An artificial intelligence company that gained fame for designing computer systems that could beat humans at games has now made a huge advancement in biological science.

The company, DeepMind, which is owned by the same parent company as Google, has created an AI system that can rapidly and accurately predict how proteins fold to get their 3D shapes, a surprisingly complex problem that has plagued researchers for decades, according to The New York Times.

Figuring out a protein's structure can require years or even decades of laborious experimentation, and current computer simulations of protein folding fall short on accuracy. But DeepMind's system, known as AlphaFold, required only a few hours to accurately predict a protein's structure, the Times reported.

Proteins are large molecules that are essential for life. They are made up of a string of chemical compounds known as amino acids. These "strings" fold in intricate ways to create unique structures that determine what the protein can do. (For example, the "spike" protein on the new coronavirus allows the virus to bind to and invade human cells.)

Nearly 50 years ago, scientists hypothesized that you could predict a protein's structure knowing just its sequence of amino acids. But solving this "protein folding problem" has proved enormously challenging because there are a mind-boggling number of ways in which the same protein could theoretically fold to take on a 3D structure, according to a statement from DeepMind.

Twenty-five years ago, scientists created an international competition to compare various methods of predicting protein structure something of a "protein olympics," known as CASP, which stands for Critical Assessment of Protein Structure Prediction, according to The Guardian.

In this year's challenge, AlphaFold's performance was head and shoulders above its competitors'. It achieved a level of accuracy that researchers were not expecting to see for years.

"This computational work represents a stunning advance on the protein-folding problem, a 50-year-old grand challenge in biology," Venki Ramakrishnan, president of the Royal Society in the United Kingdom, who was not involved with the work, said in a statement. "It has occurred decades before many people in the field would have predicted. It will be exciting to see the many ways in which it will fundamentally change biological research."

For the competition, teams are given the amino acid sequences of about 100 proteins, the structures of which are known but have not been published, according to Nature News. The predictions are given a score from zero to 100, with 90 considered on par with the accuracy of experimental methods.

AlphaFold trained itself to recognize the relationship between the amino acid sequence and protein structure using existing databases. Then, it used a neural network a computer algorithm modeled on the way the human brain processes information to iteratively improve its prediction of the unpublished protein structures.

Overall, AlphaFold had a median score of 92.5. That's up from a score of less than 60 that the system achieved in its first CASP competition in 2018.

The system isn't perfect in particular, AlphaFold did not perform well in modeling groups of proteins that interact with each other, Nature News reported.

But the advance is a game-changer.

"I think it's fair to say this will be very disruptive to the protein-structure-prediction field. I suspect many will leave the field as the core problem has arguably been solved," Mohammed AlQuraishi, a computational biologist at Columbia University told Nature News. "It's a breakthrough of the first order, certainly one of the most significant scientific results of my lifetime."

DeepMind previously made headlines when it created an AI program, known as AlphaGo, that beat humans at the ancient game of Go.

Researchers hope AlphaFold can have many real-world applications. For example, it could help identify the structures of proteins involved in certain diseases and accelerate drug development.

DeepMind is currently working on a peer-reviewed paper on its work on AlphaFold, the Times reported.

Protein folding is the process by which a protein takes its shape from a string of building blocks to its final three-dimensional structure, which determines its function.

By better predicting how proteins take their structure, or fold, scientists can more quickly develop drugs that, for example, block the action of crucial viral proteins.

Solving what biologists call the protein-folding problem is a big deal. Proteins are the workhorses of cells and are present in all living organisms. They are made up of long chains of amino acids and are vital for the structure of cells and communication between them as well as regulating all of the chemistry in the body.

This week, the Google-owned artificial intelligence company DeepMind demonstrated a deep-learning program called AlphaFold2, which experts are calling a breakthrough toward solving the grand challenge of protein folding.

Proteins are long chains of amino acids linked together like beads on a string. But for a protein to do its job in the cell, it must fold a process of twisting and bending that transforms the molecule into a complex three-dimensional structure that can interact with its target in the cell. If the folding is disrupted, then the protein wont form the correct shape and it wont be able to perform its job inside the body. This can lead to disease as is the case in a common disease like Alzheimers, and rare ones like cystic fibrosis.

Deep learning is a computational technique that uses the often hidden information contained in vast datasets to solve questions of interest. Its been used widely in fields such as games, speech and voice recognition, autonomous cars, science and medicine.

I believe that tools like AlphaFold2 will help scientists to design new types of proteins, ones that may, for example, help break down plastics and fight future viral pandemics and disease.

I am a computational chemist and author of the book The State of Science. My students and I study the structure and properties of fluorescent proteins using protein-folding computer programs based on classical physics.

After decades of study by thousands of research groups, these protein-folding prediction programs are very good at calculating structural changes that occur when we make small alterations to known molecules.

But they havent adequately managed to predict how proteins fold from scratch. Before deep learning came along, the protein-folding problem seemed impossibly hard, and it seemed poised to frustrate computational chemists for many decades to come.

The sequence of the amino acids which is encoded in DNA defines the proteins 3D shape. The shape determines its function. If the structure of the protein changes, it is unable to perform its function. Correctly predicting protein folds based on the amino acid sequence could revolutionize drug design, and explain the causes of new and old diseases.

All proteins with the same sequence of amino acid building blocks fold into the same three-dimensional form, which optimizes the interactions between the amino acids. They do this within milliseconds, although they have an astronomical number of possible configurations available to them about 10 to the power of 300. This massive number is what makes it hard to predict how a protein folds even when scientists know the full sequence of amino acids that go into making it. Previously predicting the structure of protein from the amino acid sequence was impossible. Protein structures were experimentally determined, a time-consuming and expensive endeavor.

Once researchers can better predict how proteins fold, theyll be able to better understand how cells function and how misfolded proteins cause disease. Better protein prediction tools will also help us design drugs that can target a particular topological region of a protein where chemical reactions take place.

The success of DeepMinds protein-folding prediction program, called AlphaFold, is not unexpected. Other deep-learning programs written by DeepMind have demolished the worlds best chess, Go and poker players.

In 2016 Stockfish-8, an open-source chess engine, was the worlds computer chess champion. It evaluated 70 million chess positions per second and had centuries of accumulated human chess strategies and decades of computer experience to draw upon. It played efficiently and brutally, mercilessly beating all its human challengers without an ounce of finesse. Enter deep learning.

On Dec. 7, 2017, Googles deep-learning chess program AlphaZero thrashed Stockfish-8. The chess engines played 100 games, with AlphaZero winning 28 and tying 72. It didnt lose a single game. AlphaZero did only 80,000 calculations per second, as opposed to Stockfish-8s 70 million calculations, and it took just four hours to learn chess from scratch by playing against itself a few million times and optimizing its neural networks as it learned from its experience.

AlphaZero didnt learn anything from humans or chess games played by humans. It taught itself and, in the process, derived strategies never seen before. In a commentary in Science magazine, former world chess champion Garry Kasparov wrote that by learning from playing itself, AlphaZero developed strategies that reflect the truth of chess rather than reflecting the priorities and prejudices of the programmers. Its the embodiment of the clich work smarter, not harder.

Every two years, the worlds top computational chemists test the abilities of their programs to predict the folding of proteins and compete in the Critical Assessment of Structure Prediction (CASP) competition.

In the competition, teams are given the linear sequence of amino acids for about 100 proteins for which the 3D shape is known but hasnt yet been published; they then have to compute how these sequences would fold. In 2018 AlphaFold, the deep-learning rookie at the competition, beat all the traditional programs but barely.

Two years later, on Monday, it was announced that Alphafold2 had won the 2020 competition by a healthy margin. It whipped its competitors, and its predictions were comparable to the existing experimental results determined through gold standard techniques like X-ray diffraction crystallography and cryo-electron microscopy. Soon I expect AlphaFold2 and its progeny will be the methods of choice to determine protein structures before resorting to experimental techniques that require painstaking, laborious work on expensive instrumentation.

One of the reasons for AlphaFold2s success is that it could use the Protein Database, which has over 170,000 experimentally determined 3D structures, to train itself to calculate the correctly folded structures of proteins.

The potential impact of AlphaFold can be appreciated if one compares the number of all published protein structures approximately 170,000 with the 180 million DNA and protein sequences deposited in the Universal Protein Database. AlphaFold will help us sort through treasure troves of DNA sequences hunting for new proteins with unique structures and functions.

As with the chess and Go programs AlphaZero and AlphaGo we dont exactly know what the AlphaFold2 algorithm is doing and why it uses certain correlations, but we do know that it works.

Besides helping us predict the structures of important proteins, understanding AlphaFolds thinking will also help us gain new insights into the mechanism of protein folding.

One of the most common fears expressed about AI is that it will lead to large-scale unemployment. AlphaFold still has a significant way to go before it can consistently and successfully predict protein folding.

However, once it has matured and the program can simulate protein folding, computational chemists will be integrally involved in improving the programs, trying to understand the underlying correlations used, and applying the program to solve important problems such as the protein misfolding associated with many diseases such as Alzheimers, Parkinsons, cystic fibrosis and Huntingtons disease.

AlphaFold and its offspring will certainly change the way computational chemists work, but it wont make them redundant. Other areas wont be as fortunate. In the past robots were able to replace humans doing manual labor; with AI, our cognitive skills are also being challenged.

Sifting through the trillions of molecules out there that might have powerful medicinal effects is a daunting task, but the solution biotech has found is to work smarter, not harder. Genesis Therapeutics has a new simulation approach and cross-disciplinary team that has clearly made an impression: the company just raised a $52 million A round.

Genesis competed in the Startup Battlefield at Disrupt last year, impressing judges with its potential, and obviously others saw it as well in particular Rock Springs Capital, which led the round.

Over the last few years many companies have been formed in the drug discovery space, powered by increased computing and simulation power that lets them determine the potential of molecules in treating certain diseases. At least thats the theory. The reality is a bit messier, and while these companies can narrow the search, they cant just say here, a cure for Parkinsons.

Founder Evan Feinberg got into the field when an illness he inherited made traditional lab work, as an intern at a big pharma company, difficult for him. The computational side of the field, however, was more accessible and ended up absorbing him entirely.

He had dabbled in the area before and arrived at what he feels is a breakthrough in how molecules are represented digitally. Machine learning has, of course, accelerated work in many fields, biochemistry among them, but he felt that the potential of the technology had not been tapped.

I think initially the attempts were to kind of cut and paste deep learning techniques, and represent molecules a lot like images, and classify them like youd say, this is a cat picture or this is not a cat picture, he explained in an interview. We represent the molecules more naturally: as graphs. A set of nodes or vertices, those are atoms, and things that connect them, those are bonds. But were representing them not just as bond or no bond, but with multiple contact types between atoms, spatial distances, more complex features.

The resulting representation is richer and more complex, a more complete picture of a molecule than youd get from its chemical formula or a stick diagram showing the different structures and bonds. Because in the world of biochemistry, nothing is as simple as a diagram. Every molecule exists as a complicated, shifting 3D shape or conformation where important aspects like the distance between two carbon formations or bonding sites is subject to many factors. Genesis attempts to model as many of those factors as it can.

Step one is the representation, he said, but the logical next step is, how does one leverage that representation to learn a function that takes an input and outputs a number, like binding affinity or solubility, or a vector that predicts multiple properties at once?

Thats the work theyve focused on as a company not just creating a better model molecule, but being able to put a theoretical molecule into simulation and say, it will do this, it wont do this, it has this quality but not that one.

Some of this work may be done in partnerships, such as the one Genesis has struck up with Genentech, but the teams could very well find drug candidates independent of those, and for that reason the company is also establishing an internal development process.

The $52 million infusion ought to do a lot to push that forward, Feinberg wrote in an email:

These funds allow us to execute on a number of critical objectives, most importantly further pioneering AI technologies for drug development and advancing our therapeutics pipeline. We will be hiring more top notch AI researchers, software engineers, medicinal chemists and biotech talent, as well as building our own research labs.

Other companies are doing simulations as well and barking up the same tree, but Feinberg says Genesis has at least two legs up on them, despite the competition raising hundreds of millions and existing for years.

Were the only company in the space thats working at the intersection of modern deep neural network approaches and biophysical simulation conformational change of ligands and proteins, he said. And were bringing this super technical platform to experts who have taken FDA-approved drugs to market. Weve seen tremendous value creation just from that the chemists inform the AI too.

The recent breakthrough of AlphaFold, which is performing the complex task of simulation protein folding far faster than any previous system, is as exciting to Feinberg as to everyone else in the field.

As scientists, we are incredibly excited by recent progress in protein structure prediction. It is an important basic science advance that will ultimately have important downstream benefits to the development of novel therapeutics, he wrote. Since our Dynamic PotentialNet technology is unique in how it leverages 3D structural information of proteins, computational protein folding similar to recent progress in cryo-EM is a nice complementary tailwind for the Genesis AI Platform. We applaud all efforts to make protein structure more accessible such that therapeutics can be more easily developed for patients of all conditions.

Diets are often associated with the Three Ds: Deprivation, Defeat and Depression. These emotions can trigger a cycle of compulsive overeating, food obsessions, and emotional eating binges.

Most diets focus on weight (that is, chasing that elusive number on the scale) and not on how you feel.

Diets are things youre driven to get on, but cant wait to get off. Your eating plan needs to be something you can follow and enjoy for a lifetime.

Routine dieters usually start diets on Mondays or at the beginning of a new year. Consequently, they eat uncontrolled portions of their favorite fattening foods vowing that theyll never eat them again.

The now or never mindset often results in gaining more weight before the weight loss journey begins.

Avoid dwelling on having gone back for seconds (or thirds) and/or indulging in your comfort foods.

Beating yourself up because you went off your diet will only bring on negative feelings of failure, guilt, and frustration which trigger the emotional eating cycle.

Starving yourself, that is, consciously and severely restricting calories, will drive you to want to eat everything in sight and will ignite the cycle of binging all over again.

Extreme caloric restriction and crash dieting diminish muscle mass. Since your heart is a muscle, you can compromise heart function.

The human body is not meant to be more active when it is chronically hungry.

4) DONT punish yourself with a workout.

Workouts are meant to reinforce positive feelings, i.e., increased energy and greater psychological well-being, not negative ones.

Also, dont negotiate calories with exercise.

5) DONT dwell on past lapses.

Stop recollecting what you ate with regret and guilt. Move on. Learn from your behavioral drift. Otherwise, dont look back unless you plan on going that way.

6) DO eat mindfully the next time you eat. Stay in the present. Youre not dis-tracted by the TV, computer, or phone, and youre focused on your food.

7) DO acknowledge your achievements.

At the end of each day, ask yourself what you did to get you closer to your goal and acknowledge that achievement no matter how small. These are the kinds of thoughts that will propel you forward and sustain your motivation.

Focus on what you LIKE about yourself. Stop the negative self-talk.

The Takeaway: Losing weight is not a sprint, but a slow and steady race. Rapid weight loss almost always results in rapid weight gain. Focus on progress, not perfection.

JOHOR BARU: The Johor Sports Master Plan 2030, which is expected to be launched early next year, is part of the states efforts to turn the sports industry into a major economic contributor and nurture a healthy lifestyle among Johoreans.

Johor tourism, youth and sports committee chairman Datuk Onn Hafiz Ghazi said the plan was part of the sports and healthy lifestyle initiative announced by Johor Mentri Besar Datuk Hasni Mohammad during the recent state budget tabling.

The master plan is part of our ongoing plans to promote sports tourism with the sports industry becoming one of our main economic contributors aside from nurturing a healthy lifestyle among the people.

Although the sports master plan emphasises high-performance sports initiatives through the Johor Sports Council (MSNJ), it is also trying to make sports the new economic pillar of the state, Onn Hafiz said when contacted.

He had previously said that the state government was in the final phase of completing the master plan.

He explained that it covered the requirement of four key audiences which included high performance, lifestyle, learning and business community.

The aim is also to encourage the creation of a sustainable sports ecosystem as an industry in Johor, he said, adding that the master plan would also focus on job creation, learning opportunities, events, sports technology and research, and sports tourism.

Onn Hafiz said MSNJ would be working together with Sports Johor Sdn Bhd and Loughborough University from the United Kingdom to better prepare Malaysia Games (Sukma) athletes for the Sukma XX Johor 2022 competition.

The state government has allocated RM1mil to the Johor Sports Associations Professionalism Programme aside from awarding a grant to sports associations according to the grade given by MSNJ.

The government will also provide a platform for the people to take part in sports programmes such as the Xtif Johor Prosperous Sports Carnival with a total of RM1.9mil allocation, he disclosed, saying that it involved sports such as sepak takraw, badminton and basketball.

He added that the carnival would be conducted throughout 56 state constituencies in Johor aside from other programmes such as Xtif Johor Fun Ride, Fun Run and E-sports organised under the Johor Youth Development Centre.

The Missoula Public Health City-County Health Department is offering a National Diabetes Prevention Program called Prevent T2 starting Jan. 20, 2021. It will continue through December 2021. Prevent T2 is a lifestyle balance program carefully designed to help reduce the risk of developing Type 2 diabetes. This course will equip participants with the most recent research in healthy lifestyles.

With a focus on nutritional education, physical activity guidance and stress management tools, Prevent T2's one-year course will provide group support to help you reach your health and wellness goals. Prevent T2 is designed for making and sustaining healthy lifestyle changes, supporting prolonged results and prevention of chronic diseases. All classes will be provided online. Classes are held weekly for 16 weeks, followed by four biweekly sessions and then six monthly sessions.

Prevent T2 is paid in full by Missoula City, County and Medicaid insurances. Full scholarships are also available. If you or someone you know is looking to improve their health and prevent chronic disease, this is an excellent program!

Dr Jignesh Kothari opines, I think nuts are very good. I have them every day, and so does my son. I think it should be a part of our everyday diet. Unfortunately, it is interesting that, with braces, they are prevented from having nuts, because they could break the braces. There is a massive list of dos and donts that came when a young child came to us to get fixed with conventional braces. Now, with the Invisalign system, which is removable, all my kids are free to eat everything. We encourage them to have badam and akhrot and everything. I think it is very healthy and they should have it every day.

Nutrition Expert Rujuta Diwekar says, I think every raging teenager should have at least a handful of cashews every day. Also, some growing children have these growing pains. So a handful of peanuts is really good for them. Walnuts, badam, pista- these are what they should be eating. Useful in fibre, an easy source of Vitamin B and great in taste, they make for an excellent quick snack and a much healthier alternative. I think if you are going to get dental help to take better care of your oral health, it should empower you to eat everything healthy instead of preventing you from eating food that is critical for your growth stage.

Building for care: MidMichigan Health completes Heart and Vascular Center focused on care continuum – Concentrate

The second phase in a multi-year investment in health care in the region, MidMichigan Health recently completed construction of the new Heart and Vascular Center located on the campus of MIdMichigan Medical Center - Midland. At more than 170,000 square feet, the new facility consolidates all of the Medical Centers heart and vascular physician offices and specialty clinics, creating a regional destination for heart and vascular care.

Now housed under one roof, the Center focuses on everything from prevention and screening programs, outpatient services, is home to several specialty clinics, emergency services, advanced treatment and research, as well as rehabilitation and restoration.

The initial phase of the $62.8 million project was the new Orchard Building, completed in mid-2019, which is attached to the Heart and Vascular Center, houses support services including laboratory, diagnostic imaging and cardiovascular testing.

This project has been three years in the making and to see it complete, and see our vision as a reality is really impactful, says Sunita Vadakath, M.D., vice president of service lines for MidMichigan Health. The Center will advance our care model, streamline many of our services, encourage collaboration among providers and help us deliver unmatched patient care.

MidMichigan Health provides for care along the entire spectrum of heart and vascular needs, with the exception of pediatric cardiac care and transplant procedures.

We have designed a comprehensive program to look at the entire care continuum, says Dr. Vadakath. From screening and preventative sessions around heart disease, to complex procedures like the Transcatheter Aortic Valve Replacement (TAVR) and the Watchman Left Atrial Appendage Closure Implant, among others.A waiting room at the MidMichigan Health Heart and Vascular Center.

Situated in between the emergency department and MidMichigan Healths inpatient hospital towers, the new Center offers direct adjacent access to both operating and cath lab facilities.

For convenience and ease of care, we now have one integrated system that allows us to treat patients much more efficiently, even if that involves multiple specialties, says Dr. Vadakath. In cases of urgency, time is at a premium in cardiac care, so this will allow us to continue to strengthen our care model and provide the best environment for positive outcomes.

The integration also helps build on MidMichigan Healths strong performance in emergency response for cardiac care, particularly patients that suffering from ST segment elevation myocardial infarction (STEMI) the most serious type of heart attack.

In 2019, MidMichigan Health completed more than 82,000 outpatient heart and vascular visits and more than 4,000 inpatient heart and vascular discharges, with 62% of the overall heart patients coming from outside Midland County.

The Center advances MidMichigan Healths quality of care through the ability to expedite treatment and services, especially helpful for patients who are not living within the county and traveling for specialty care.

With so many of our patients coming from outside Midland, often it means an entire day for both patients and their family or friends, says Dr. Vadakath. Especially when it comes to some of our more complex procedures, there is advanced testing and multiple specialists involved. So, we wanted to make sure that that time was easy for both patients and their families.

That meant designing for a calm and healing environment built around soothing and natural surroundings. That was achieved through adding abundant natural light, a large water feature and indoor living landscapes.

The design reflects our intent to look at and treat the patient in a more holistic way, says Dr. Vadakath.

The progression and depth of MidMichigan Healths cardiovascular program has been significant since starting with open-heart surgery and angioplasty in 2007.

In a little over a decade, weve transformed from a smaller regional hospital to now a regional cardiac referral center, says William Felten, M.D., cardiovascular service line chief. It gives us the ability to provide advanced cardiac care, function as the patients one-stop service, and do that as efficiently as possible.

The added benefit is the new Center gives MidMichigan Health the ability to recruit top-notch talent, not just for heart surgeons, but for other specialties, as well.

Currently, MidMichigan Health serves 23 counties and approximately 938,000 patients as far north as Cheboygan.

Part of the expanded cardiovascular programs includes the addition of a demonstration kitchen, which gives MidMichigan Health the ability to support wellness programs and incorporate all aspects of a healthy lifestyle, including live cooking demonstrations in a kitchen for classes on-site. The Center also houses the space to host conferences and other events.

I am training as hard as ever. I actually took three weeks off, to rest for the first time in my life and it made me feel better and now I think I have to work at learning how to do that. Usually, I work as hard as I can, but I think that rest and taking time off will actually help strengthen any limitations I have. Plant-based eating is part of training to my fullest and working with myself.

Venus Williams: It's really obvious to me that living a plant-based lifestyle has helped me. Happy Viking impressed me so much and I want to continue to make plant-based eating a lifestyle choice, and help other people do it too, since going plant-based is what gave me the ability to get back on the court and do what I love. So that in itself is why it's so organically amazing. This is the real deal, and it led me to be able to do what I love.

Venus Williams: Yes. In the morning I have an interesting routine and I don't do what most other athletes do. I don't eat before I train. So sometimes I will have a protein shake or fruit for a quick boost, in the middle of my morning workout, if I need it. I will do a 5- or 6-hour workout, in the morning. But generally, I go without food until lunch, and then I havea massive lunch. But I use the protein shakes for recovery. It's not just a protein drink, it's great for energy and for recovery. The amino acids are great for recovery. A protein drink is something that your body really does need. My favorite flavor? I've got to with chocolate.

Venus Williams:Educate yourself. First know why you're doing it. There are so many benefits to your health. There is so much we could talk about that we probably don't have time for this conversation.

But there are a lot of benefits from it: Less inflammation in the body, less disease, more energy.And if you are the kind of person to go fromzero to 100, then go for it. But otherwise, try cutting out meat and then dairy, and do it in phases. You can just ease into it. Some people start with one meal a day. So there are a lot of ways to get started. But know why.

Venus Williams:What do I eat? Well, nothing in the morning. Because as I said I don't eat before training.

Then I have a massive lunch. It depends on my mood. But I love to have beans and rice and lentils. That's what I love... I am so hungry after training, and I have to go from the gym to the office and I orderlunch so that it's waiting there for me. And when it gets there I am so hungry and happy to eat.

Some days I will have fried rice but generally, that is not enough protein, or I will have a Happy Viking shake for the protein or a super green shake. Something healthy.

Dinner is always a big question mark. Usually, it'ssomething easy like sweet potato.I love soups, hummus. I don't like a lot of carbs. I don't eat a lot of carbs after training. I naturally seem to do a keto die
t, and a lot of time I also do intermittent fasting, which know is hot right now. A lot of times I take my Happy Viking protein shake with me, so if I get hungry I can eat that instead of something unhealthy. But for dinner, it's always a question. CAIRO: Any Cairene with a real love for Lebanese pastries can tell you how tough it is to find a freshly baked manousheh made the authentic Lebanese way here in Cairo.

This is precisely why fans of this Lebanese breakfast pizza rejoiced when Beiruts very own the Lebanese Bakery (TLB) finally opened in Egypts capital city last September.

Located in the Maadi neighborhood, the branch like all TLB outlets was founded with the mission to pay homage to the manousheh, to subtly enhance and modernize traditional recipes with creativity and attention to every bite, according to TLBs website. The bakerys aim remains to serve a delicious take on traditional manousheh and mouajjanet the quintessential everyday Lebanese food.

The options on the manousheh section of the menu include From the Garden, Cheese, Meat, and All Day Breakfast. TLB serves a selection of manoushehs with free-range eggs cooked sunny-side-up or paired with kashkawan cheese, sujuk, awarma or shakshuka sauce. There is also a section of salads, sides and kaak, and a vast selection of freshly baked mouajjanet and sweets. We were also impressed to find that you can select your own dough for your manousheh, with the options of whole wheat or multigrain. Besides the items on its well-curated menu, the bakery also sells its own condiments including wild cucumber pickles, cocktail pickles and an eggplant makdous (pickled eggplant).

We visited the bakery on a cool Friday morning. It is located in a beautiful two-story building with both indoor and outdoor seating areas. We grabbed a cozy table outside to enjoy the November sun.

We selected a labna wa joz side, ras asfour manousheh, zaatar and akkawi cheese manousheh, and an assortment of mouajjanet.

The food arrived promptly, and was served with a proper cappuccino (one of the best coffees Ive had in Cairo and reason enough in itself to drop by for breakfast).

First to our table was labneh wa joz a magnificent labneh, walnut and black olive side dish served with freshly baked bread. The tanginess of the labneh was brought out by the bitter taste of the olives, but it was the choice to include crunchy chopped walnuts that really elevated this dish, which could easily suffice as a breakfast item on its own.

Next up was the zaatar and akkawi cheese manousheh (aka nes-nes or half-and-half); a ras asfour manousheh composed of diced beef, rocket leaves and pomegranate molasses and topped with fresh pomegranate; and finally, a hot-out-the-oven selection of mouajjanet.

The zaatar and akkawi cheese manousheh transported us back to our Beirut foodie trips the sourness of the zaatar perfectly complemented by the saltiness of the akkawi cheese, with the manousheh crust striking just the right balance between thin and fluffy.

As delicious as that was, however, our manousheh-of-the-year contender was the delicious ras asfour manousheh. From the juiciness of the beef and the hint of pungent sweetness created by the pomegranate molasses to the crunchiness of the rocket and pomegranate, this was the highlight of the meal.

The mouajjanet selection was a hit too both in terms of overall taste and baking quality. We sampled the thyme mouajjanet, spinach fatayer, cheese mini-pizzas, and small sfeehas. Straight out of the oven with just the right amount of filling, the mouajjanet complemented this hearty Lebanese breakfast perfectly.

Sadly though, we had no room left for any of TLBs sweet pastries. The sweet manoushehs carob molasses and tahini; Nutella; or Halawa with strawberry and pistachios looked superb and well definitely go back to try them some other time.

Cairo-based Manousheh fans who cant make the trip out to Maadi can request delivery. Theres also a scheduled pick-up option if youre in a hurry.

Personally though, wed highly recommend visiting the bakery on an early morning and enjoying your manoushehs in the cool breeze.

While there is some evidence that people who are living with HIV and other chronic illnesses may have an increased risk of experiencing severe symptoms and death from Covid-19 this young Limpopo woman who was born with HIV says she is doing everything in her power to ensure that she takes her medication daily.

Twenty year-old Vhuhwavho Nevhungoni from rural Thathe-Vondo village in Limpopo outside Thohoyandou was born HIV positive.

She has faced the virus since her childhood and mastered the difficult process of sticking to her medications and taking good care of herself.

While growing up I made it my point that I had to take my medications all the times to avoid being sick and lead a healthy lifestyle, she said.

It is a huge responsibility for a young person to always remember that you have to take your medications on time almost every day, but I was able to do it and now I am happy and healthy, she said.

When South Africa went into hard-country wide lockdown earlier this year to combat the spread of Covid-19, Nevhungoni feared that she would not be able to receive her medication which she collects on monthly basis.

But thanks to her medical aid she had been able to receive her ARVs without a hitch.

At first I was worried that I will not be able to receive my medication due to the lockdown but to my surprise I was able to receive them with ease, said Nevhungoni.

As I am on a medical aid, I usually collect my medication at a private pharmacy every-month, so even during the hard lockdown they made sure that they kept my package safe, until I went to collect, she said.

According to the World Health Organization (WHO) a record of 26 million people are on antiretroviral treatment.

There is a growing problem however in 2020 with the number of people who are not being treated believed to top 12 million.

That gap is jeopardising our goal of ending AIDS as a public health threat by 2030. Covid-19 has had a profound effect on people living with HIV, as it has for many diseases, said WHO Director-General Dr Tedros Adhanom.

The increased risk has been compounded by disruptions to treatment for people living with HIV.

Sticking to Covid-19 precautions

Nevhungoni said despite taking her ARVs, she has been sticking to all Covid-19 precautions to ensure that she is protected from the pandemic.

I always follow all the safety cautions that everyone else is following such as wearing of mask, sanitizing all the times and mostly keeping to myself. I also try to limit the number of friends whom I spend time with, she said.

Though she managed to overcome various forms of stigma and discrimination while growing up in Vhembe, Nevhungoni said she feels that something still needs to be done to educate rural people about issues pertaining to HIV/Aids.

There is a fear of being judged by your peers and people in rural villages in particular and this stops young people from being open about their HIV status.

The in her third year studying Social Work student at the University of Pretoria, Nevhungoni said that she feels free to live openly with her HIV status in urban areas in Gauteng as she believes that in those areas people have better understanding when it comes to HIV/Aids than in rural communities in Vhembe.

In rural villages some people are judgemental when it comes to people who are living with HIV hence it is not easy for one to disclose their HIV status. Some children in rural villages have been called witches because of their HIV status and have found it difficult during school trips to take medications in front of everyone in fear of being judged, said Nevhungoni.

Nevhungoni also believes a lack of support groups for people living with HIV in rural areas compared to those living in urban areas is a major impediment to this sort of transparency.

In Vhembe something needs to be done to broaden the support, which might go a long way in ending stigma in rural areas. Some people end up defaulting on their medications if they are not in support groups, as they become depressed, she said.

Living without her mother

After she losing her mother to HIV when she was only two years old, Nevhungoni was legally adopted by the late Sue Anne Cook, who is the founder of Vhutshilo Mountain school which looks after HIV/Aids orphans in Vhembe to ensure their growth.

According to UNAIDS, modelling of the pandemic long terms impact on the HIV responses shows that there could be an estimated 123 000 to 293 000 additional new HIV infections and 69 000 to 148 000 additional AIDS-related deaths between 2020-2022.

The collective failure to invest sufficiently in comprehensive, rights based, people-centred HIV responses has come at a terrible price. Implementing just the most politically palatable programmes will not turn the tide against Covid-19 or end AIDS, said Winnie Byanyima, UNAIDS Executive Director.

May you live in interesting times. Today seems the epitome of this Chinese curse. Ours is the time of the COVID-19 pandemic. The world we live in is partially like the old days and partly an uncertain Brave New World.

There is no uniform experience. Life goes on pretty much as normal for some. For many, a challenging and difficult new normal has evolved. And, for a few, it comes to a catastrophic end. For the vast majority, the biggest impact has been the government-mandated restrictions.

Unfortunately, it is not even clear what the truth about the disease really is. Much of the public response is driven by the media and its need to fill the airways 24/7. There is nothing like a crisis to capture our attention (and viewers).

Online sources and social media fuel dire (and sometimes crackpot) opinions as well. Collectively they have turned the pandemic into a panic.

We are told to follow the science, but what does that mean? What precisely should we do in response to whatever component of science we follow?

For example, we know that COVID-19 is virulent. For at-risk populations, and sadly some healthy people, it is deadly. However, the same can be said about the annual flu outbreak, particularly in years where the vaccine misses the target. Life is always full of risk, and we should view this in the same manner we do every choice we make. I could get in a fatal car crash on the way to the grocery store, yet I accept that possible fate every day. This is no different; only the probabilities change. It is a question of determining the acceptable magnitude of risk, not eliminating it.

We must recognize that much of the science is based on probability and modeling. Early estimates stated that the overall mortality rate was approximately 3.4%; subsequently, that figure has been lowered to below 1% (across all groups). Those figures are misleading. The mortality rate for patients over 80 is more than 13% (that figure is staggering if understandable). However, it is far lower among healthy younger people, approximately 0.15% for those in their 30s.

These and other factors (impact of side effects, transmission mechanisms, etc.) fluctuate almost daily as we learn more about the pathology of the disease. On which statistics are we to base our policy? How do you state emphatically that one particular course of action is valid and efficacious for all groups and across all geography?

The short (and honest) answer is that you cant. The real question for us today is, How will we live our lives?

At one extreme, we can choose to be scared by the science (as the media would have us do). We can live in fear. We can shut down business and hope that massive federal expenditures will solve the problem. We can destroy the well-being of thousands of our citizens, leaving them without meaning in their lives. We can pretend that such actions will not come home to roost in the future. I believe those risks are every bit as relevant and potentially dangerous as denying the nature of the disease and pretending that there is no problem, a path that will inevitably lead to unnecessary death and tragedy.

Furthermore, following the rules creates no guarantee. In todays world, you can quarantine, mask up, wash your hands frequently, and still get infected. Likewise, you can live a healthy lifestyle, have no underlying conditions, and still become deathly ill.

But there are clear rational actions. We should continue to pursue vaccines and aggressively disseminate them when they are available (again accepting the risks involved with side effects). We should do a better job of protecting at-risk populations (e.g., provide full protective equipment for workers in nursing homes). We can also take prudent precautions (wearing a mask, doing extra cleaning and sterilizing, maintaining separation, etc.). These are not onerous and appear to have a positive impact. Why would we not do this?

However, I have a greater fear for the future if we shutter our society and crash the economy. The risk of massive dependence on government largess is real. We have seen the result of such conditions on the diverse underclass from minority urban neighborhoods to the former coal communities in Appalachia. Do we really want to expand that experience to consume others who were productive members of society, consigning them to the depressive effects of perpetual subservience to a dysfunctional government?

We are better than that. We do not retreat in the face of adversity. We do not lock our doors and whimper in the night as the tide of adversity swallows us up. We have never capitulated because the road ahead was hard, full of risk, or plagued with danger. We are willing to sacrifice for the greater good of our country.

This is what it means to be American at least that is who we used to be.

Cancer is a relatively common disease in the U.S. Nearly four in ten men and women will be diagnosed with cancer at some point during their lifetime, according to some estimates. Cancer is also on the rise among millennials, who make up the largest generation in the U.S. workforce. At the same time, the number of cancer survivors in the U.S. is increasing steadily. It is estimated that by 2040, the number of cancer survivors in the U.S. will reach 26.1 million, a more than 50% increase compared with 2019.

With a growing number of cancer survivors in the workforce, employers need to better understand and be equipped to help support them, given the challenges these employees may face, said Candice Sherman, CEO of NEBGH. Some people are unable to work due to treatment or the effects of treatment, but many are able to continue to work or to return to work after some period. In fact, most cancer survivors are eager to work because it provides a coping mechanism, enables a sense of purpose and motivation, boosts self-confidence, fosters social support and aids in financial stability.

The guide, Cancer Survivorship: Challenges and Opportunities for Employers, identifies six major categories of challenges that many cancer survivors face and how employers can best support them. These six categories include physical health, mental and social health, wellness and healthy lifestyle, family and friends, finances and work. For each category, the guide highlights the specific challenges survivors may face and provides action steps employers, and in some cases, physicians, can take to help address these challenges. The guide also includes a list of considerations for cancer survivor related issues in light of the current pandemic and other potential health crises in the future.

There are financial and productivity benefits for employers that include a decrease in turnover costs, which can be high. In addition, it can be costly to lose the expertise, contacts and customer relationships employees have built. Cancer survivors generally have higher annual medical costs than employees without cancer because of ongoing monitoring as well as the side effects of treatment or longer-term effects of the disease itself, but supporting survivors work and well-being enhances the value of these healthcare expenditures.

The United States health care spending is getting out of control. Even though more citizens have health insurance due to the Affordable Care Act, the premiums and deductibles are so costly that many people cannot afford to use the insurance they have. The inflated costs of pharmaceuticals, hospital care and medical equipment technology have made taking care of yourself almost impossible.

Heart disease is the leading cause of death and the second-leading cause of disability in the country. The U.S. is responsible for 28.4 percent of the global spending, about $30.7 billion per year. Over recent years, the number of Americans suffering from heart failure has grown. In addition, 50 percent of people diagnosed with heart failure will die within five years, making hospital readmissions for patients with heart failure a national focus, as hospitals now must deal with an economic penalty for someone who is readmitted within 30 days of discharge.

Cardiovascular disease is the leading cause of death in Connecticut. Many different problems can occur and often the result is a heart attack or stroke. The most common cause of heart attack and stroke is a buildup of fatty deposits on the inner walls of the blood vessels called atherosclerosis. In heart failure, the muscle may become damaged and weakened. This leads to a heart that cannot pump blood efficiently to the rest of the body. Over time, the heart cannot keep up with the normal demands from the body. Almost all diseases of the heart will ultimately result in the diagnosis of heart failure. Most common symptoms of heart failure are shortness of breath, fatigue, weakness and swelling in the legs. The key to preventing heart failure is to reduce risk factors, including unhealthy diets, physical inactivity and tobacco use.

If the average American adult could find a way to be active for 30 minutes, five days a week, the savings could be huge. Just a 5 percent decrease in spending due to heart failure would result in a $1.5 billion savings. But thats not all. Improving the health of Americans would lower risk of stroke, diabetes, hypertension, dementia and cancer, just to name a few. Most importantly, this increase in activity will improve peoples quality of life. The financial implications would be enormous.

The American Heart Association recommends 150 minutes per week of aerobic activity. But this doesnt necessarily mean going to the gym and pumping iron. Activity can be walking, swimming, dancing or even yard work. It also doesnt have to be 30 consecutive minutes of activity. It could be walking for 10 minutes three times a day. At first glance, it can seem like a lot. However, once you take a step back and think about your day-to-day activities, it becomes much more reasonable.

In 2017, heart disease was by far the leading cause of death in the state of Connecticut. Almost half of all deaths were attributed to heart disease. This is an ongoing problem that has no end in sight. The solution is not going to be easy. We need to see more lobbying for insurance companies to pay for preventative care such as gym memberships, fitness watches, exercise equipment, physical therapists and dieticians.

Its true that cost sharing, co-pays and deductibles are necessary to prevent overutilization of health care. It makes the insured more responsible for their health and well-being. But out-of-pocket costs have become increasingly difficult for people to afford. They are unlikely to make good decisions and pay for a healthy lifestyle if their budget is tight. Higher upfront costs to the insurer for these preventative measures will save the health system and insurance companies billions in the long run.
