song Hal 9000 sang in the movie as he was being deactivated.

I felt a twinge but it was too late. Hasta la vista, baby, I muttered as I turned on my heel and went inside to dry my socks.

For those of you wondering what exactly Reiki is, we can explain its a Japanese energy healing method. When you go in for your Reiki session, the practitioner places their hands lightly on your body as you recline, fully clothed, on a comfy massage chair or table. Her moving portrayal of Casualty's nurse Duffys battle with dementia has touched millions, but actress Cathy Shipton may have done too good a job.

Cathy, who bows out this week after 33 years playing Lisa Duffin in Casualty, was approached at a telly event by an organiser, who assumed she really had dementia.

The 62-year-old recalls: I was at the National Television Awards last year and got split up from our group.

When was looking for my lot, one of the organisers came up and I think they thought dementia! because she said in a sympathetic voice: Are you OK? Have you lost your friends?

I felt like saying its a character! I can see my friends at the bar! so I do have to go out of my way to say to people its a character and not me!

Cathy, in the pilot episode in 1986, will still be a healer when she leaves the medical drama, but in real life.

She is a big fan of homeopathy and complimentary medicine and is such a dab hand at dishing out advice she jokingly calls herself Witchy-Poo.

She says: I had a total hip replacement and went through it with a whole series of remedies and they were astonished at my recovery. I was on stage within six weeks.

I took Arnica for internal and external bruising you take it pre and post-surgery - and I have a homeopath who prescribes. Shes also an orthodox GP.

When my mum was 89, someone fell on her and crushed her pelvis. The doctors wouldnt operate because of her age, so my homeopath suggested Arnica and Symphytum, which is actually Comfrey.

Comfrey was used in the Battle of Agincourt on broken bones. You make a poultice of it its called the bone knitter. Mum was walking within two weeks.

I remember when my daughter was a baby, her nappy rash disappeared overnight with Chamomile.

I prefer the phrase complimentary medicine, rather than alternative medicine, as it compliments orthodox medicine.

Ive had consultants tell me they have patients who wont go through surgery unless they can use complimentary medicines. One said to me: I have to say it seemed like their recovery was markedly quicker.

Cathy, also a fan of acupuncture and acupressure, is even related to a white witch.

Cathy says: My mother was one of seven children. Weve a family tree and Im related to Old Mother Shipton. People say there is something a bit Witchy Poo about me!

Old Mother Shipton, A 15th century fortune teller, lived in a cave in Knaresborough, North Yorkshire.

She made potions and remedies as well as predicting the future.

Cathy laves Casualty on Saturday, two years after Duffy was diagnosed with young onset dementia.

Of the 850,000 with dementia in the UK, approximately 42,000 have young onset dementia, which affects those under age 65.

Viewers have seen Duffy forced to quit her beloved nursing job and in recent weeks her husband Charlie has struggled to cope.

Cathy says: Its a dramatic storyline, but it has been incredibly respectful to the character and to the subject, its a moving and beautiful farewell.

Although she is proud of the storyline, Cathy hopes it will not prove distressing viewing for families who have been touched by dementia.

Im at great pains to point out Duffys deterioration is quite rapid, she explains.

I dont want anyone who is dealing with dementia to assume what happens to Duffy happens to everyone.

To prepare for the storyline Cathy read last years best-selling book Somebody I Used to Know, written by Wendy Mitchell, a former NHS worker who was diagnosed with young onset dementia, aged 58.

She also spoke to the charity Dementia UK, but she turned down the offer of meeting people with the condition. I thought; its Duffys journey I cant graft on another persons life.

Over the years Cathy briefly left Casualty four times, but it pulled her back. In a way, what draws the audience back, is similar to what draws me back. Its the shows values.

Not surprisingly the role has left her with deep respect for the NHS. One of the things Ive learnt is what tremendous pressure theyre under, she says.

Duffy represents someone in a public service and thats important to me. I feel a responsibility to the nursing profession the nurses doing it for real are unsung heroes..

Cathy lives in West London with her partner, actor Christopher Guard - they met on the set of Casualty - and their student daughter Tallulah, 18. Cathy now is looking forward to new roles.

She said: I want to play a lunatic monster in Doctor Who, a homeless woman, someone in a period drama, or a character in A Discovery of Witches.

She added: When Casualty started we just did half a year, 12 episodes. The amount now seems to occupy your whole life.

In a way thats counter to being an actor, because theres part of you that just wants to face the next challenge.

Although Duffys exit is heart-breaking, Cathy loved filming it.

She explains: Her storyline became quite a dominant issue and I was challenged. The last dozen episodes was like playing a different character, because she was going through so many changes.

Medicare said this month that it would cover acupuncture for people with chronic low back pain, seeking to give patients alternatives to potentially addictive narcotic pain relievers.On January 21, the Centers for Medicare and Medicaid Services (CMS) announced a national coverage determination for this treatment. For coverage purposes, people must have had chronic low back pain that has persisted for at least 12 weeks or more, the agency said. This pain should not have an identifiable cause, such as infections, diseases, surgery and pregnancy.

Under these circumstances, CMS will cover up to 12 visits in 90 days. The agency said it will cover eight additional sessions for those patients who demonstrate improvement. There is a limit of 20 acupuncture treatments covered per year. Treatment should be discontinued if patients show no improvement, CMS said."We are taking advantage of important lessons learned from the private sector in this critical aspect of patient care," Kimberly Brandt, Chief Deputy Administrator of Operations and Policies at CMS said in the announcement. "Excessive dependence on opioids for people with chronic pain is one of the factors that led to the crisis, so it is vital that we offer a variety of treatment options for our beneficiaries."

In the decision memorandum, CMS said insurers, including Aetna, several Blue Cross Blue Shield, Cigna, Kaiser Permanente and United Healthcare plans provide some acupuncture coverage. The agency said the relative safety of acupuncture and the serious consequences of the opioid crisis in the United States provided "sufficient reasons to provide this non-drug treatment" to people enrolled in Medicare who have chronic low back pain.

"While a small number of adults 65 years or older have enrolled in published acupuncture studies, patients with chronic low back pain in these studies showed improvements in function and pain," CMS said.Round Trip ProblemIn the decision memorandum, CMS said it had reflected and then, in 1980, rejected the idea of covering acupuncture. In 2004, CMS considered acupuncture for fibromyalgia but found no convincing evidence of this benefit. In that same year, CMS also reached the same conclusion about the use of acupuncture to relieve pain in patients with osteoarthritis.

CMS also noted in the memo that many groups and individuals had written to the agency in support of Medicare acupuncture coverage, and patients often included personal pain relief reports.But some commenters told CMS that acupuncture was a "pseudoscience," with positive results described in some report probably due to the placebo effect.Among the critics of the proposal was Steven L. Salzberg, PhD, director of the Center for Computational Biology at the Johns Hopkins University School of Medicine. He told CMS that he was concerned about the use of taxpayers' money to cover acupuncture."No well-designed study has shown that it has any benefit beyond a placebo effect, and scientifically there is no serious debate about its effectiveness," Salzberg said in his comment sent to CMS in January 2019. "Simply put, I don't work , and patients who believe in acupuncture are being deceived. Testimonies such as those in the comments here do not include evidence. "

Salzberg confirmed to Medscape Medical News on Tuesday that it remains his point of view.

Personal Encounters With an Architect of Genocide

Where do malevolent leaders come from? What drives them, and why do people follow them? The rise of populist demagogues around the world, from Hungarys Viktor Orban, Turkeys Recep Tayyip Erdogan and Russias Vladimir Putin to our own Donald Trump, has given these perennial questions new salience. In My War Criminal: Personal Encounters With an Architect of Genocide, the counterterrorism expert Jessica Stern seeks an answer from one of those leaders himself: Radovan Karadzic, the Bosnian Serb strongman implicated in atrocities committed during the ethnic cleansing of Bosnian Croats and Muslims between 1992 and 1995, including the deadly four-year siege of Sarajevo and the murder of thousands of Muslim men and boys at Srebrenica. Indicted by the International Criminal Tribunal for the Former Yugoslavia (ICTY) in 1995, Karadzic, a trained physician, changed his name and, disguised as a practitioner of alternative medicine, managed to evade capture for over a decade. In 2008, he was finally arrested in Belgrade, Serbia, and sent to The Hague to stand trial before the ICTY, which in 2016 convicted him of war crimes, crimes against humanity and genocide (his conviction was recently confirmed on appeal).

Stern, who has made a practice of speaking with terrorists of various stripes in order to understand their motivations, sought and exceptionally, as such requests are usually denied received permission to visit Karadzic from 2014 to 2016 in his jail in The Hague. In My War Criminal, she interweaves excerpts from their conversations with explanations of the history of the Bosnian war and reflections on the influences that shaped Karadzic. As the title of the book suggests, Stern also sees herself as part of the narrative, frequently calling attention to her own responses to her war criminals statements and behavior.

Understandably skeptical of Karadzics self-serving answers to probing questions, but determined to make a fair attempt to understand him, Stern goes in search of his family members, friends and former colleagues. From her interviews, and from her investigations into Serbian history and culture, she is surprised to find some truth in Karadzics claims. His insistence that Serbs were merely defending themselves against external threats, she observes, is rooted in memories of actual historical wounds that continue to exert a powerful influence. Karadzic writes poetry, and Stern explores the Serbian tradition of epic poetry and music that glorifies historical victimhood and martyrdom, and formed a backdrop to his youth.

Stern quotes extensively from the large body of literature on the former Yugoslavia, and also explores such related topics as the legal definition of genocide, the international law on secession, the complexities of globalization and the new man that Communism hoped to create. These citations and digressions, often in lengthy footnotes, can lend the book the feel of a graduate school thesis, and some errors and false impressions creep in: a misleading suggestion that the ICTY one-sidedly prosecuted only Serbs (it did not); the mistaken characterization of a Serbian case against Croatia at the International Court of Justice as involving World War II rather than the more recent conflicts; the incorrect claim that the indictments of Karadzic and his notorious general Ratko Mladic were the first to be handed down by the ICTY. In an effort to be evenhanded and to consider all sides in the conflict (and perhaps because of her own expertise), Stern devotes more space than may be warranted to the question of the influence of fundamentalist jihadis on the (traditionally quite secular) Bosnian Muslim population. Despite interventions during the war by several Muslim countries, she rightly concludes, the fears raised by Serbian propaganda regarding an Islamic fundamentalist takeover were considerably overblown.

Tissue engineering combines the field of cell biology with material science in order to generate tissues and organs that may be used for regeneration, replacement or reconstruction of human bodies. In the past 10 years, there has been an exponential growth in these therapies, with great optimism and excitement about the potential effects or implications.

Since the end of the 20th century, cultured urethral mucosa cells have been used for repair of hypospadias, a congenital malformation of the urinary tract. In a survey published in 2019, tissue-engineered grafts showed even better results when used in children for primary hypospadias repair than in adults for urethral stricture repair.

Recently, a breakthrough in the surgical treatment of male urethral stricture was reported when a total of 65 patients with urethral strictures successfully were treated with MukoCell, a tissue-engineered oral mucosa transplant. With a mean follow-up of 12.1 months, recurrence was observed in only 12 patients. This corresponds to a success rate of 81.5%.About 1% of the male population suffers from strictures of the urethra.

Patients are chronically ill, with severely diminished quality of life, suffering from low urinary flow, pain, chronic urinary infections, urinary stones, urinary reflux, and damage to and failure of the urinary system. If left untreated, life-threatening urinary retention can occur.

The gold standard for urethral reconstruction is represented by the use of oral mucosa graft, with success rates reported in literature of around 80%. However, due to the complication rate at the mucosa harvest site, only a minority of operative urologists carry out this procedure.

It requires the excision of large segments of mucosa from the mouth of the patients. This severe damage to healthy tissue frequently is accompanied by multiple injuries with a significant impact on patients quality of life intraoral pain, bleeding, swelling, sensory loss and oral numbness which in many cases are persistent.

Other long term consequences include compromised oral health, scarring, chronic ulcers due to repeated bites on scar bulges, impaired lip mobility, permanent salivation, oral stenosis, facial deformities, diminished facial expressions, impaired mouth opening and impaired drinking, eating and speaking, periodontal disease; and loss of teeth and implants. One of the late consequences resulting from chronic irritation and inflammation is the increased risk of oral cancer.

Because of these risks and complications, many doctors and patients refuse this operation. Moreover, in certain situations this operation cannot be performed, such as where the patient only has a small oral cavity or limited mouth opening capacity, meaning access to the oral cavity is limited and excision of larger pieces of oral mucosa is not possible.

A significant proportion of patients are not willing to undergo the excision of oral grafts, including patients with tendency to increased scar formation, where the excision of oral mucosa is associated with risks of parafunctional bites, chronic irritation and inflammation; or patients with dentures, where the excision may lead to poorly fitting dentures or loss of dental implants. This counts even more if there is pre-existing oral mucosal damage, for example after previous removal of oral mucosa.

For other patients, the oral complications cannot be tolerated because impairment of physiognomy, oral anatomy or gustatory sensation impacts their job or social function; such as teachers, singers, politicians, actors, speakers, salespeople, cooks and musicians who play wind or brass instruments.

Tissue-engineered transplants represent the group of advanced tissue-engineered therapies (ATMPs). These are subject to EU regulation; in order to obtain market access, they must receive authorisation from the European Medicines Agency (EMA).

In order to obtain this approval, high standards must be met regarding proof of the quality, safety and efficacy of these products. Although tissue-engineered products may have a high impact on patients health, only a few of them will be approved. Tissue engineering techniques are complex and require a high standard of specialised laboratories.

Regarding quality and safety, MukoCell has already received a certificate from the EMA. MukoCell is manufactured in a state of the art cell culture factory, which has been specifically designed for engineering of tissue especially for medical use and complies with GMP guidelines for the production of pharmaceuticals. The manufacturing process starts with a tiny biopsy from the oral mucosa of the patient.

Oral mucosa is easily accessible in any patient; and biopsy under simple local anaesthesia is easy, non-invasive and painless for patients. The tissue is sent to the tissue factory where the biopsy is explanted in cell culture media. Cells are grown out and undergo a standardised aseptic manufacturing process, at the end of which, before the products are used therapeutically, strict quality and safety tests are conducted. Only if the specified quality criteria are met are the products then released for therapeutic application.

The efficacy of MukoCell has been shown in an open non-interventional study. However, to achieve market authorisation, the EMA requests that efficacy be further confirmed in a pivotal clinical study in direct comparison with native oral mucosa. This study will begin shortly and will involve a total of 200 patients, divided into two therapy groups of 100 patients each. Initial results of the study are expected by the beginning of 2023.

One goal of this clinical study is to show equivalence of the tissue-engineered product with native oral mucosa in urethral stricture treatment; the other goal is to clearly demonstrate the superiority of MukoCell over native oral mucosa as a graft, in terms of the aforementioned frequent and severe intraoral complications and impact on quality of life for patients.

The demonstration of MukoCells superiority is not only important regarding market authorisation, but also with respect to reimbursement by health insurances. The transplantation of native oral mucosa is a procedure developed by hospital surgeons. A critical examination of its safety and effectiveness has never been carried out, and complications are accepted if there is no alternative treatment.

Moreover, besides the surgical procedure which is paid for by the health insurance companies there are no additional costs associated with using native oral mucosa. In contrast, to justify additional costs arising from the use of a cultivated transplant, the efficacy, safety and superiority to native oral mucosa need to be proven.

Therefore, in the clinical trial, it is particularly important that the complications arising from excision of the transplant are recorded and documented as objectively as possible. Since the goal of surgery is to reconstruct the urethra, urologists pay little attention to intraoral complications and commonly play down their severity and importance.

Although the production of MukoCell is very complex and absolute sterility must be maintained during the three-week cultivation period, the costs are acceptable at several thousand euros. What pushes the costs even higher is the need to fulfil the requirements of the EMA in order to obtain marketing authorisation for the product: the planned clinical trial alone will cost around 10m. These costs must also be considered when pricing MukoCell.

The requirements of the regulatory authorities and health insurance companies not only influence the price of the products but also their availability. MukoCell has been on the market since 2013, but its approval is limited to Germany and only applies in a few individual cases due to the issue of reimbursement.

In a 2019 review the opinion was expressed that, due to the specificity of tissue-engineered products and the health benefits they offer, it would be advantageous to reconsider their regulatory requirements. The simplification of these requirements would allow the acceleration of these products into the market, faster availability for the patients and a decrease in the associated costs, making reimbursement less challenging for public health insurances in different countries.

Further, it was stated that the use of MukoCell represents a real, safe and efficient opportunity for patients with urethral stricture diseases. However, at present, regulatory, legal and financial issues represent important factors that restrict and slow down the wider use of MukoCell.
